Questionnaire survey concerning practical use of generic immunosuppressive agents and the European Society for Organ Transplantation (ESOT) guideline was conducted in 130 clinical institutions including transplant hospitals where the members of The Japan Society for Organ Preservation and Biology were working. Forty five institutions answered the questionnaire survey (the recovery rate: 34.6%). Use of generic immunosuppressive agents in the institutions has been different among the agents; tacrolimus (6.7%), cyclosporin (8.9%) and mycophenorate mofetil (8.9%) and corticosteroid injection (51.1%). The results indicated that use of generic immunosuppressive agents, especially in the agents that require therapeutic drug monitoring (TDM) for dose adjustment, have not been spread in Japan. The most of the institutions (greater than 62%) agreed with the ESOT guideline for generic substitution of immunosuppressive agents. It is considered that the Japanese guideline is also required to prepare according to ESOT guideline, though the use of generic products have not been popular yet Japan.

As a transplantation surgeon, I celebrate and admire the success of uterus transplantation and birth after uterus transplantation. It was brought about by the scientific achievement in organ transplantation. But, as the history of uterus transplantation is short, many key issues on it remain unresolved. Living donor or cadaveric donor, age of donor, operation for hysterectomy in living or cadaveric donor, preservation time of uterus, histocompatibility, immunosuppressive drugs, diagnosis and treatment of rejection, problems in the newborn baby, medical cost and comparison with surrogate conception are discussed.

腎移植の成績は,免疫抑制剤の進歩により,格段に良くなった。それまでは,拒絶反応を少しでも少なくするために組織適合性HLAの一致が求められ,適合者が選択されてきた。しかし,免疫抑制剤の進歩により,HLAの適合度が腎移植成績に与える効果は低くなった。その後,高感度の抗HLA抗体検査が可能となり,組織適合性検査の重要性が見直されてきている。腎移植における組織適合性検査の重要性を認識できるよう,腎移植の現況を解説し,組織適合性検査の関わりの詳細を解説する。また,現在の組織適合性検査の問題点,今後の展望を述べた。

2019年末からの中国武漢での報告後、瞬く間に世界中に拡散した新型コロナウイルス感染症 (COVID-19)は、現在も収束することなく、2020年8月31日時点で全世界で2500万人以上が感染していると報告されている。COVID-19は、移植医療の在り方に大きな影響をもたらし、世界中の多くの移植施設で、生命に関わる臓器の移植を除き、生体移植を中心とした待機可能な移植は一時停止を余儀なくされた。ドナーからの伝播の可能性、院内感染の可能性、移植患者への感染対応など、様々な状況を想定し、院内のコンセンサスを得ることで、現状では多くの施設が移植医療の提供を再開した。しかし、安心安全な移植医療を提供するためには、COVID-19以前とは異なる、新しい移植医療の様式を実践していく必要がある。また、移植患者は免疫抑制剤を服用しており、ウイルス感染症に脆弱である一方、COVID-19は、感染後期での免疫抑制剤使用による重症化抑制も知られている。相反する病態を理解し、免疫抑制剤調整を含めた移植患者の管理を適切に行うことも重要である。これまでに得られたCOVID-19と移植医療に関する文献的報告をレビューし、新しい移植医療の様式について提言したい。
