Tributyltin (TBT) is an organotin compound that disrupts the endocrine system of aquatic organisms, and its obesogenic toxicity to various species is well known. However, the mechanism by which TBT disrupts the endocrine system has not been clarified. Therefore, to investigate the effects of TBT in fish, we exposed juvenile medaka (Oryzias latipes) to TBT and analyzed the gene expression changes using mRNA-Seq. As a result of this analysis, it was clear that toxicity-related genes, such as cytochrome P450 superfamily genes connected to hormonal metabolism, and peroxisomeproliferator-activated receptor signaling pathway genes related to obesity, were significantly affected by TBT. Thus, our mRNA-Seq results identified candidate genes for involvement in the mechanisms of TBT toxicity in TBT-exposed medaka. mRNA-Seq could be a strong tool to investigate and further understand the toxic effects caused by pollutants.

This study aims to evaluate the academic activities of female neurosurgeons at all branch meetings of the Japan Neurosurgical Society and identify related issues they encountered. The programs of all seven branch meetings of the Japan Neurosurgical Society (Hokkaido, Tohoku, Kanto, Chubu, Kinki, Chugoku/Shikoku, and Kyushu) were used to determine the number of presentations and chairpersons by sex. The covered period was from January 2008 to December 2020, which was available for viewing during the survey. Of note, only the Kinki branch used data from January 2008 to December 2019. The Neurologia Medico-chirurgica (NMC), the journal of the Japan Neurosurgical Society, was also reviewed to identify publication achievements during the same period. In all seven branches, the percentage of presentations given by female physicians increased from 7.9% in 2008 to 9.6% in 2020 (p < 0.05).Conversely, the percentage of female chairpersons in all branch meetings did not change over time and it was significantly lower (1.1%) than that of female presenters (7.9%) for all branch meetings combined in over 13 years (p < 0.01). In the NMC, the number of articles with female physicians as first authors did not increase or decrease over the years. We conclude that efforts to smoothly promote female neurosurgeons as chairpersons and increase the number of female first authors are necessary to facilitate their academic activities.

Purpose: This study aimed to develop a simple screening test for mastication, “the Sakiika (squid jerky) transport test (STT), which evaluates the vertical jaw movement coordinated with the lateral tongue movement during stage I transport, and investigate the possibility of its clinical application. Methods: The study included 73 people with dysphagia (mean age, 78.5 ± 7.8 years; median age, 79.0 years; interquartile range, 75.0–84.0). The STT evaluated the ability of a participant to transport a piece of squid jerky placed on the midline of the tongue to the molar region. The STT score was defined as the number of vertical jaw movements occurring as the tongue transported food to the molars. A cutoff value was set by comparing the STT scores with masticatory function evaluated via a videofluoroscopic swallowing study and with food texture evaluated using the Food Intake LEVEL Scale (FILS). Results: The STT scores counted by the two examiners had a κ coefficient of 0.79, indicating good reliability. The STT score was significantly associated with both the presence of masticatory movement ( p = 0.019) and food texture classified by FILS ( p = 0.032) at cutoff value of “3” (3 vertical movements). The STT showed 62% sensitivity and 75% specificity for masticatory movements. Conclusion: The STT could be a useful screening test to assess the presence or absence of food transportation to the molars for mastication in older patients with dysphagia. In addition, the STT could be useful in identifying the need to modify food texture to meet functions.

本稿は、用言のテ形(書いて、白くて、静かで、など)で接続されている複文を、従属節の主節に対する依存の程度(従属度)の違いによって分類することを目的とした。その従属度を測定する基準の一つとして、「主節末のモダリティや否定辞のスコープによる複文の構造」という統語的側面 を考察した。これらのスコープを観察することで、複文は、主節末のスコープが主節命題と従属節命題まで及ぶ構造(α構造)と、主節末のスコープが主節の命題のみにしか及ばない構造(β構造)に分けられた。本稿では、このα・βの構造の違いが、従属度を反映し、分類の統語的基準になると考えた。また、従属度を測定するもう一つの基準として、「節間の関係的意味を成立させる要素」という意味的な側面 を考えた。この要素は、(1)従属節の、複文全体における命題形成の機能、(2)主節・従属節間の論理関係、(3)主節・従属節間の時間・順序関係、(4)主節・従属節の述語の主語の異同、の四つである。本稿では、これらの要素が相互に関連しながら緊密に節同士が関係しあって複文を構成するものを従属度の高いテ形、これらの要素間にあまり関係がなく、節間の緊密な関係もみられないものを従属度の低いテ形、とした。この二つの基準から、テ形節は、テ1(付帯状況)、テ2(継起的動作)、テ3(原因・理由)、テ4(並列)、テ5(発言のモダリティ成分)に分類され、テ1からテ5の順番で、従属度が低くなっていくことを述べた。

Low- and middle-income countries (LMICs) face a critical shortage of basic surgical services. Adequate neurosurgical services can have a far-reaching positive impact on society’s health care and, consequently, the economic development in LMICs. Yet surgery, and specifically neurosurgery has been a long neglected sector of global health. This article reviews the current efforts to enhance neurosurgery education in LMICs and outlines ongoing approaches for improvement. In addition, we introduce the concept of a sustainable and cost-effective model to enhance neurosurgical resources in LMICs and describe the process and methods of online curriculum development.
