When pharmacists provide drug guidance to in-patients,they are often asked about health foods such patients are regularly taking in expectation of anti-cancer effects.However,pharmacists cannot always answer these questions based on scientific evidence.<br>To further evaluate the efficacy of health foods for this purpose,we did a survey of the literature concerning 5 frequently used health foods said to have anticancer effects using the PubMed and Ichushi search services,obtaining 1,300 papers from the former and 1,142 papers from the latter.However among them,we could not find any providing data from randomized controlled trials and thus there was no clear scientific evidence,though some of the papers noted an improvement in the quality of life of patients.<br>In conclusion,when pharmacists provide guidance on health foods to patients and their families,it is important for them to collect detailed information and evaluate them on a scientific basis.

We established a reporting system for the use of antimicrobial agents for methicillin resistant Staphylococcus aureus (MRSA) infections in January 2003 to help ensure they are used properly.Then,in October 2005,we set up an infection control team (ICT) to monitor the use of four agents for MRSA infections : vancomycin,teicoplanin,arbekacin and linezolid.Changes in the antimicrobial use densities of the agents,the number of patients receiving them and the therapeutic drug monitoring (TDM) execution rate were investigated for three periods : I : before the reporting system (2002),II : while reporting system was in use (2004),III : during ICT monitoring (2006).<br>There was no significant change in the antimicrobial use densities of the agents for MRSA infection,while the number of patients receiving them decreased significantly in period III (p<0.05).Also in period III,the antimicrobial use density of teicoplanin increased significantly (p<0.05),there was a marked decrease in the number of patients receiving vancomycin (p<0.01)and the TDM execution rate increased significantly (p<0.01).<br>The systematic activities performed by the ICT were effective in ensuring the proper use of antimicrobial agents for MRSA infection.Further,by enabling us to share information with other medical staff and conduct rapid interventions,they have been playing an important role in infection control in our hospital.

It is an important task of the pharmacist to triage appropriate over-the-counter (OTC) drugs for patients to achieve efficacy and safety.In this study,we investigated the present situation regarding the triage of OTC drugs for patients based on pharmacists interviewing patients in a fact-finding survey which determined how pharmacists in a drugstore communicate with patients during such interviews.<br>We analyzed patterns of pharmacist communication with patients for the purpose of triaging OTC drugs in the drugstore and compared them with typical recommended communication patterns in the United States.In the drugstore,there were 3 patterns of initiating dialog with patients purchasing OTC drugs which were (1)&ldquo;What are your symptoms?&rdquo;2)&ldquo;What is the name of the drug that you want?&rdquo;and (3)&ldquo;What disease do you have?&rdquo; <br>Using the first pattern we conducted a role play simulating the triaging of OTC drugs by pharmacists actually working in drug stores to standardized patients.In response to the patient&rsquo;s description of his or her current symptoms,pharmacists used various speech patterns to explain the suitability of drugs selected and the cautions in use.In interviews conducted by inexperienced pharmacists,time was wasted in communication and there was no logical flow in them.Afterwards,some pharmacists said that they had not been able to obtain enough information from patients for triaging OTC drugs and others thought it was necessary to set algorithms for this purpose.These opinions suggest that pharmacy education in Japan should include training that makes pharmacists realize their responsibility in triaging OTC drugs for patients.

We investigated the relationship between the serum vancomycin concentration and the initial dosage regimen. Furthermore, we evaluated the dose and dosage interval for the therapeutic level (trough< 10μg/mL, peak 25-40μg/mL) and the toxic level (trough>15μg/mL).<BR>The percentage of the concentration within the therapeutic level was 0% and the percentage over the toxic level was 71% regarding a vancomycin dose of 500mg every 6 hours. The percentage of the concentration within the therapeutic level was 0% and the percentage over a toxic level was 23% regarding a dose of 500 mg every 12 hours. The percentage of the concentration within therapeutic level was 0% and the percentage over the toxic level was 33% regarding a dose of 1, 000mg every 12 hours. The percentage of the concentration within the therapeutic level was 21% and the percentage over the toxic level was 11% regarding a dose of 1, 000mg every 24 hours. The percentage of the concentration within the therapeutic level was 33% and the percentage over the toxic level was 10% regarding a dose of 1, 500mg every 24 hours. As a result, a dose of 1, 500mg every 24 hours was found to achieve the most appropriate therapeutic levels. On the other hand, a dose of 500mg every 6 hours showed a poor percentage of the concentration over the toxic level.
