Acute transverse myelitis (ATM) has been described as an uncommon complication of vaccinations and is rarely accompanied by inflammatory peripheral neuropathy. We report a case of a 77-year-old woman who developed ATM and acute motor axonal neuropathy (AMAN) following vaccinations against seasonal and 2009 A/H1N1 influenza. She manifested ophthalmoplegia, quadriparesis and sensory impairment. MR imaging showed a longitudinally-extensive spinal cord lesion, and nerve conduction study revealed motor axonal polyneuropathy. Despite prompt treatment, her symptoms poorly recovered. While concurrent ATM and AMAN may suggest the presence of a common antigen, their scarcity indicates the importance of other factors causing immunologic disruptions.

Protein production in plants is a hot topic because there are many benefits relative to bacteria, yeasts, and animals, but the amount of protein expression in plants is less. It is argued that editing 5'UTRs increases the amount of translated proteins. However, obtaining such 5'UTRs is difficult due to the cost, time and effort required in experiments. To solve this, we predict the amount of translated proteins by machine learning. In this paper, we propose a method, named “R-STEINER, ” that generates 5'UTRs that increase the amount of proteins of a given gene. The proposed process involves building a model for predicting the amount of translated proteins, generating 5'UTRs, selecting them and increasing the proteins according to the model. This method enables us to obtain 5'UTRs that increase the amount of translated proteins without real synthesis experiments, resulting in reduced cost, time and effort. In our study, we built a prediction model for Oryza sativa and synthesized the 5'UTRs generated by R-STEINER. We confirmed that the model can predict the amount of translated proteins with a correlation coefficient of 0.89.

[Purpose] The aim of this study was to investigate whether gaze stabilization exercise derives sensory reweighting of vestibular for upright postural control. [Subjects and Methods] Twenty-three healthy volunteers participated in this study. The center of pressure of the total trajectory length was measured before (pre), immediately after (post), and 10 min after (post10) gaze stabilization exercise, in the static standing position, with the eyes open or closed, on the floor or on foam rubber. The sensory contribution values of the visual, somatosensory, and vestibular systems were calculated using center of pressure of the total trajectory length value in these measuring conditions. [Results] The center of pressure of the total trajectory length on foam rubber in post and post10 were significantly lower than that in the pre. The sensory contribution values of vestibular in post10 stages were significantly higher than that in pre-stage. [Conclusion] Gaze stabilization exercise can improve the static body balance in a condition that particularly requires vestibular function. The possible mechanism involves increasing sensory contribution of the vestibular system for postural control by the gaze stabilization exercise, which may be useful to derive sensory reweighting of the vestibular system for rehabilitation.

This study aimed to evaluate the effects on the medical economy of the use of tracing reports by pharmacy-based pharmacists for pharmaceutical interventions, including to reduce leftover medicines. These effects were estimated by analyzing 267 tracing reports issued by pharmacy pharmacists over a period of 1 year, 2020–2021. We estimate that these interventions created cost savings of USD108170.02/year (USD104800 via pharmaceutical interventions, USD3370.02 via interventions to reduce leftover medicines). The cost savings from pharmaceutical interventions prompted by patient follow-up was estimated to be USD47650. The medical economic effect per tracing report was estimated to be USD392.51 from pharmaceutical interventions, USD12.62 from reducing leftover medicines, and USD445.33 from pharmaceutical intervention prompted by patient follow-up. Overall, therefore, pharmaceutical interventions by pharmacy pharmacists using tracing reports, including those designed to reduce leftover medicines, may benefit the medical economy.

Background: This cross-sectional study evaluated the association between unhealthy lifestyle behaviors and the prevalence of chronic kidney disease (CKD) in middle-aged and older men.Methods: The subjects included 445 men without a history of cardiovascular disease, stroke, or dialysis treatment, who were not taking medications. Unhealthy lifestyle behaviors were evaluated using a standardized self-administered questionnaire and were defined as follows: 1) lack of habitual moderate exercise, 2) lack of daily physical activity, 3) slow walking speed, 4) fast eating speed, 5) late-night dinner, 6) bedtime snacking, and 7) skipping breakfast. The participants were divided into four categories, which were classified into quartile distributions based on the number of unhealthy lifestyle behaviors (0–1, 2, 3, and ≥4 unhealthy behaviors).Results: According to a multivariate analysis, the odds ratio (OR) for CKD (defined as estimated glomerular filtration rate [eGFR] <60 mL/min/1.73 m2 and/or proteinuria) was found to be significantly higher in the ≥4 group than in the 0–1 group (OR 4.67; 95% confidence interval [CI], 1.51–14.40). Moreover, subjects’ lack of habitual moderate exercise (OR 3.06; 95% CI, 1.13–8.32) and presence of late-night dinner (OR 2.84; 95% CI, 1.40–5.75) and bedtime snacking behaviors (OR 2.87; 95% CI, 1.27–6.45) were found to be significantly associated with the prevalence of CKD.Conclusions: These results suggest that an accumulation of unhealthy lifestyle behaviors, especially those related to lack of habitual moderate exercise and presence of late-night dinner and bedtime snacking may be associated with the prevalence of CKD.

Objective The use of intravenous in-line filters is effective for the mechanical removal of large particles, precipitates, bacteria, fungi, large lipid globules, and air. However, the routine use of in-line filters remains controversial. Many patients with hematological diseases frequently suffer from bloodstream infections (BSIs) with fatal outcomes. Methods The year before cessation of an in-line filter was defined as the "filter period" and the year after its cessation was defined as the "non-filter period." The number of central line-associated bloodstream infections (CLABSIs), which are defined through surveillance, the catheter utilization rate, the number of patient deaths within 7 days after removal of the central venous catheters (CVCs), and the overall survival rate following CVC insertion were measured. Results During both periods, 84 patients had a total of 140 CVCs with a total number of catheter days of 3,407. There were 10 CVCs with CLABSIs, and the overall CLABSI rate was 2.9/1,000 catheter days, including 4 CVCs with CLABSIs (2.5/1,000 catheter days) during the filter period and 6 CVCs with CLABSIs (3.3/1,000 catheter days) during the non-filter period. The CLABSI rate, catheter utilization rate, and mortality did not differ significantly between the two periods. The only independent variable that was found to be significantly associated with the development of CLABSIs was a neutrophil count of <500×106/L (p<0.05). Conclusion Our study revealed that the cessation of in-line filters from CVCs does not significantly influence the incidence of BSIs and mortality in patients with hematological disease. To confirm our results, however, a large-scale randomized controlled study is warranted.

Objective A lower frequency for the peroxisome proliferator-activated receptor γ coactivator 1α (PPARGC1A) Ser482 allele has been reported in elite-level endurance athletes among Caucasians, although this gene polymorphism has not been found to be associated with aerobic capacity in German, Dutch or Chinese populations. The purpose of the current study was to examine the associations between the Gly482Ser polymorphism and aerobic fitness in 112 Japanese middle-aged men. Methods The PPARGC1AGly482Ser polymorphism was identified according to a TaqMan® SNP genotyping assay. Habitual physical activity was objectively measured using an accelerometer. The lactate threshold (LT), an index of aerobic fitness, was measured based on a submaximal graded exercise test performed on an electric cycle ergometer. The association between the LT and the Gly482Ser polymorphism was assessed according to a multiple regression analysis and analysis of covariance, with adjustment for potential confounders (age, body mass index, cigarette smoking, physical activity level and regular exercise). Results A significant association was observed between the PPARGC1AGly482Ser polymorphism and LT, as carriers of the Ser482 had higher LT values than the Gly482 carriers. Conclusion The current results suggest that the PPARGC1ASer482 allele is associated with a higher aerobic capacity in Japanese middle-aged men.

After exercise training, improvement in cognitive function is associated with high parasympathetic nervous activity. However, the relationship between cardiac autonomic nervous activity and cognitive function after acute exercise may differ from that after chronic exercise, because parasympathetic nervous activity decreases with acute exercise. Here, we examined the relationship between parasympathetic nervous activity and cognitive function after acute exercise. Twelve male participants performed cognitive tasks in exercise and non-exercise conditions, with a randomized crossover design. Participants in the exercise condition ran on a treadmill for 10 min, with a running speed corresponding to a rate of perceived exertion (RPE) of 10–12 in each subject. Cognitive tasks were performed before and after both conditions. Heart rate variability during cognitive tasks was measured to evaluate autonomic nervous system activity. In the exercise condition, RPE was 11.1 ± 0.2 (mean ± SE) immediately after exercise. Exercise at RPE 10–12 improved reaction times in Go trials (from 687.8 ± 55.2 to 568.2 ± 45.9 ms, P < 0.05). In the non-exercise condition, cognitive performance remained stable throughout the experiment. In addition, parasympathetic nervous activity (high-frequency component of heart rate variability) remained low after exercise compared with before exercise (from 586.4 ± 122.5 to 372.8 ± 92.9 ms2, P < 0.05). In contrast, parasympathetic nervous activity increased in the non-exercise condition (from 516.6 ± 94.9 ms2 to 642.5 ± 85.6 ms2, P < 0.05). The present results suggest that improvement in cognitive function after acute exercise may be related to reduced parasympathetic nervous activity.

The catfish Synodontis nigriventris shows a unique habit taking a stable upsidedown posture in free water regardless of an above, one-sided illumination. This upsidedown posture can be observed when the catfish is apart from objects because the catfish usually orients its ventral side towards the water bottom or objects due to a so-called ventral substrate response. Thus, it is not easy to study the mechanism of the upside-down posture. To resolve this problem, the frequency of the upside-down posture was measured by using various sizes of vessel in which the catfish was kept. Video analysis showed that the frequency of the upside-down posture depended on the space size around the catfish. The smaller the size became, the higher the frequency of the upside-down posture became. Furthermore, the frequency of the upside-down posture depended on the shape of the vessel bottom. Curved-bottom vessels induced the upside-down posture more frequently than flat bottom. These findings suggest that a small, curved-bottom vessel is ideal for researching the upside-down postural control mechanism.

It has recently been reported that hepatitis B virus (HBV) reactivation in patients with hepatitis B surface antigen (HBsAg)-negative lymphoma during or after cytotoxic therapy occurs after the use of rituximab and stem cell transplantation for hematologic malignancies. However, clinical data on HBV reactivation in multiple myeloma patients have not been extensively reported. This is the first reported case of HBV reactivation in an HBsAg-negative myeloma patient treated with bortezomib (BOR) as salvage therapy and not stem cell transplantation. By closely monitoring HBV-DNA and early administration of entecavir, severe hepatitis was avoided and BOR therapy was continued. We suggest the importance of close monitoring of HBV-DNA for transplant-ineligible myeloma patients treated with BOR as salvage therapy. [J Clin Exp Hematopathol 52(1) : 67-69, 2012]
