Event-related magnetic fields (ERFs) were recorded in seven subjects when they were presented with a rectangle and Japanese Kanji characters, followed by vocalizations. The ERF averages for the Kanji characters were different from those for the rectangle at latencies of about 100ms over the occipital lobe and 150-500ms over the temporo-parieto-occipital region. This observation suggests that the first stage of visual word processing might arise in the occipital lobe.In some cases, difference between the ERFs for the Kanji characters and those for the rectangle were also seen in the frontal lobe at latencies greater than 250ms. This observation suggests that frontal lobe activity may be also involved in visual word processing.Three equivalent current dipoles were estimated on the bilateral precentral gyrus (motor area) and the posterior part of the superior frontal gyros (supplementary motor area) at a latency of about 500ms during vocalization.In some cases, early responses at latencies of less than 150ms were obtained in not only the occipital lobe but also the frontal and temporal lobes. This suggests that parallel processing might occur during visual word processing.

就学前幼児6例の人工内耳装用経験に基づき, 初回マッピングとその後のマップの調整や装用状況について調べた。 初回マッピングでは, T/Cレベルを幼児の表情や行動の変化によって測定するが, 先天性難聴幼児5例中4例がCレベル測定時に目を覆ったり部屋の電気がピカピカしていると視覚刺激のように感じ, その後の2回目のT/Cレベル測定を嫌がり, 内2例はヘッドセットを装着することも嫌がって, 終日装用まで時間がかかった。 このことから幼児のT/Cレベル測定では, 目を覆う反応の前に見られる身体接触を求めるなどをCレベルとして次の電極の測定に移り, 測定刺激が不快レベルにならないよう慎重にする必要があると考えられた。 また, マップ作製後の装用では, 感度調節ツマミを最適感度レベルより低い値にして, 装用時の音への反応を観察しやすくすることが, マップの調整に有効であった。

A patient with right exophthalmus due to exacerbation of relapsing sinusitis who had undergone bilateral radical paranasal sinus surgery in our hospital 6 months earlier visited us and was diagnosed with acquired immunodeficiency syndrome (AIDS) after many tests. The patient was a 36-year-old male who had consorted with prostitutes. Esophageal candiditis and tumors of the liver and the colon were found. Blood tests showed a reduction in lymphocytes, particularly in T-cells and an CD4/CD8 ratio of 0.2. HIV (human immunodeficiency virus) antibody titer was increased. Remission of exophthalmus came after therapy with steroids and antibiotics.

A 49-year-old male was treated for chronic tonsillitis with brachytherapy 40 years ago. The inflammation of the tonsils was improved by the treatment. On August 18th, 1998, he consulted our hospital complaining of sudden throat pain. The pain deviated to the left, intensified with eating and continued for several hours every day for one month. An X-ray revealed a metallic foreign body in his left tonsil. A left tonsillectomy was done on January 14th, 1999, and the foreign body was removed completely. It was a small gold tablet, 3×1×1mm in size. A scintillation counter proved that the metallic foreign body was a radon (²²⁰Rn) seed. This is a rare case of a radon seed buried in the left tonsil of a patient for 40 years.
