肺塞栓症の塞栓源として下肢深部静脈血栓症が知られている.肺塞栓症を合併した孤立性ヒラメ筋静脈血栓症の3症例を経験したので報告する.症例1: 59歳,女性. Cushing syndromeに対する腹腔鏡下副腎摘出術後3日目に胸部圧迫感,低酸素血症が出現.症例2: 54歳,女性.卵巣癌の既往があり1カ月前より左下腿鈍痛が出現.症例3: 44歳,女性.両下腿に腫脹,鈍痛があり,階段昇降時に息切れを自覚.いずれも下肢超音波検査でヒラメ筋静脈のみに限局した血栓,肺血流シンチで肺血流欠損像,胸部造影CT検査で多発性肺動脈血栓を認めた. 3例中1例にウロキナーゼによる血栓溶解療法,全例に抗凝固療法を行い症状の改善が得られた.ヒラメ筋静脈血栓症は臨床症状が乏しいため見落とされやすい.しかし肺塞栓症の合併,血栓の中枢側進展,再発を繰り返すことがあり,積極的に診断,治療,予防する必要がある.

A case of schwannoma originating in the right intrathoracic vagus nerve was reported. A 72-yearold woman had an abnormal shadow in the right upper mediastinum on a chest roentgenogram. A CT scan and MRI revealed a tumor in the upper mediastinum compressing the trachea, right carotid artery and right subclavian artery. An incisional biopsyof the mass in the right neck was revealed to be benign schwannoma. The mass was 55 ×42×29mm in size and originated from the right vagusnerve at the proximal level and extended to the branch of the recur-rent laryngeal nerve. There was no evidence of invasion of the mass into the surrounding organsand tissues were evident. Subcapsular ex tirpation was done preserving the nerve fibers. However, postoperative hoarseness due to dysfunction of the recurrent laryngeal nerve occurred. Mediastinal schwannoma originating in the vagus nerve is notso common ; 29 cases have been reported in Japanese literature.

The use of a reagent for digoxin assay of fluorescence polarization immunoassay with TDX analyzer in the determination of serum deslanoside concentration was studied. Deslanoside calibrators with the concentration of 0.0-6.0ng/ml were prepared. The assay item for digitoxin was used for deslanoside after modification of the parameters. The coefficients of variation of within-run assay and between-run assay in this method were less than 9% at various concentrations tested. By use of a ultrafiltration method, the protein binding percent of deslanoside was determined to be zero or very small. In a patient with renal and hepatic failures, the half life (t1/2) and the apparent volume of distribution (Vd) were calculated to be 131 hours and 4.1l/kg after intravenous administration of deslanoside, respectively. On the other hand, an equation of Y=1.25X+0.08 was obtained from a correlation between digoxin concentration (X) given by normal digoxin assay and concentration of deslanoside (Y). Therefore, with a simple method, one can calculate the deslanoside concentrations.
