Elneopa NF No. 1 and No. 2 infusions are complete parenteral nutrition solutions packaged as four-chambered bags. They have been used for home parenteral nutrition, with insulin injected into the bags for patients whose blood glucose becomes elevated. In this study, the stability of insulin in No. 1 and No. 2 bags was investigated. The quantity of insulin in Elneopa NF No. 2 was significantly lower than that in Elneopa NF No. 1. When insulin was injected into the upper chamber of either product, decreases in insulin levels were not observed. In contrast, the levels of insulin injected into the lower chamber of both products significantly decreased, with a larger difference in Elneopa NF No. 2. As the amino acid content is different between No. 1 and No. 2, amino acids may be considered a potential cause for the degradation of insulin in the bags. In addition, decreases in insulin levels were observed as the solutions passed through infusion sets just after flushing began, with both Elneopa NF No. 1 and No. 2. In conclusion, the concentration of insulin injected into the Elneopa infusion bags decreases, especially in No. 2 bags, and insulin is absorbed by the infusion sets.

Elneopa NF No. 1 and No. 2 infusions are total parenteral nutrition solutions packaged in four-chambered infusion bags. They have been used as home parenteral nutrition, with various drugs injected into the infusion bags, for treating patient symptoms. In this study, we investigated the stability of six drugs, including famotidine, scopolamine butylbromide, furosemide, bromhexine hydrochloride, betamethasone sodium phosphate, and metoclopramide hydrochloride in the infusion bags under dark conditions at 4℃ for 7 days. Additionally, we developed a high-performance liquid chromatography method to determine drug concentrations in the infusions. The concentrations of injected famotidine, scopolamine butylbromide, and betamethasone sodium phosphate remained unchanged when the four chambers of Elneopa NF No. 1 and No. 2 were opened and the infusions were mixed. Their respective concentrations in the upper and lower chambers also remained unchanged. The concentration of furosemide in the upper chamber of the No. 1 infusion bag decreased after 5 days, although no change was observed in the other chambers and the mixed infusions with the four chambers opened. The concentration of bromhexine hydrochloride slightly decreased in the upper chambers (approximately 3%) after the co-infusion but decreased significantly in the other chambers and the mixed infusions with the four chambers opened. The concentration of metoclopramide hydrochloride significantly decreased in the upper chambers after the co-infusion; however, no change in concentration was observed in the other chambers and the mixed infusion with the four chambers opened. The results of this study provide useful information on home-based parenteral nutrition.

改訂された薬学教育モデル・コアカリキュラムの実務実習では,「代表的な8疾患」に対する実践的な臨床対応の基本を,患者との関わりを通して修得することが求められている.しかしながら,実習施設によって体験できる疾患が限られることもあるため,薬局実習から病院実習を通して,代表的な8疾患を学習できるよう配慮する必要がある.そこで,薬局実習の期間中に,より広範な疾患を体験して病院実習に引き継ぐため,実習の受入薬局では体験が困難な疾患を,他の薬局(以下,協力薬局)で体験する薬局間連携実習を試行した.協力薬局で学ぶ疾患を予め選定し,1日ずつの実習を3週間程度の間隔で計3日間行った結果,対象者(13名)全員が服薬指導と薬歴の記入を体験(のべ患者数:3–23例,中央値5例)し,患者での薬学的管理を実践することができた.この薬局間で連携する実習は,より多くの疾患を薬局実習で体験する手段の一つになり得ることが示された.

臨床の現場で活躍する薬剤師を育成するにあたり,薬物療法の実践能力を修得することは非常に重要である.福山大学では,実務実習セミナーを通し,実習期間中に様々な施設で実習をしている学生及び,教員がともに集まり,討議をすることで,学生間の情報共有を可能とし,また発表内容を「薬物療法の実践」に焦点化することで,学生の薬物療法に係る能力を醸成してきた.しかし,COVID-19パンデミック下で,対面による実務実習セミナーが困難となり,ビデオ会議システムを用いた実務実習セミナーを実施した.ビデオ会議システムを用いることの利点,デバイスの問題への対応,自宅からの参加ならではの副次的効果,さらに,今後の可能性について紹介する.

The role of medication history at pharmacies has increased as a means to realize improved pharmaceutical care. Furthermore, the education guidelines of clinical clerkship for pharmaceutical education were revised in 2013 to provide better clinical training. The principal aim of the new guidelines was to allow the students to experience medication management, such as medication counseling and the recording of medication histories, frequently. In these circumstances, pharmacy students should acquire, to some degree, the cardinal skills of medication management by the initiation of their clinical clerkship. In this study, we established methods to quantitively evaluate the ability of the medication management at pharmacies. Additionally, we conducted the same practice with pharmacists engaged in pharmacies for less than three years to compare their ability with that of the students. In the practice, the participants presumed the patient characteristics and disease state from a scenario and created the medication counseling phrases from a simulated prescription. Subsequently, they recorded a medication history electronically following the SOAP format. The results showed the scores of the pharmacists regarding medication counseling phrases and medication history descriptions were much higher than those of the students. In contrast, the scores of presumption skills in terms of patient characteristics and state were equivalent. To enhance clinical skills, education programs should include problem-oriented system exercises to integrate various factors of patients’ characteristics as well as to build up student knowledge. This study also suggests that the modification of an electronic medication history system might be efficacious for pharmaceutical education.

The third advanced workshop of the Pharmaceutical Society of Japan for pharmaceutical teachers was held from October 12th to 14th, 2013, and participants discussed an outcome-based approach to curriculum development in pharmacy education. In this article, I report the outcome-based spiral curriculum model of group 2A, which was designed to enable pharmacy students to understand a patient's condition, and to provide a basic practical ability in medical therapy. In the curriculum, pharmacy students will learn biochemistry and functional morphology in the first and second years, skills to interview patients in the third year, pathophysiology and pharmacotherapeutics in the third and fourth years, skills to estimate patient disease from physical examination in the fourth year, and practice in understanding real patient conditions in a clinical clerkship in the fifth year. The curriculum also included learning and evaluation methods.

Ulcerative proctitis (UP) is a prevalent condition associated with increased morbidity, and topical mesalazine (5-aminosalicylic acid [5-ASA]) is known to inhibit the inflammatory processes in UP. We successfully devised mesalazine suppositories, in which 250mg mesalazine was equally distributed and remained stable for at least 2 weeks. We evaluated the effect of using mesalazine suppositories twice a day (BID) on two UP patients. The results demonstrated that mesalazine suppositories were efficacious, well tolerated and safe for the long-term maintenance of UP remission.

The text mining and full-text searching function were used to analyze the digitalized daily reports of the practical training in pharmacology submitted to the web system in Fukuyama University. Collocations connected to the word “inspection” were searched in the daily reports using the text mining and the full-text search functions of the system. Many collocation groups connected to “inspection” were found in the daily reports and the greatest number of collocations was associated with “preparation of drugs”. Practical training in narcotic dispensing has two different aspects: inspection and experience training. The number of people who reported a relation between “inspection” and “narcotics dispensing” in the daily reports was very similar to those who reported a connection between “experience” and “narcotics”. Practical training to handle narcotic dispensing is the most fundamental training that the pharmacist must undertake. The progression of team-based medical care has caused medical personnel to recognize the practical training in pharmacology, and the number of the people who reported a relation between “inspection” and “team medical care” in 2011 increased in comparison to 2010. Moreover, the progression of cooperation among hospitals, pharmacies, and local blanches of the Japan Pharmaceutical Association is beneficial to practical training. Practical training in pharmacology is based on the core curriculum, but undergoes periodic modification due to societal circumstances. Therefore, these results suggest that the construction of web system for submitting daily reports is useful for analyzing the daily reports.
