Pharmaceutical consultation targeting outpatients at the Fujita Health University Hospital (Japan) provides support to patients undergoing anticancer drug treatment. This study aimed to explore factors that affect the comprehension of cancer chemotherapy among outpatients who received cancer treatment at our hospital. A questionnaire survey was conducted, and comprehension was scored on a scale of 1–5 (1, no comprehension; 5, full comprehension). When factors other than age and sex [the influence of which on comprehension has been reported in previous reports] were noted, differences in comprehension between the questionnaire items were comparatively analyzed according to the presence/absence of the relevant factors. Overall, 536 patients were included. Age (<70 years) and pharmacist interventions were identified as factors contributing to a comprehension score. The levels of comprehension regarding the name of the cancer chemotherapy, content/schedule of the treatment, purposes of the prescribed drugs, and objectives of blood tests were significantly higher in the group that received the pharmaceutical interventions; conversely, the level of comprehension for the self-management of adverse events was significantly lower in this group than in the group that did not receive any pharmaceutical interventions. Age and interventions by the pharmacist affected the comprehension of cancer chemotherapy by patients.

Physiological and psychological effects of rose aromas have been reported. Many of these reports focused on the effect of rose essential oils, but the effect of rose fresh flowers on humans has not been sufficiently reported. We were also interested in the possibility of using rose hydrosol, a byproduct of the rose essential oil manufacturing process, for aromatherapy. In this study, the physiological and psychological effects of rose ‘Wishing’ fresh flowers and their hydrosols on humans were evaluated. R–R power spectral analysis of heart beats revealed the sedative effect of the fresh flowers’ scent. On the other hand, hydrosols did not show such an effect. Gas chromatography (GC) and GC-mass spectrometry (GC-MS) analysis indicated the possibility that the sedative effect of the fresh flowers’ scent was derived from β-caryophyllene, phenylethyl acetate, and 3,5-dimethoxy toluene. It is possible that fresh rose flowers may be an alternative to rose essential oils in aromatherapy.

Background: To avoid lateral misplacement of midcervical pedicle screws, we developed a method for Minimally Invasive Cervical Pedicle Screw (MICEPS) fixation via a posterolateral approach. This intramuscular approach allows for horizontal pedicle screw insertion and reduced intraoperative bleeding. We reviewed our initial experience with MICEPS fixation for patients with cervical metastases.Methods: This study included 18 consecutive patients who received cervical spinal surgery for metastatic tumor. We treated 12 patients with conventional cervical pedicle screw fixation, and 6 patients with the MICEPS fixation technique. Average follow-up was 14 months (range 3 to 34). We inserted 117 pedicle screws using the navigation system. Average fusion area was 4.9 vertebrae (range 3 to 8). Alpha-angles between a line perpendicular to the posterior cortex of the vertebral body and the screw trajectory in the transverse plane were also measured.Results: The average surgical time was 250 min (range 151 to 420 min) with the conventional pedicle screw fixation and 234 min (range 154 to 300 min) with the MICEPS fixation. The average total blood loss was 780 mL (range, 180-1430 mL) in the conventional pedicle screw fixation group and 180 mL (range, 70-400 mL) in the MICEPS fixation group. At the level of midcervical (C3-5), average alpha-angles was 52 degrees (range 43 to 62) in MICEPS fixation group, and 39 degrees (range 19 to 55) in conventional cervical pedicle screw group.Conclusions: The MICEPS fixation technique uses an intramuscular approach, which is minimally invasive and reduces intraoperative bleeding. Intramuscular approach allows for horizontal pedicle screw insertion, and reduced critical screw deviation.

Morphine, oxycodone, and fentanyl are commonly used to control cancer pain. Because these drugs have differences in receptor affinity or pharmacokinetic parameters, changing the opioid formulation may result in an unexpected outcome, depending on the patient’s condition. This study investigated whether low serum protein levels influence the effectiveness of opioid rotation by determining the impact of serum albumin levels on the analgesic effect before and after opioid rotation from morphine or oxycodone to fentanyl in cancer patients. The patients were classified into 3 groups according to their serum albumin levels before opioid rotation: group 1, <2.5 g/dL; group 2, from 2.5 g/dL to <3.0 g/dL; and group 3, ≥3.0 g/dL. There was no significant change in the percentage of patients with good pain control after rotation in group 1 or group 2; however, the percentage of patients with good pain control increased significantly in group 3. When the percentage of patients whose numerical rating scale scores increased, were unchanged, or decreased after rotation were compared, a significant difference in the percentage of those showing improvement was noted among the 3 groups and between groups 1 and 3. These findings suggest that monitoring serum albumin levels during fentanyl therapy is useful for pain management, and that the effectiveness of opioid rotation to fentanyl in patients with serum albumin levels of <2.5 g/dL should be carefully evaluated after rotation.

This technology is concerned with a new type of extruded polystyrene foam (XPS), which gives high performance of thermal insulation without containing any fluorocarbons as a blowing agent. Avoiding such typically harmful materials will help solve global environmental problems like ozone layer depletion and global warming. Kaneka Corporation developed this technology for the first time in the world. As a blowing agent of this new XPS in place of fluorocarbons we adopted isobutane, an excellent green gas that helps prevent ozone layer depletion and global warming. However, there is a problem: it is difficult to secure high performance of thermal insulation and flame retardation when isobutane is used. This originates from the fact that isobutane has a higher thermal conductivity than fluorocarbons, and is highly flammable. To overcome the former problem, we found a novel cell structure to secure high performance of thermal insulation, and we developed new technologies to control it. To solve the latter problem, namely securing flame retardation, we clarified the mechanism to suppress combustion of isobutane, and developed a new flame retardation technology by the addition of specific compounds. As a result of a combination of these technologies, we were able to develop and commercialize a new XPS with high performance of thermal insulation. These technologies directly contribute to prevent adverse effects on the global environment such as ozone layer depletion and global warming, by not using fluorocarbons. And, in addition, these technologies indirectly but more greatly contribute to global warming prevention by the reduction in carbon dioxide emissions through a high effect of energy conservation as a thermal insulator. Therefore, the thermal insulator with these technologies is valuable for environmental protection. And as the needs for a material for better energy conservation will increase, the contribution of this technology will expand much more in the future.
