Objective: There have been many previous reports about the relationships between jaw deformity and articulation disorders, but most of such studies were conducted in cases with mandibular prognathism. In contrast, well-defined studies have not been conducted to date in patients with retrognathia although it is considered that such patients have articulation disorders because of the relationship with tegmentum in the anterior tooth area and association with abnormality of the perioral muscles. We previously reported on the effects of orthognathic surgery on vowel sounds in patients with retrognathia. The purpose of this study was to investigate the influence of orthognathic surgery on poor articulation of Japanese consonants in cases with retrognathia. Materials and Methods: The eight consonant sounds [ka], [ki], [sa], [ɕi], [ta], [tɕi], [pa], [pi] were recorded as speech samples. The investigation was carried out in 11 Japanese female patients with mandibular retrognathia requiring surgery for jaw deformity and malocclusion both at the preoperative and postoperative stages of 3 and 6 months. These subjects were compared with 10 healthy controls. The duration of the consonant sound and the sound pressure level were analyzed. Results: The duration of the consonant sounds [ka], [ki], [ɕi] and [tɕi] was significantly longer in patients with mandibular retrognathia compared with the controls. The duration of the consonant sounds except [pa] and [pi] after the operation was generally shorter than before the operation. Especially, significant shortening was observed in [ki] in the comparison between before and 6 months after the operation. The sound pressure level for [ki] and [tɕi] was significantly smaller. No significant differences were found in the sound pressure level of the consonant sounds between before and after the operation, but the level was getting larger by 6 months after the operation. Conclusion: In patients with mandibular retrognathia, the duration of consonants was extended especially in the alveolar fricative sound, velar plosive, and alveolar affricate; furthermore, the sound pressure level was low. However, the articulation disorders tended to be reduced at 6 months after orthognathic surgery by correcting the occlusion and jaw position.

Objective: We measured the formant frequency of vowels in retrusion patients and normal healthy control patients, and made a comparative study of the purpose in phonology. We wanted to know how orthognathic surgery has influenced the articulation of vowels in patients. We investigated how the relationships between the position of the maxilla and mandible and incisor occlusion affected vowel sounds. And because resonance space is believed to affect sound frequencies, we investigated the effect of the width of the airway preoperatively and postoperatively on vowel sounds.Methods: Five vowel sounds in 10 Japanese patients with mandibular retrusion requiring surgery for jaw deformity and malocclusion preoperatively and at 3 and 6 months postoperatively were recorded and compared with those in normal healthy control patients. Also, the effect of the positional relationship between the maxilla and the mandible and incisor occlusion on these vowel sounds was investigated by correlating skeletal and denture patterns of cephalometric measurements taken preoperatively and at 3 months postoperatively with these vowel sounds. The width of the airway was measured preoperatively and 3 months after the operation, and the correlation with the frequency of the vowels was examined.Results: In /e/ F1, /o/ F2-F1, significant differences were observed between the preoperative patients and the control patients. However, these differences disappeared at 3 and 6 months postoperatively.A significant difference was admitted with F1 of /a/ and F1 of /e/ in comparing the preoperative with the postoperative patients 6 months by a rank test with a sign of Wilcoxon.Both preoperatively and postoperatively, positive correlations were observed between F2-F1 the narrow, open vowels /i/ with SNA and SNB. In the preoperative patients, some correlations were observed between F1 the semi-narrow, open vowel /e/ and denture patterns.Significant increases were observed between the preoperative and 3 months postoperative patients in the width of the hypopharyngeal cavity. Many correlations were observed between the change of width of the airway and the vowel frequency, particularly in the posterior tongue vowel /u/. But other vowels had few correlations.Conclusion: A change of occlusion of the front teeth and a change of frame by the mandible correction operation greatly influenced close vowels and half close vowels that were a front tongue vowel, especially the influence of /i/ and /e/ in retrusion patients. It was an interior tongue vowel that the change in the airway influenced the vowel frequency of only /u/.

This study investigated the relation between personality characteristics and types of jaw deformities and evaluated the influence of orthognathic surgery on personality characteristics.The subjects consisted of 52 patients (16 males and 36 females) in whom jaw deformities were surgically corrected and the mean age at surgery was 26.3 years. They were divided into four groups based on the types of jaw deformities such as mandibular protrusion with or without maxillary retrusion, mandibular retrusion with or without maxillary protrusion, open bite and asymmetry.Personality characteristics were analyzed using Minnesota Multiple Personality Inventory (MMPI) and Rosenberg's Self-Esteem Scale before and six months after surgery, and the Kruskal Wallis test and t-test were used for statistical analyses.Preoperative MMPI scores were not significantly different from normal values of their generation as a whole, but the distribution of Depression Scale and Self-Esteem Scale scores among the four groups were significantly different. The Depression Scale score in the asymmetry group was higher than those in the other groups and the Self-Esteem Scale score in the asymmetry group was lower than those in the other groups. After surgery, the Depression Score decreased in the asymmetry group and the Self-Esteem Scale score increased in all groups except the open bite group.In conclusion, it is considered that patients with asymmetry are likely to have an inferiority complex and orthognathic surgery for such patients has a favorable effect on personality characteristics because the deformity and the change of full-face appearance are easily recognized by themselves.

顎間骨が位置異常を示す両側唇顎口蓋裂術後患者に対して,唇側からの顎間骨の骨切りと顎裂部の骨移植を同時に行う顎間骨整位術の有用性を本学会雑誌の21巻2号に報告した。しかし,この到達法は,顎間骨部への血行の考慮と骨切り操作の点において検討の余地があることが判明した。そこで,1996年4月から2009年3月までの13年間に,同様の症状を呈した17名の両側唇顎(口蓋)裂術後患者に対し,口蓋側から顎間骨への到達と骨切りを行なったところ,以下の結論を得た。<br>1.全症例とも本到達法と骨切りは安全かつ容易に行えた。<br>2.切歯歯根尖より充分距離をおいた顎間骨の骨切りが可能であった。<br>3.顎間骨移動時の骨干渉部の削合も頭部の後屈で容易に行えた。<br>4.以上のことから,顎間骨整位術を施行する場合,顎間骨部への到達と骨切りは口蓋側から行うべきであることを推奨する。
