One-dose packaging improves the convenience of patients in drug therapy and is useful for securing medication adherence. As psychiatric patients take plural, sometimes many, drugs before bedtime, there are increasing demands for utilizing one-dose packaging in dispensing drugs for these patients. Suvorexant is a potent and selective antagonist against orexin 1 and 2 receptors and its use as an insomnia remedy is rising rapidly. On the package insert of suvorexant (Belsomra®) tablets, there is a description "Store in the press through package (PTP) until use to protect from light and moisture". So far, however, little information has been available about the correct storage conditions regarding the one-dose packaging of Belsomra® tablets. In this study, we investigated the stability of one-dose packaged Belsomra® tablets, which were preserved for 30 days at room temperature or at room temperature under 85% relative high humidity (high humidity), with a focus on the changes in appearance, hardness, mass, disintegration, content, and dissolution. Suvorexant content and the appearance of one-dose packaged Belsomra® tablets did not change even after 30 days. However, the tablets greatly lost hardness and showed an increase in mass, possibly due to the hygroscopicity of this formulation. These changes resulted in the prolonged disintegration time of one-dose packaged Belsomra® tablets and in the retarded dissolution of suvorexant from the tablets. These results suggest that Belsomra® tablets are hygroscopic and less stable. It is, therefore, desirable to avoid long-term, one-dose packaging of this formulation.

We previously reported the association of the estimated glomerular filtration rate (eGFRcreat) calculated from the serum creatinine level (S-Cr) measured using the Jaffe method with the GFR (eGFRcys) estimated from the serum cystatin C level (CysC). However, few studies have compared the eGFRcreat using the enzymatic method with the eGFRcys. It is unclear whether there are differences in the results of renal function assessment. The purpose of this study was to compare the eGFRcreat calculated from the S-Cr with the eGFRcys calculated from the CysC in patients in whom the S-Cr and CysC were simultaneously measured using the enzymatic method, examine the correlations of respective parameters, and clarify physiological factors involved in differences among the parameters. The subjects were 1334 patients treated in 5 institutions. The mean values and correlation coefficient were statistically analyzed using Student's t-test and Pearson's test, respectively. Influential factors between formulae were analyzed using multiple regression analysis. The mean eGFRcreat was 67.0 mL/min/1.73 m2, being significantly higher than the mean eGFRcys (63.2). Multiple regression analysis showed that factors influencing differences in the S-Cr and CysC included the sex, age, serum albumin, and blood urea nitrogen BUN/S-Cr. Furthermore, factors involved in the overestimation of the eGFRcreat in comparison with the eGFRcys included the serum albumin and BUN/S-Cr. The differences between the eGFRcreat calculated from the S-Cr and eGFRcys were less marked than when adopting the Jaffe method in our previous study. However, the eGFRcreat were higher than the eGFRcys in patients with malnutrition or dehydration.

Objective: Consultation contents from medical consumers can become a useful information for medical workers.  However, report which investigated about them is limited.  In this study, we investigated the question contents from medical consumers with telephone.Methods: Nineteen hundred records on the consultation from medical consumers with telephone from 2007 to 2011 were subject to this study.  Using appropriate keywords and check the records, corresponding cases were extracted.Results: Resources of drug information which medical consumers used had various one such as the Internet, television, books, and newspapers.  However the medical consumers did not necessarily understand drug information correctly from the Internet and books.  This means that support by pharmacists was necessary in those cases.Conclusion: We think that information sharing in the medical consumers and medical workers based on a good relationship is important for proper use of drugs.

A questionnaire survey was conducted to investigate the use of health foods among outpatients with diabetes mellitus (DM) at the Health Sciences University of Hokkaido Hospital.Responses were obtained from 69.2% of the patients (180 out of 260).The proportions of patients who had used health foods previously or were using them at the time of the survey were 16.7 and 37.2%,respectively,indicating that more than half of the patients had experience of taking health foods and this was irrespective of sex and age.The most frequently consumed health foods were Aojiru (n=25)followed by Kurozu (n=24)and blueberry extract (n=17).Among the health foods taken,those that influence blood sugar considerably were guava leaves polyphenol (n=16),Gymnema sylvestre extract (n=1),Gymnema sylvestre tea (n=1),and Aloe Vera (n=1).One patient was taking a Chinese health food that contained glibenclamide.Many patients took health foods to keep healthy and as a nutritional supplement,and most of them had not consulted their doctors or pharmacists about the use of health foods.More than 70% of the patients targeted by this study had complications such as hypertension.Since there is a possibility of health foods aggravating DM and its complications and of interactions between them and drugs used to treat DM,doctors,pharmacists and other co-medical workers should provide patients with information on the ingredients of health foods as well the adverse effects that they could have.
