Patients with severe COVID-19 often experience complications including coagulopathy and fatal thrombosis. COVID-19 pneumonia sometimes leads to acute respiratory distress syndrome, requiring extracorporeal membrane oxygenation (ECMO), during which thrombosis and bleeding are major causes of death. Anticoagulation such as heparin is essential for COVID-19 patients on ECMO; however, bleeding might be caused by not only heparin, but also acquired von Willebrand syndrome (AVWS). To date, no study has examined ECMO-related bleeding and AVWS in COVID-19 patients.We report a COVID-19 patient who experienced bleeding from AVWS in addition to disseminated intravascular coagulation (DIC) during ECMO. The level of high–molecular weight VWF multimers decreased during ECMO therapy, and these findings promptly improved after discontinuation of ECMO. Plasma levels of VWF antigen were extremely high, probably due to endothelial cell damage caused by COVID-19. On the other hand, plasma levels of ADAMTS13 activity were moderately reduced, to 20–30% of normal. The patient was successfully treated with cryoprecipitate in bleeding during ECMO without a reduction in heparin, which might have induced thromboembolism. Bleeding found in this patient might be caused by AVWS and DIC.Severe COVID-19 patients are in a thrombotic state and need to receive anticoagulant therapy. However, once they receive ECMO therapy, bleeding symptoms could be observed. In such cases, physicians should think of AVWS in addition to the side effect of heparin and DIC.

Changes in the timing of bird migration in spring and autumn in a coastal forest near the city of Niigata, central Honshu, Japan, were analyzed based on 27 years of bird-banding records. Half of the bird species studied, including all migratory types except residents, arrived or departed significantly earlier in spring due to an increase in spring temperatures. The rate of change we observed in spring migration timing due to changes in temperature was identical to or slightly greater than those reported in studies from other countries. The spring arrival of the Narcissus Flycatcher Ficedula narcissina and the Japanese Thrush Turdus cardis, both long-distance summer migrants to the nearby mountains, became earlier (advanced), however, for reasons that remained unclear. Median capture date in autumn was significantly associated with year for five species. Of these, the median capture date of the Japanese White-eye Zosterops japonicus, a resident and wandering bird, and the Black-faced Bunting Emberiza spodocephala, a wandering bird, advanced annually, while for the Japanese Robin Luscinia akahige and two other species (all long-distance migrants), it was delayed. We hypothesize that forest succession from a simple pine forest to a mixed forest with well-developed sub-canopy and shrub layers may have strongly influenced the Japanese White-eye and the Black-faced Bunting due to changes in population structure in the study area, resulting in an earlier median autumn capture date. Forest succession may also have influenced the Japanese Robin's food resources, enabling it to stay longer in the study area and resulting in a delay in autumn departure date. Thus, changes in bird migration timing differ according to different environmental factors in spring and autumn.

本稿は野党期自民党の議員立法を分析対象としている。自民党は1955年の結党以来、1993年から1994年の一時期を乃増手政権の座にあり、「野党期」といわれる時期は極めて短い。それにもかかわらず敢えてこの時期の議員立法を研究対象とするのは、官僚機構の協力を得られにくくなる野党の立場としての、いうなれば「素のままの」自民党の政策形成能力を測りたいがためである。政策形成に関する議論においては戦後から長期の間、官僚主導論が占めていたが、1980年代後半からは逆に、政官関係でいうところの「政」の側、特に自民党の政策形成能力の再評価、すなわち自民党の長期一党支配の中で「族議員」と呼ばれる特定の政策分野に精通した議員が出現し、従来官僚主導と言われた政策形成の場面において、それらをしのぐ能力を発揮しているという議論が出てきている。しかしながら、野党期自民党の議員立法、そして自民党を離党した「族議員」の野党所属期における議員立法を検討すると、自民党がそうした能力を持ち合わせているかということに対しては疑問が生じる。そこには常々「責任政党」を標榜してやまない自民党の「素のままの」政策形成能力が現われていると言っても過言ではない。

Hybrid rockets have the technical problem of a low fuel regression rate, which thereby causes a low fuel mass flow rate. Owing to recent developments in additive manufacturing technologies, using a solid fuel grain with a complicated geometrical port is one method for improving the flow rate. Star fractal geometry was employed as a complicated geometry. Combustion experiments using the grain with a star fractal port were performed. A conventional circular port was also tested for comparison. Local fuel regression rate, axial-direction-averaged local fuel regression rate, thrust, specific impulse and c* efficiency were evaluated. The local regression rate was high near the injector as a result of the impingement of injected oxidizer flow on the surface. While the axial-direction-averaged local regression rate of the star fractal port was slightly less than or almost comparable to that of the circular port, the thrust of the star fractal port was higher than that of the circular port because of the higher fuel mass flow rate. In addition, there was little difference in the specific impulse and c* efficiency when comparing the star fractal and circular ports. Therefore, star fractal geometry is superior to the circular port as the port geometry for hybrid rocket fuel grain.

We retrospectively obtained data of patient background and pretreatment characteristics from medical records and identified the predictive factors of febrile neutropenia (FN) in patients with non-small cell lung cancer (NSCLC) treated with docetaxel alone or in combination with the anti-vascular endothelial growth factor (VEGF) antibody bevacizumab. Patients were eligible for inclusion in the study if they were 20 years or older, diagnosed with NSCLC, and received docetaxel monotherapy alone or in combination with bevacizumab at the Department of Respiratory Medicine, Kobe City Medical Center General Hospital, between July 1, 2011, and March 31, 2018. Eighty-one patients with recurrent or advanced NSCLC were included. Multivariate stepwise logistic regression analysis with backward selection revealed that lower baseline Eastern Cooperative Oncology Group performance status (ECOG-PS) scores of 1 and 2 (odds ratio (OR), 5.098; 95% confidence interval (CI), 1.045–24.879, p = 0.021) and baseline platelet count below 18.8 × 104/µL (OR, 3.861; 95% CI, 1.211–12.311, p = 0.022) were significant factors influencing the FN occurrence rate. Our results demonstrated that ECOG-PS 1–2 and lower baseline platelet count were significant risk factors of FN in patients with NSCLC receiving docetaxel-based chemotherapy. Moreover, the combination of anti-VEGF antibodies and docetaxel might be associated with increased FN frequency. Despite the limitations of this study including its retrospective design, single-center site, and small sample size, baseline ECOG-PS score and platelet count may be regarded as important indices to identify patients for prophylactic granulocyte-colony stimulating factor (G-CSF) treatment before docetaxel-based chemotherapy.
