The efficacy of Amfenac Sodium (Fenazox<SUP>&reg;</SUP>), nonsteroid anti-inflammatory agent on temporomandibular joint pain was clinically evaluated in 28 temporomandibular arthrosis patients. Amfenac Sodium dosage of 200 mg was given orally in 4 times daily for two weeks.<BR>The results were as follows:<BR>1) Temporomandibular joint pain and limitation of mouth opening because of pain improved remarkedly.<BR>2) Effectiveness of Amfenac Sodium was &ldquo;Excellent&rdquo; in 5 cases (17.9%), &ldquo;Good&rdquo; in 12 cases (42.9%) and &ldquo;Fair&rdquo; in 10 cases (35.7%).<BR>3) Side effects were observed in 8 of the 28 cases. Most of those 3 cases had slight gastrointestinal disorders and only two cases were dropped.<BR>4) The usefulness of Amfenac Sodium was in 3 cases (10.7%) &ldquo;Excellent&rdquo;, 13 cases (46.4%) &ldquo;Good&rdquo; and 10 cases (35.7%) &ldquo;Fair&rdquo;.<BR>It is suggested that Amfenac Sodium is useful for relief from temporomandibular joint pain in temporomandibular arthrosis patients.

我々は開口障害の顎関節症患者について, 顎関節造影検査をX線テレビシステムと断層撮影装置を使用して行ってきた。しかし, 上関節腔穿刺時における中頭蓋窩損傷などが問題であった。最近画像支援による外科処置が医科領域で報告され, その有効性が述べられている。しかし, 歯科領域では少ない。当教室で開発した歯科用小照射野CT (以下Ortho-CT) は, X線透視とX線CTの両者の撮影が可能な小型のコーンビーム型X線Computed Tomography (CT) で, 顎顔面領域の小範囲の精査に有効である。我々は, Ortho-CTを利用し顎関節造影検査を行った結果, 良好であったのでその検査法について報告する。<br>本検査法では, 上関節腔に穿刺する場合の安全角度と安全距離を計測した。さらに3次元画像を参照しながら安全性を考慮して造影検査を行った。対象症例は開口障害を伴う顎関節症であり, 検査は安全に行え, 円板の位置と形態の, 造影像はMRIと同じ所見を示した。<br>今後, Ortho-CTを応用したこの検査法は顎関節造影検査に有効であると考えられた。
