Event-related magnetic fields (ERFs) were recorded in seven subjects when they were presented with a rectangle and Japanese Kanji characters, followed by vocalizations. The ERF averages for the Kanji characters were different from those for the rectangle at latencies of about 100ms over the occipital lobe and 150-500ms over the temporo-parieto-occipital region. This observation suggests that the first stage of visual word processing might arise in the occipital lobe.In some cases, difference between the ERFs for the Kanji characters and those for the rectangle were also seen in the frontal lobe at latencies greater than 250ms. This observation suggests that frontal lobe activity may be also involved in visual word processing.Three equivalent current dipoles were estimated on the bilateral precentral gyrus (motor area) and the posterior part of the superior frontal gyros (supplementary motor area) at a latency of about 500ms during vocalization.In some cases, early responses at latencies of less than 150ms were obtained in not only the occipital lobe but also the frontal and temporal lobes. This suggests that parallel processing might occur during visual word processing.

Patients with multi-electrode cochlear implants can distinguish vowels well, but the ‘sounds’ they hear through this device are unknown. In order to simulate vowels through multielectrode cochlear implants, we synthesized waves which have translated formant frequencies using a computer and reverse fourier transformation .Through a digital-analog converter, we heard the synthesized sounds. Each sound is quite different from each vowel, but we can easily distinguish them from each other. With the use of this procedure, cochlear implants can be expected to be improved.

口蓋裂例における鼻副鼻腔疾患の実態を調べるため, 口蓋裂患者101例を対象として, 前鼻鏡, レントゲン撮影(単純, 断層), 鼻腔通気度, 嗅裂内視鏡等の諸検査を行い, さらに, 鼻疾患と中耳の関係をみるために, 耳鏡検査, 標準純音聴力検査, インピーダンスオージオメトリーも行ったところ以下の結果を得た.1)唇顎口蓋裂は鼻中隔蛮曲症, 下甲介肥大を高率に伴う.2)口蓋裂患者は副鼻腔炎を高率に伴い, これは中耳疾患の病因の一つである可能性がある.3)副鼻腔炎の頻度は口蓋裂の裂型や鼻中隔蛮曲の有無とは相関がない.4)口蓋裂に高率に伴う副鼻腔炎, 鼻中隔蛮曲, 下甲介肥大は口蓋裂患者の鼻腔抵抗を高めている.5)口蓋裂患者の嗅覚障害は軽度で, この主因は副鼻腔炎による嗅裂の閉塞にあると思われる.

The clinical usefulness of Tsumura-Saireito, which was judged by tympanoaudiometric tests, was compared with that of cepharanthin in 64 ears of 42 children with otitis media with effusion. The trial was a randomized controlled study.The efficacy rate for the 32 ears treated with Tsumura-Saireito was 43.8% and that for the 32 ears treated with Cepharanthin was 18.8%; the difference was significant (p<0.05). No side effects were observed in either group.These results indicate that Tsumura-Saireito is more effective in children with otitis media with effusion than Cepharanthin.

就学前幼児6例の人工内耳装用経験に基づき, 初回マッピングとその後のマップの調整や装用状況について調べた。 初回マッピングでは, T/Cレベルを幼児の表情や行動の変化によって測定するが, 先天性難聴幼児5例中4例がCレベル測定時に目を覆ったり部屋の電気がピカピカしていると視覚刺激のように感じ, その後の2回目のT/Cレベル測定を嫌がり, 内2例はヘッドセットを装着することも嫌がって, 終日装用まで時間がかかった。 このことから幼児のT/Cレベル測定では, 目を覆う反応の前に見られる身体接触を求めるなどをCレベルとして次の電極の測定に移り, 測定刺激が不快レベルにならないよう慎重にする必要があると考えられた。 また, マップ作製後の装用では, 感度調節ツマミを最適感度レベルより低い値にして, 装用時の音への反応を観察しやすくすることが, マップの調整に有効であった。

In 289 patients with cleft lip and palate, cleft palate, submucous cleft palate and congenital velopharyngeal insufficiency, various aspects of pharyngeal stops were studied: incidence, place of articulation, factors which influence the occurrence, and elicited consonants (except/k/).Observing those videofluoroscopy of the patients, we selected ones whose back of the tongue (and epiglottis) touched the posterior pharyngeal wall while they pronounced [ka]. The incidence of pharyngeal stops for [ka] was approximately 9%.Pharyngeal stops were divided into three types according to the site of articulation: oropharynx, oro-and laryngopharynx and epiglottis. The oro-and laryngopharynx type, in which the epiglottis as well as the back of the tongue made contact with the posterior pharyngeal wall, was most common. Among the factors which influence the occurrence of pharyngeal stops were the following vowels. When low vowels followed/k/, such as [ka, ko], pharyngeal stops were most frequent. They were somewhat less frequent during the production of [ku], and rare during that of [ki, ke]. It was observed that the site of articulation of [ka, ku, ko] was different from that of [ki, ke] in a single patient. Moreover, the occurence of pharyngeal stops was influenced by the number of syllables: that is, they appeared less frequently in words or sentences than in single syllables. Pharyngeal stops were also observed during the production of/p/and/t/.Pharyngeal stops are considered to be variable abnormal articulations.

A 25-year-old male produced a continuous high-pitched tone (6.1KHz, 37.2dB SPL) which was not audible to the patient. He had a sensorineural deafness over 1KHz with a dip of 45dB at 6KHz. The sound was considered to be due to a spontaneous oto-acoustic emission.

Characteristics of hearing disorders due to retro-cochlear lesions have been recognized as disproportionally poor discrimination of speech to the hearing level in pure tone audiometry and disturbances in tone decay test, Bekesy audiometry and binaural hearing test. There have been numerous patients whom we diagnosed as having retro-cochlear hearing loss because they had these characteristic. However, we have encountered quite a few individuals who failed to obtain speech understanding after cochlear implantation (CI) in spite of poor speech discrimination before surgery.From these clinical observations and review of recent literature, it appears that true retro-coc-hlear hearing disorder is far less frequent than we thought and that the retro-cochlear pathway may not have an important role in the process of speech recognition, although the cochlea plays an important role in transforming speech sound into electric signals. The main function of the retrocochlear tract may be to detect the direction of the sound source, as the vestibular tract contributes to equilibrium of the body in a reflex manner.Thus, in perception of speech, the cochlea and the auditory cortex play more important roles than the retro-cochlear pathway does. From the clinical points of view, cochlear implantation can be indicated even for patients who have any discrepancy between the result of pure tone audiometry and speech discrimination test.
