While walking, we often change our direction and speed as necessary. The movement made to change the direction is divided into two types: the spin turn and the step turn. The axis of the lower extremity in the spin turn is on the same side as the direction in which the turn is made, whereas in the step turn, it is on the opposite side. In this study, with respect to the spin turn and step turn, the center of pressure trajectories and the electromyography patterns of the lower extremity were measured. Based on these results, the function of the lower extremity required for the spin turn and step turn is explained and discussed.

Walking is the main means of locomotion. Locomotion is performed not only forward but also backward in daily life. However, there are few explanations of backward walking, compared to forward walking, in textbooks of motion analysis. In this study, previous studies of backward walking are reviewed, and the trajectories of center of pressure and electromyogram patterns assessed. The functions of the lower leg and the trunk in backward walking are discussed with reference to the results.

In clinical practice, muscle weakness is often considered a functional disability similar to limitation of range of motion. Physical therapists use manual muscle testing (MMT) as described by Daniels et al. for the general assessment for muscle weakness. This test quantitatively assesses muscle strength by the amount of joint movement, muscle contraction, and the effect of gravity and resistance. However, it is not possible to evaluate the strength of individual muscles involved in a single joint movement, or to separate a single muscle into fibers with different actions when using this method. It is also difficult to assess qualitatively whether muscle activity is assessed at a specific time during movement. In this paper, we examine the possibility of evaluating muscle weakness using MMT for knee extension, hip extension, and hip abduction by changing some of the test positions and the direction of manual resistance based on kinematics. For knee extension, we describe how to check the alignment of the patella and how to vary the movement speed to check for muscle activity at a given time. Each of these approaches are presented based on electromyogram data.

We administered physical therapy to a patient with post-stroke right hemiplegia who had difficulty eating with the fingers of the affected hand. The functioning of the fingers was preserved, but their dexterity decreased because the affected upper limb could not maintain forward elevation. We believed that instability of the trunk and scapular region of the affected side influenced finger movements of that side. Therapeutic gain, after providing physical therapy to the trunk and affected shoulder region, was assessed using surface electromyography. Alignment of the trunk and affected shoulder region as well as feeding activity improved. Surface electromyography evaluation during forward elevation of the affected upper limb showed an improvement in the muscle activation pattern. Stability of the chest and scapula was necessary to attain finger dexterity in this patient.

Sitting down is one of the items of activities of daily living, and it involves movement from a standing to a sitting position. This paper outlines the components of this sitting-down movement. Sitting down and standing up are inverse motions. The electromyography pattern of the sitting-down movement was analyzed and is discussed with reference to physical therapy. An improved understanding of the components of this movement is desirable.

The thalamus is the last relay site on the way to the cortex for almost all sensory information. In addition, it is connected with the beginning and adjustment of movement. The function of the thalamus is described in this text, and the application of physical therapy is considered.

This study examined the influence of posture change of hemiplegic stroke patients in their forward stepping motion. The results suggest that forward stepping motions differ in terms of joint motion, electromyogram pattern, and center of pressure owing to hip joint and tibial external rotation. This study introduces an intervention based on evaluation of the forward stepping motion.

This case report describes a patient with femoral neck fracture and body instability in the forward direction at the end of the stance phase during walking. Physical therapy for this patient included both muscle strength training of the hip extensor muscles and weight-transfer exercises with reaching movements using one upper limb while holding a T-cane. With physical therapy, weight transfer in the forward direction during the stance phase accompanied hip extension. Moreover, forward body instability at the end of the stance phase disappeared and walking ability improved. The effects of physical therapy for this patient are discussed with reference to improvements in joint angle values.

Motor adaptation and learning are processes in which the efficiency of movement is improved with regular practice. In these processes, changes in neuromuscular function are observed. In this report, we review a previous study of changes in neuromuscular function in a motor learning process, and assess the physical therapy.

Somatic sensation has an important role in the control of movement. The information from a receptor produces perception in the cerebral cortex. Movement is adjusted using this sensory information. In addition, movement is sometimes adjusted by reflex through the spinal cord or brain stem, and it is not perceived. In order to perform a suitable sensory stimulus, physiological consideration of the observed phenomenon is required. In this paper, a previous study of sensory motor function is reviewed, and physiotherapy is discussed.

It is necessary to interpret the behavior in order to improve the behavior of a paralyzed upper limb in patients with cerebrovascular disorders. If the motor analysis is incorrect, the behavior will not be improved by physical therapy. Therefore, motion analysis is an important first step in physical therapy. In addition, muscle tone can be adjusted by stimulating the skin and muscles in physical therapy. This paper outlines the content of physical therapy based on kinematic and physiological interpretations of the characteristic movements of paralyzed upper limbs of patients with cerebrovascular disorders.

健常者を対象に複雑性の異なる手指対立運動の運動イメージが上肢脊髄神経機能の興奮性に及ぼす影響についてF波を指標として検討した。対象は健常者15名とした。F波測定は背臥位で安静時 (安静試行), 右母指と示指による対立運動のイメージ (課題1), 右母指と他指の対立運動を示指, 中指, 環指, 小指の順でイメージ (課題2), 示指, 環指, 中指, 小指の順でイメージ (課題3) した状態で行った。また, 各課題をどの程度想起できていたかを知る目的でアンケート調査を実施した。その結果, 振幅F/M比は安静試行に比べて課題2で有意に増加し, F波出現頻度は安静試行に比べて課題1と課題2で有意に増加した。また, アンケートの点数は課題1と課題2に比べ課題3で有意に低下した。本結果より, 複雑な運動イメージでは上肢脊髄神経機能の興奮性がより増加するものの, 運動イメージの想起が難しければ上肢脊髄神経機能の興奮性は変化しない可能性が考えられた。

〔目的〕運動イメージの明瞭性の個人差が複雑性の異なる手指対立運動のイメージ課題における上肢脊髄神経機能の興奮性に及ぼす影響についてF波を用いて検討した.〔対象と方法〕対象は健常者30名とし,Vividness of Movement Imagery Questionnaireの得点結果より運動イメージの明瞭性の高い群,低い群にふりわけた.運動イメージ課題は複雑性の異なる3種類の手指対立運動とした.各群における安静試行と各課題の振幅F/M比の変化を比較検討した.〔結果〕振幅F/M比は運動イメージの明瞭性の低い群において安静試行と比較して課題2,課題3で有意に増加した.〔結語〕運動イメージの明瞭性の個人差が運動イメージ中の脊髄神経機能の興奮性の違いに影響を及ぼす可能性が示唆された.

We report the case of a man in his 50s with decreased endurance in gait after brain tumor resection. His main complaints were that he felt pain on the right side of his waist and that he could not walk for long periods. We observed that during the left loading-response phase, he could not adduct his left hip joint, nor could he move his pelvis to the left. Therefore, during the initial swing phase, he put his right plantar foot down quickly and his trunk tilted forward. This resulted in flexion of the thoracolumbar transition during the right loading-response phase and extension of the thoracolumbar transition during the right mid-stance phase. The patient repeated this gait pattern, and after about 3,000 steps, he developed pain in the right lumbar region. We considered the main problem to be weakness of the left hip adductor muscle, and treated the patient with physical therapy. As a result, left hip adduction became possible during the left loading-response phase, and the pain in the patient’s right lumbar region improved, resulting in improved endurance of walking. In this case, physical therapy with a focus on the left adductor magnus muscle was effective at improving the pain in the patient’s right lumbar back and the functioning of the hip adductor muscles required during the left loading-response phase.

In daily life and sport situations, external stimuli can be used to control exercise. In physical therapy, periodic auditory stimulation is said to be effective at improving patients' walking ability (walking speed, stride length, and cadence). A metronome, hand clapping, and vocalization are used by therapists to deliver periodic auditory stimuli. Prediction of the rhythm is necessary to perform efficient exercise based on auditory stimuli. Reaction time and synchronization tapping tasks are used to examine the ability to perform exercise according to rhythm prediction. The mechanisms of motor control in the central nervous system are different for periodic and non-periodic movements. This study describes the results of research on motion control according to rhythm prediction based on reaction time and synchronization tapping tasks, the ability to reproduce rhythmic movements with hand clapping and foot stepping, and rhythm tasks and sensory function, and discusses clinical applications of rhythm tasks.

Sensory information is required to perform appropriate movements. Not all sensory information is input during movements, and only necessary sensory information is extracted by the nervous system. When promoting sensory input for cases with sensory disorder, it is important to conduct exercises with low frequency and pay attention to the joint movements while the subject consciously performs the exercise. In this paper, we describe the relationship between sensory function and motor function, and present self-training for cases with sensory disorder.

【目的】運動学習は、結果の知識(KR)によって学習効果に影響することが報告されている。我々は第43回日本理学療法学術大会で、運動学習の練習終了後のテストにおいて、2試行に1回の言語的KR付与の練習が学習効果を認めることを報告した。今回、この先行研究の結果より練習中の学習戦略について検討した。<BR>【方法】対象は本研究の参加に同意を得た健常人23名(男性16名、女性7名、年齢25.3±2.1歳)とした。本研究では握力学習を試行した。被験者は、利き手最大握力の50_%_握力を目標値とした。KRは目標値の上下2_%_誤差範囲を正答KRとし、その範囲内の試行で「正答」、正答KRより低い値は「下」、高い値は「上」という言語的KRを用いた。全学習試行にKRを付与する群(100_%_KR)、2試行に1回KRを付与する群(50_%_KR)、3試行に1回KRを付与する群(33_%_KR)、KRを付与しない群(0_%_KR)に被験者を無作為抽出した。実験は学習前試行、学習試行、学習後試行の順で行った。学習試行では、群分けのKR付与頻度に応じて10試行を1セットとし計3セット実施した。測定はデジタル握力計GRIP-D(竹井機器工業株式会社)を使用し、文部科学省の体力測定における握力測定法に準じて実施した。学習試行中の目標値と実測値のずれとしてRoot Mean Squared Error(RMSE)を算出した。RMSEが目標値に対するパーセンテージとなるようNormalize Root Mean Squared Error(NRMSE)への正規化を行い、学習試行の各セットにおいて群間比較した。<BR>【結果】各セットのNRMSEは次の結果を示した。第1セットは0%KR(31.6±18.6)に対して100%KR(10.6±2.6)は低下を認めた(p<0.05)。第2セットは0_%_KR(33.2±7.4)に対し100_%_KR(9.3±3.4)、50_%_KR(10.1±2.7)、33_%_KR(10.1±3.0)は低下を認めた(p<0.01)。第3セットは0_%_KR(29.1±9.2)に対し100_%_KR(7.5±4.2)、50_%_KR(12.6±6.9)、33_%_KR(10.6±1.9)は低下を認めた(p<0.01)。<BR>【考察】言語的KR付与の頻度は、学習戦略に影響を与えることを示唆した。学習初期は内的基準の修正にKRが使用され、試行回数が増加するにつれて、内部モデルの強化のためにKRが有効的に使用される。しかし、100%KRのようにKRが高頻度であるとKRに依存的になり、学習において重要とされる内部モデルの強化は乏しくなると考える。Salmoniらのガイダンス仮説やSwinnenの内部フィードバックへの注意と学習の関係からも同様のことが示されている。<BR>【まとめ】言語的KRの頻度は学習過程に影響し、付与頻度による戦略の違いが運動学習に影響を及ぼすことが示された。
