The purpose of this study was to illustrate the actions of the gluteus maximus (GM), medial hamstrings (MH), and lateral hamstrings (LH) during forward-reaching in a sitting position using electromyography (EMG). We found that as the distance of forward-reaching increased, EMG activity of the GM, MH, and LH muscles increased to maintain forward-bending of the trunk. In addition, lower GM activation increased at a shorter reaching distance than the upper GM. From the analysis of posture during this task, we also found that the distance covered during forward-reaching was increased by up to 15 cm mainly through flexion of the thoracic spine. This mechanism enhances the distance of forward-reaching achieved by flexion of the hip joint, but extension of the lumbar spine. From these results, we suggest that forward reaching is not simply caused by flexion of the hip joint and that each fiber of GM, MH, and LH muscles has a different function in the sequence of activities that leads to a change in posture.

We initiated physical therapy for a female patient with Alzheimer’s disease who suffered a left trochanteric fracture that caused a short and unstable left stance phase in gait. Initially, we performed muscle stretching and strengthening, and weight-bearing exercises in the sitting and standing positions, but the patient could not follow the therapy and did not achieve the target strength because of attention deficit due to Alzheimer’s disease. However, the patient was capable of independent activities of daily living (ADLs) such as dressing and grooming. Therefore, we initiated an exercise therapy which involved putting on and taking off a sock. After the therapy was started, the strength of the patient’s left gluteus maximus and external and internal oblique muscles improved and the instability in her gait stance decreased. This experience shows the necessity of selecting a therapy focused not only on the impairments but also on the patient’s abilities to perform ADL.

Toilet activity, which is done every day attended with the physiological phenomenon of "excretion", has high necessity in the activities of daily living (ADL). We analyzed it in terms of "sense", and describe how to guide a person who has difficulty in this activity, with some specific approaches. It is important to carry out all of the activities of daily living not only in the rehabilitation room but also in a practical environment, and we need to take it into consideration in the physical therapy.

The purpose of this study was to investigate the electromyographic (EMG) activities of the multifidus, longissimus, and iliocostal muscles in the standing and forefoot standing positions in order to understand the relationship between each muscle activity and thoracolumbar extension in the forefoot standing position. This study recruited 10 healthy male volunteers (mean age: 28.4 ± 6.1 years). The EMG activities of the multifidus, longissimus, and iliocostal muscles were measured in the standing and forefoot standing positions. The values of the integrated EMG activities of each task were compared. Each muscle showed a significant increase in the value of the integrated EMG activities in the forefoot standing position (p<0.01). The activities of the iliocostal muscles were separated into two types: Group A, in which the EMG activities did not increase much, and Group B, in which the EMG activities clearly increased. In group A, the forefoot standing position involved extension of the hip joint, and lumbar alignment was almost identical to that in the standing position. In group B, lumbar lordosis and anteversion of the pelvis were significantly greater in the forefoot standing position than in the standing position.

In physical therapy for patients with central nervous system disorders, the goal is often the acquisition of walking movement. Physical therapy is required to always consider the risk of falls in hospitalized life and to judge the degree of independence for each individual. The authors believe that it is necessary not only to rely solely on the determination of gait independence, but also to supplement it by observing movement, which is a necessary clinical ability required of a physical therapist. In this paper, we focus on the movement of initiating gait and present approaches for improving lower limb dysfunction while deepening the understanding of normal movement.

We believe that it is important to determine impairments from motion observation in the process of pursuing the ADL problems of patients. Physical therapists are involved in a wide variety of illnesses, and there are many opportunities to confront the impairments caused by falls. Most of the injuries caused by falls are due to stumbling. Regarding motion observation, it is necessary to focus on pelvic tilt at the time just before toe-off on the swing leg side, and carefully examine which factor corresponds to it, and to look at the joint movement in detail. We think that it is important to consider this. Here, we look at the factors that cause stumbling from an examination of cases.

In this study, we investigated the muscle activity that causes spinal and pelvic movements and changes inspinal and pelvic alignment when performing forward reaching in the sitting position at 0.5 seconds, which issimilar to the speed of the flexion phase of standing up movements. We measured the muscle activities of thelongissimus, multifidus and ilicostalis muscles, and the angular changes of the spinal column and hip joint of20 healthy men performing anterior reach motion in the sitting position. The results show that the activity ofall muscles weakened at the time of 20% from the start of movement and tended to increase significantly after40%. In addition, the angle change of the spinal column and hip joint showed flexion of the lower thoracicspine from the time of 20% to 40%, and after 40%, the anterior reach distance increased significantly as theflexion angle of the hip joint increased. The results of this study show that there is a difference in the purposeof exercise between the phase up to 40% of the time from the movement start and the phase that follows.

本研究は,運動観察時に提示される運動の筋収縮強度に対する主観的認識の違いが脊髄運動神経機能の興奮性に及ぼす影響を明らかにすることを目的とした。対象者は健常成人22 名とし,F 波は右母指球上の筋群より導出した。測定の流れは,安静時のF 波を1 分間測定し,4 分間の休息後,運動観察時のF 波を1 分間測定した。観察課題は,母指の内転・外転運動(無負荷映像)と2 種類のセラバンドで母指に負荷を与える(低負荷映像・高負荷映像)とした。また,対象者の認識に基づいて,主観的高負荷,主観的低負荷,主観的無負荷に分類し,各条件間の振幅F/M 比相対値を比較した。振幅F/M 比相対値は,主観的無負荷・低負荷条件と比較し主観的高負荷条件で増加した(p <0.05)。結果より,運動観察において対象者に強い筋収縮が必要な運動であると認識させることで脊髄運動神経機能の興奮性は増加することが示唆された。

Visual observation and electromyographic evaluation of the movement from the sitting position to the supine position were performed. In the movement from the sitting position to the side-lying position, the forearm of the upper limb supports the lateral bending of the trunk after elevation of the pelvis. The pelvis is elevated by the muscle activity of the oblique muscle on the opposite side, which is important.

To deepen understanding of the normal for sitting from supine, 10 men and women were examined using motion analysis and electromyography. Motion was classified into three categories, namely sitting from supine with arm support on one side, sitting from supine with arm support on both sides, and sitting from supine with the sitting position with extended legs. The muscle activities of the right and left rectus abdominis muscles and right and left abdominal oblique muscle overlapping sites were examined using electromyography. A common feature of the three motions was the absence of rotation or side bending of the trunk. In addition, as segmental rotation and lateral bending of the trunk do not occur, the activities of the upper limbs are suggested to be important with regard to raising and rotating the body.

This study examined and compared the pelvic tilt angle and movement characteristics of a forward-reaching task at different speeds in the sitting position. Seven healthy males each performed a forward-reaching movement toward a target 20 cm ahead within periods of 2 seconds, 1 second, and 0.5 second. It was found that the thoracic vertebra tended to bend immediately after the initiation of movement in all the tasks. Additionally, in the 2-second and 1-second tasks, the pelvis showed a change in angle, tilting forward at initiation of the movement. In the 0.5-second task, however, the pelvis exhibited a slight change in angle, tilting backward to maintain posture at the initiation of movement.

We performed physical therapy for a patient who had great difficulty with walking due to pain-defense contraction of the right knee joint after total knee arthroplasty (TKA). This patient had much trouble restoring knee range of motion (ROM), even after TKA, because of a long period of suffering from pain in the right hip joint which limited the ROM. When walking, he could hardly move his knee joint and always showed knee flexion; his right hip joint was bent and his trunk leaned forward. He could not perform internal rotation of the right hip in the right stance phase. Instead, he elevated the left scapula and the left side hip joint and executed external rotation of the right side hip joint. We thought these problems resulted from limitation of ROM of the right knee joint, and we prescribed some exercises for ROM. Subsequently, ROM of the right knee joint and the joint position sense improved a little, but the patient still bent his right knee joint and showed no improvement of walking. After rethinking these problems, we concluded that we should provide therapy considering his habits before TKA. We set the bed in front of his trunk after he leaned against the bed because of relief of muscle hypertonia and position sense of knee joint. We asked the patient to lean against the bed to try and lengthen his hamstring. Subsequently, he showed less, compensatory motion of the trunk and pelvis, and internal rotation of the right hip joint gradually became possible, easing his walking difficulty. 【はじめに,目的】着地動作におけるpreactivationは,着地の事前に生じる筋活動で,下肢の関節安定化を担う作用があると言われている。膝前十字靭帯損傷患者では,しばしば着地直後からknee-in&toe-outなどの膝関節不安定性に起因する動的アラインメントの変化が起こる。これらの患者では,筋力低下や関節可動域制限の影響も受けて,安静時の下腿回旋アラインメントの異常を呈することが多く,着地後の動的アラインメント異常を誘発する要因にもなる。着地動作に関しては,着地後の筋活動変化や下肢各関節角度変化の影響について検討されたものが多く,着地動作のpreactivationと下腿回旋アラインメントの関係を検討されたものはない。この関係を検討することで,着地動作における動的アラインメントについて筋電図学的に考察することができると考える。そこで本研究では,筋力低下や関節可動域制限などの機能障害を有さない健常者を対象に,テーピングにより人為的な下腿回旋アラインメント変化が,着地動作における膝関節周囲筋のpreactivationに及ぼす影響を明らかにすることを目的とした。【方法】対象は神経外科学的,整形外科学的な既往を有さない健常男性12名(平均年齢25.4±4.2歳,身長171.6±4.3cm,体重61.0±6.2kg)とした。また,ニトリート社製のテープEB50mmを用いて(このテープを貼付するためのアンカーテープにCB38mmも使用)対象者の左下腿を膝関節20°屈曲位で可及的最大の内旋位および外旋位になるように固定した。このときの下腿回旋角度は,上前腸骨棘と膝蓋骨中央を結んだ線と膝蓋腱のなす角度とした。また,上記のアラインメントを保持する際,テープの貼付によって膝関節伸展制限が生じるため,対象者の下腿回旋アラインメントを中間位に保ったまま膝関節伸展を制限するテープの貼付も行った。これらのテープの貼付はランダムに実施した。その後,対象者には上肢の影響を除くため両上肢を胸の前で組ませ45cm台からの左片脚着地動作を十分に練習させ3回実施した。着地動作はメトロノームに合わせて1秒間で実施した。この時,対象者の左内側広筋,左外側広筋,左内側ハムストリングス,左外側ハムストリングよりテレメトリー筋電計MQ-air(キッセイコムテック社製)を用い双極導出法にて筋電図を記録した。そして,重心動揺計(ユニメック社製)からの信号も筋電図と同期収録し,それをもとに接地の100ms前の筋電図積分値を算出し,本研究でのpreactivationとした。また,左側の上前腸骨棘,大転子,膝関節外側裂隙,外果,第5中足骨頭の計5箇所にマーカーを貼付し,側方からデジタルビデオカメラを用い動作を60Hzのサンプリングで収録し,解析ソフト(imageJ)を用いて股・膝・足関節の矢状面角度を算出した。中間位の各筋の筋電図積分値を1とした相対値を求め,内旋位・外旋位の各筋の相対値について検討した。そして各筋の相対値について正規性の検定を行い,正規性を認めたことから内旋位・外旋位のデータを対応のあるt検定により比較した。有意水準は5%とした。【結果】下腿の平均回旋角度は中間位16.1°±1.2に対し,内旋位12.7°±1.4,外旋位19.4°±1.4となった。また各実験後において,テーピングの緩みによる下腿回旋角度変化は生じなかった。preactivation時の矢状面上における下肢各関節の角度変化も認めなかった。筋電図積分値相対値は,外旋位に対して内旋位での内側・外側ハムストリングスに有意な増加を認めた(p<0.05)。【考察】本結果では下腿を内旋位にした際,内側・外側ハムストリングスの筋活動に有意な増加を認めた。Andrewsらは下腿を内旋位にすることで,脛骨前方引き出しなど膝前十字靭帯の緊張を介した筋活動がハムストリングスにみられると報告している。また,浦辺らは着地初期では下腿は内旋し,直後から下腿が外旋運動に変わると述べている。したがって,今回観察された下腿内旋位でのpreactivationにより,着地動作において外側ハムストリングスが着地初期の下腿内旋の制動に,また内側ハムストリングスがその後の下腿外旋の制動に作用しやすくなり,着地後の動的アラインメント変化を軽減させることができる可能性も考えられた。【理学療法学研究としての意義】本研究では45cm台からの着地動作であるが,下腿回旋アラインメント変化によって着地動作のpreactivationに相違を生じることが明確になった。本結果より下腿回旋アラインメントの調整は着地動作においても関節安定化に貢献できる可能性が示唆された。

We were involved in the physiotherapy of a patient with cerebrovascular disorder. The patient had repeated aspiration pneumonia. The trunk takes a bent position in a patient with repeated aspiration pneumonia, and the neck leans back. We performed physiotherapy for the facial expression and mastication muscles. As a result, the position of the patient's neck and trunk improved. Furthermore, the muscle tone of the trunk muscles was promoted by our treatment of the facial expression and mastication muscles, allowing the cervical vertebrae to rotate. In this article, we report the influence of the trunk on induction of the facial expression and mastication muscles and lordosis of the cervical vertebrae.
