国立情報学研究所の「学術雑誌公開支援事業」により電子化されました。The Elderly Abuse Prevention Law was adopted on November 1, 2005, and is slated to come into force on April 1, 2006. This is associated with the fact that the problem of elderly abuse has bubbled to the surface in recent years and begun to be regarded as a significant social problem. Given this, questions such as how homecare support centers, which handle living assistance for the elderly in a comprehensive manner, should respond to this problem and what can be done to relieve this problem through the practice and functions of care management, which has come into being as a result of the establishment of the elderly care insurance system, demand urgent consideration. The purpose of this paper is to compare the results of the research into the realities of elderly abuse at home, compiled by the Ministry of Health, Labor and Welfare, and those of the similar research that the author has conducted on in-home elderly people in Aomori Prefecture, review the functions and roles of care management and identify challenges for the practice of care management.
3 0 0 0 OA 自死遺族のグリーフワークを促進する民間信仰の実態とサポートシステムの構築
我々は、民間信仰の「イタコ」を利用した自死遺族のグリーフワークを促進する要素を明確にすることを研究目的とした。「イタコ」を利用した対象者群と、利用しなかった対象者群の半構造化面接の結果を質的に分析し、それぞれに6つの因子が導出された。「イタコ」を利用した遺族は全てソーシャルサポートを受けておらず、「イタコ」を利用し、語ることによる心の浄化と、故人との内的な対話を通した相互理解や赦しの獲得がグリーフワークの促進要素として見いだされた。

福祉系4年制大学の学生が特別養護老人ホーム等の配属実習先において、遭遇した実習現場職員の利用者との関わり場面の自由記述から、レジデンシャルワーカーとしての専門職資質、特に利用者の権利擁護や虐待予防の問題を中心に若干の課題を検討した。その結果、社会福祉施設という対人援助の実践現場においても、良質なサービスの提供、利用者の尊厳の保持、利用者の意向の尊重、権利擁護、虐待防止などの基本的に職員に求められる行動規範について問題のある職員が存在することが示唆された。特に、施設の利用者は心理的虐待やネグレクトを受けやすく、自己尊重と尊厳を維持する権利、適切な介護を受ける権利、質の高いサービスを受ける権利、人格が尊重される権利などについて、権利侵害が多いことが類推される。したがって、レジデンシャルワーカーを養成する教育組織における人権教育の必要性が求められ、施設においても倫理綱領の作成と職員の行動規範の確立、人権意識啓発のための研修、職場内のスーパービジョン体制の整備などを通して、真の意味で利用者の快適なソフト面での生活環境を整備することが集眉の課題である。

本研究は、認知症高齢者ケアにおける実存主義的な視点から考察したものである。これまで、認知症高齢者のケアについては、ほとんどがケアする側の視点で行為やサービスのあり方、負担感などと関連して議論されてきた。すなわち、主語は「ケアする側」であり、ケアされる側は受け手として常に受動的な立場に置かれてきた。しかし、これまでも、ケアを受ける当事者の経験を理解することが大切である、ということが主張されてきた。認知症になる高齢者にとって、その経験は言うまでもなく初めての経験である。ケアされる側の認知症高齢者は、自分の心身についてなにが起こっているか、的確に認識し、判断することは困難である。認知症というハンディキャップを持ちながらも、一生懸命に生きている高齢者の「今・ここ」での状況を、日常生活の中で具体的に把握することが求められている。 そのため、認知症高齢者が老い衰えてゆく過程、記憶が薄れてゆく過程をどのように体験しているのか、人間存在の根源に根ざした実存的把握が必要不可欠である。 このため、本稿では、実存主義ソーシャルワークの系譜や意義などを概観した上で、ある認知症高齢者の生活関与観察を通して、長年在宅生活を送ってきた、ある、1人の高齢者が、施設入所に至る3日間の状況を記録し、個人の主観的経験へ着目する分析を通して、そのストレングス視点で捉えた行為や、認知症高齢者の言動の奥に込められている苦悶の声や存在不安を〈汲み取る〉ことの必要性や、実存的に理解することの重要性を考察した。This paper aims to study care of demented elderly from an existential viewpoint. Discussions on the care of demented elderly hitherto have almost exclusively been from the carers viewpoint – treating them as passive recipients of the services given by carers. However, in recent years, there have been contentions to the effect that it is important to understand the inner world of the demented elderly receiving care based on their subjective experiences. Being demented and receiving care are situations that they have never experienced before. Conceivably they have difficulty in understanding what is happening to their body and mind. There is a need to concretely grasp the "here and now" within the daily lives of demented elderly who, despite their handicap (i.e. dementia), are living their lives to the fullest.To that end, it is essential to have an existential grasp of how demented elderly experience the process of their aging and deterioration and the decline in their memories.The research presented in this paper at first surveys the origins and significance of existential social work, and then presents a record and analysis of the participant observation of three days in the life of a man suffering from senile dementia – the critical three days before his placement in a facility after having spent many years housebound. Through the analysis of the individual's subjective experiences, this paper shows the necessity of understanding the behavior of the demented elderly from a strength perspective, of perceiving their anguish and existential anxiety through their words and actions, and it confirms the importance of existentialist understanding for the care of demented elderly people.

本研究は、青森県内のホームヘルパーを対象に対人援助におけるコミュニケーションスキルの活用の現状を調査し、その実態を明らかにすることにある。その結果、(1)ヘルパー業務の中で身体介護や家事援助と並んで、相談業務という心理社会的業務を行っているヘルパーが50%にのぼる、(2)何らかの形でコミュニケーションスキルを活用しているものは、79%存在する、(3)最も多く活用されている技法は、うなずき・相槌・共感・明確化・繰り返しなどである、(4)利用者との信頼関係や親密性を維持するためのコミュニケーション効果については、情緒的・精神的・心理的に安定する、ことや行動障害の軽減・性格が穏やかになる・存在認知が出来るなどが挙げられている。これらの結果から、対人援助の専門職としてのホームヘルプサービス業務の確立のためには、コミュニケーションスキルが不可欠なものとして活用され、利用者の精神的・心理的・情緒的支援に活用されなければならない。

2000年6月に成立した社会福祉法は、我が国の社会福祉パラダイムを地域福祉の推進と規定した。個別地域社会において、地域福祉を推進していく上に、「福祉コミュニティ」の形成は不可欠である。本研究では、地域福祉の歴史、背景を考察し、地域福祉政策も福祉コミュニティづくりの理念と方向を持つものであることを考察した。さらに、先行研究の地域福祉概念や構成要件をレビューした。その結果、地域福祉は、地域を基盤にする社会福祉の構築と環境づくり、福祉コミュニティづくりが一体化されたものであり、福祉コミュニティづくりが伴わなければ、在宅福祉を軸とし、地域を基盤とした福祉が構築されたとしても、真の地域福祉とは言えない。福祉コミュニティは、コミュニティづくりの目標であると同時に、コミュニティを構成する一つの社会状態をつくるというものでもある。従って、地域福祉の推進の上で、「福祉コミュニティ」づくりは重要な意義をもつ、と結論付けた。
