成人の吃音患者に,社交不安障害(social anxiety disorder,以下SAD)が40%以上もの高い確率で合併する(Blumgartら,2010).SADにおいては,人と接する場面で強い不安を覚えるばかりでなく,社会生活上に大きな支障を及ぼす可能性があり,精神科・心療内科で薬物療法を行われることが多い.しかし,吃音にSADが合併している場合は,吃音をよく知っている言語聴覚士とも協力したほうがSADから回復し,従来の生活に戻れる可能性が高まる.症例は16歳男性,授業で本読みをすることに恐怖を感じ,不登校となった.心療内科でSADと診断され薬物療法を受けるが,本読みのある授業は欠席していた.耳鼻咽喉科に紹介され,環境調整,言語療法,認知行動療法を併用した結果,3週間後に授業を欠席せず登校可能となり,通常の高校生活に戻ることができた.吃音症にSADが合併した症例は,医師による薬物療法だけではなく,耳鼻咽喉科医・言語聴覚士の積極的介入が有用であると考えられた.

ゼリー状の食塊は液体に比べて誤嚥しにくく、喀出しやすいために嚥下障害患者の経口摂取開始において頻用されている。その要因として液体と異なりゼリー状の食塊はその物性(硬さ、付着性、凝集性を持つ)のために咽頭への流入速度が遅いことが考えられる。しかし、現在までに嚥下造影検査においてその仮説を裏付ける目的であらかじめ物性の分かっている二つの嚥下物の咽頭期嚥下動態の違いを、嚥下惹起遅延を評価するのに有用と考えられているパラメーターを用いて比較した報告はない。そこで進が 1994 年に報告した laryngeal elevation delay time (LEDT) という咽頭期嚥下の遅れを評価するパラメーターを用いて、異なる物性を持つ二つの嚥下造影剤における咽頭期嚥下動態の違いを検討した。その結果、われわれが用いた LEDTは、1) 液体造影剤とゼリー状造影剤の二つの物性の違いをよく表し、2) 低粘性造影剤を用いることで咽頭期嚥下の遅れを評価する有効なパラメーターであることが確認され、3) ゼリー状の食物形態が咽頭期嚥下惹起遅延による誤嚥を来す症例の食事に有用であることを裏付けるパラメーターであると考えられた。

Back support muscle suit is applied to the work of lifting heavy material in the material handling field, in order to reduce the workload. The work model is defined based on the idea of value steam map, and the possibility of introduction is shown using productivity, which was calculated from the number of handled products during the test and normalized by the one of the subject in the month, and the result of subjective evaluation of 13 workers, which showed that 9 workers could feel better or neutral during the work with muscle suit than without it. And, it was shown that the productivity of the workers with muscle suit was almost the same as the one without muscle suit. The workers who felt neutral are the ones who felt better when they lifted the loads in the lower part of the palette. It was also shown that, based on the characteristics of the subjects, such as age mainly, the result of the subjective evaluation was considered to be reasonable from the viewpoint of power of standard worker.

頸部郭清術の嚥下機能に対する影響について検討を行った。頸部郭清術を行う際に迷走神経咽頭枝を損傷すると咽頭筋麻痺によりクリアランスが低下する。今回の検討では再建例も多く、クリアランス低下に加え、喉頭挙上制限や、嚥下惹起が遅延することで、誤嚥のリスクが高まることが明らかとなった。そのため、極力迷走神経咽頭枝を温存すべきであると考える。

症例は84歳の女性。平成14年2月腹痛・嘔吐・下痢・血便を主訴に入院。既往歴として高血圧・狭心症・糖尿病があり内服治療中であった。便細菌培養にて病原性大腸菌O-1を検出, CT・下部消化管内視鏡検査にて下行結腸に壊死を伴う炎症を認め, 細菌性腸炎による脱水を誘因とした虚血性大腸炎と診断した。入院翌日のCEAが106.1ng/mlと異常高値を示し, 悪性腫瘍の検索を行ったが病変を認めず。26日後のCEA再検査では正常化していた。3月13日下行結腸切除術および人工肛門造設術施行した。病理診断上悪性所見を認めず, 免疫組織化学染色によるCEA局在も正常であった。検索上, 虚血性腸炎におけるCEA上昇は, 今らの1例報告のみであった。CEA高値を呈した機序は不明であった。発症時に細菌性腸炎に罹患していたことによるCEA産生活性化の可能性や, 膿瘍腔内に便汁が多量に貯留した可能性などの複合的な要素の関与が考えられた。

Aggregation of the 42-mer amyloid β (Aβ42) plays a central role in the pathogenesis of Alzheimer's disease. Our recent research on the systematic replacement of Aβ42 with proline suggested that the formation of a turn structure at Glu-22 and Asp-23 could be essential to the potent aggregative ability and neurotoxicity of Aβ42. We verified the existence of this turn structure in the minor conformer of wild-type Aβ42 and E22K-Aβ42 (Italian mutation), by solid-state NMR using dipolar assisted rotational resonance (DARR). In E22K-Aβ42, the ionic interaction between Lys-22 and Asp-23 might promote the turn formation at this site. In order to identify the toxic conformation of Aβ42, we synthesized Aβ42-lactam(22K-23E) as a conformationally restricted analogue of the minor conformer, whose side chains of Lys-22 and Glu-23 are linked with an amide bond. Aβ42-lactam(22K-23E) showed much stronger aggregative ability and neurotoxicity than E22K-Aβ42. This implies that the minor conformer with a turn at Glu-22 and Asp-23 of Aβ42 should be considered as a toxic form. Neurotoxicity of Aβ42 is closely related to the radicalization at both Tyr-10 and Met-35. Our previous study reminds us that the S-oxidized radical cation at position 35, the ultimate toxic radical species, would be produced effectively through oxidation by the phenoxy radical at position 10 in the toxic conformer. Electron spin resonance (ESR) spectrometry using spin-labeling with MTSSL revealed that these residues are close to each other in Aβ42. This finding clearly accounts for the reason why the toxic conformer is more pathogenic than the physiological one.
