This review summarizes new findings related to beneficial effects of melatonin (N-acetyl-5-methoxytryptamine) on reproductive physiology. Recently many researchers have begun to study the local role of melatonin as an antioxidant. We focused on intra-follicular role of melatonin in the ovary. Melatonin, secreted by the pineal gland, is taken up into the follicular fluid from the blood. Reactive oxygen species (ROS) are produced within the follicles, during the ovulatory process. Melatonin reduces oxidative stress as an antioxidant, and contribute to oocyte maturation, embryo development and luteinization of granulosa cells. Our clinical study demonstrated that melatonin treatment for infertile women increases intra-follicular melatonin concentrations, reduces intra-follicular oxidative damage, and elevates fertilization and pregnancy rates. Melatonin treatment also improves progesterone production by corpus luteum in infertile women with luteal phase defect. Melatonin treatment could become a new cure for improving oocyte quality and luteal function in infertile women.

Aim: Current Japanese guidelines state the target level of low-density lipoprotein cholesterol (LDL-C) of <100mg/dL for secondary prevention of coronary artery disease (CAD). However, this level was set considering the results of trials mainly conducted in Western countries. In addition, the effect of achieving target LDL-C on secondary prevention is unknown. Methods: We examined the effects of achieving target LDL-C on clinical outcomes. Patients who underwent percutaneous coronary intervention at Juntendo University Hospital (Tokyo, Japan) from 2004 to 2010 and received follow-up coronary angiography (CAG) were analyzed. The study population was divided into two groups based on the follow-up LDL-C. The incidence of major adverse cardiovascular events within 3 years after the follow-up CAG was examined. Results: A total of 1321 consecutive patients were enrolled. Sixty-three percent of the patients achieved the target LDL-C. The rate of 3-year events was lower in the group that achieved the target LDL-C (achieved group). The adjusted relative risk reduction in the achieved group was 26% (p=0.02). In the sub-analysis among the four groups stratified by baseline LDL-C of 140 and follow-up LDL-C of 100, the adjusted hazard ratio for 3-year events was 1.84 (95% confidence interval; 1.10-3.24)in Group 3 (baseline <140, follow-up ≥100) and 2.05 (1.18-3.74) Group 4 (baseline ≥140, follow-up ≥100) [Group 2 (baseline ≥140, follow-up <100) as reference]. Conclusions: Our data suggested that follow-up LDL-C <100mg/dL was appropriate for secondary prevention of CAD in Japanese population.

The Limulus amoebocyte lysate (LAL) test is the most sensitive and reliable assay for the detection of trace amounts of bacterial endotoxins (lipopolysaccharides, or LPS), and is an accepted in vitro alternative to the rabbit pyrogen test for evaluations of parenteral drugs, biological products, and medical devices. There are three principal LAL tests, which can be categorized as both semi-quantitative and quantitative methods, including gel-clot, turbidimetric, and chromogenic assays. Since the 1970s, these tests have been successfully formulated and commercialized by US and Japanese manufacturers. More recently, in addition to the recombinant factor C-based assay, a novel product containing all of the recombinant coagulation factors from horseshoe crab has been developed, which may lead to the creation of a next generation LAL alternative. Furthermore, there are antimicrobial peptides called “host defense peptides (HDPs)” that play a key role in innate immune responses. The LAL test for HDP-related studies is challenging, because the active site of endotoxin could be masked by the binding with HDPs. Thus, it is very important to properly evaluate the actions of HDPs (human defensins and cathelicidin peptide LL-37) such as the neutralization of LPS, immunostimulatory functions, and anti-endotoxin activity. Moreover, sensitive detection of LPS in cell culture media should be conducted to address the problem of endotoxin contamination in the media. Here, we discuss the progress of LAL-based endotoxin assay technologies, as well as their applications and limitations, with a focus on innovative functional studies of HDPs.
