膵癌早期診断研究会が主導して行った,早期診断された膵癌の実態調査について報告する.40例のStage 0膵癌と119例のStage I膵癌が集積された.膵癌全体に占めるStage 0膵癌とStage I膵癌の割合は約2%であり,Stage 0膵癌は0.6%であった.症状を認めたために医療機関を受診した症例は38例(23.9%)と少なかったのに対して,検診にて異常を指摘され受診した症例は27例(17.0%),他疾患の経過観察中に異常を指摘された症例は85例(53.5%)と無症状で医療機関を受診した症例が多かった.検診にて異常を指摘された27例中,膵管拡張を指摘された症例が19例と画像における副所見の指摘から精査を行った症例が多かった.術前の病理診断では,超音波内視鏡下穿刺吸引法を用いた症例(30.8%)と比較して,内視鏡的逆行性胆管膵管造影下にて病理検体を採取した症例(77.8%)が多かった.予後は良好であったが,14.5%の症例で術後の残膵に膵癌が新たに発生した.今回の調査が,膵癌の早期発見ならびに予後改善に寄与をすることが期待される.

今回,我々は腸管子宮内膜症に対し,腹腔鏡手術を施行した5例を経験したので,報告する.症例は20~40歳代の未産婦の女性で,4例は繰り返す腹痛と腸閉塞症状の検査により腸管子宮内膜症が疑われ,1例は原因不明の消化管穿孔の診断で,腹腔鏡補助下に腸管切除が行われた.3例は回腸に,2例は直腸に狭窄病変を認めた.病理組織学的検査より,全例で腸管子宮内膜症と診断された.いずれも術後経過は良好で退院し,現在のところ再発の所見は認められていない.低侵襲性に優れた腹腔鏡手術は,腸管子宮内膜症に対し術後の早期回復と早期退院に有用で,拡大視効果により子宮・付属器が温存でき安全に手術を行える可能性が示唆された.

This paper presents an investigation of the thermoelectric performance of DLC films deposited on glass substrates using RF plasma CVD method. The respective thermoelectric performances of Si-doped DLC film and non-Si doped DLC film were evaluated and compared. The DLC films showed a Seebeck effect, and they had p-type semiconductor characteristics.The values of DLC films’ Seebeck coefficients were 1/5 - 1/100 compared to those of the conventional thermoelectric materials. At temperatures of 80-200°C, the Seebeck coefficients of Si-doped DLC and non-doped DLC were almost identical. The resistivity value of DLC films decreased exponentially with increasing temperature. Furthermore, the DLC film values were much larger than those of conventional thermoelectric materials: 105 to 1010 times larger. The thermal conductivity of DLC films was about one-half that of conventional thermoelectric materials at room temperature. The results presented above suggest that reducing DLC film resistivity through control of deposition conditions, doped element composition, and other means must be examined to raise the thermoelectric performance of DLC film.

Four cases of chronic complete acromioclavicular dislocation were treated by the contrived coracoid process transfer method. We report the operative procedure and clinical results.The coracoid process, with its attached coracoacromial ligament and the conjoined tendons, is partially osteotomized and transferred to the clavicle. The scarred acromioclavicular ligament is repaired by the coracoacromial ligament and the conjoined tendons are substituted for the coracoclavicular ligament. The acromioclavicular joint is fixed by a Kirschner wire for eight weeks.This surgical procedure was applied to four patients who had been classified as type III by Tossy's classification. The follow-up period ranged from 11 months to 3 years and 2 months.One case required re-operation because of breakage of the transferred coracoid process. But now, all patients have returned to previous activities and there have been no problem with ADL.The clinical results of all patients were judged to be satisfactory.
