”Rootless containers” is a concept to run the entire container runtime and containers without the root privileges. It protects the host environment from attackers exploiting container runtime vulnerabilities. However, when rootless containers communicate with external endpoints, the network performance is very low compared to rootful containers because of the overhead of the user-land TCP/IP implementation called ”slirp4netns”. In this paper, we propose ”bypass4netns” that accelerate TCP/IP communications in rootless containers by bypassing slirp4netns. bypass4netns uses sockets allocated on the host. It switches socket file descriptors in containers to the host's socket file descriptors by intercepting syscalls and injecting the file descriptors, using ioctl (SECCOMP_IOCTL_NOTIF_ADDFD). We confirmed that rootless containers with bypass4netns can achieve more than 10 times faster throughput than rootless containers without it.

[Purpose] The soleus H-reflex is depressed at stimulation rates greater than 0.1 Hz. This reflex depression is referred to as postactivation depression. Postactivation depression reflects the reduced efficacy of the Ia-motoneurons synapses when they are evaluated after a previous activation. The aim of this study was to determine whether the recovery of motor functions in the lower extremities affects the PAD of the soleus H-reflex in patients with subacute stroke undergoing rehabilitation. [Subjects and Methods] Eight patients with subacute stroke patients were recruited. Postactivation depression, Fugl-Meyer score (lower-limb portion), walking velocity, the Modified Ashworth Scale, and center of pressure sway during standing were measured within three days of admission to rehabilitation and 50 days later. [Results] After rehabilitation, Fugl-Meyer scores, center of pressure path length, and walking velocity were significantly improved, and postactivation depression had significantly increased. There was a significant positive correlation between the rates of change of postactivation depression and center of pressure path length. [Conclusion] The results demonstrated that postactivation depression is partially normalized after rehabilitation in patients with subacute stroke, and suggested that the recovery in lower extremity function after stroke particularly standing stability is affected by spinal synaptic plasticity.

Background: Cardiac resynchronization therapy (CRT) is effective in reducing morbidity and mortality in systolic heart failure patients with cardiac dyssynchrony as demonstrated in studies with primarily Western populations. Although CRT devices with a defibrillator (CRT-D) became available in Japan since 2006, their efficacy remains uncertain in Japanese patients. In this prospective, multicenter study, the efficacy of CRT-D therapy in an all-Japanese population was compared with the study conducted in the US, Multicenter InSync ICD Randomized Clinical Evaluation (MIRACLE-ICD). Methods and Results: Ninety-three patients were evaluated according to the subject selection criteria of the MIRACLE-ICD study, and 80 patients were enrolled. Results at baseline and 6-month post-CRT-D implantation were compared in terms of composite clinical response (CCR) and other secondary endpoints. Quality of life (QOL) was assessed with a validated Japanese version of the Minnesota Living with Heart Failure questionnaire. CCR was improved in 55 patients (68.8%), unchanged in 14 (17.5%), and worsened in 11 patients (13.7%) (MIRACLE-ICD general phase: 62.0%, 13.4% and 24.6%, respectively). Non-inferiority was verified by 1-sided test with 10% equivalence margin. QOL score improved significantly (50.0±26.2 vs. 23.6±20.2, P<0.01). Conclusions: The MOMIJI study demonstrated that CRT-D effectiveness as assessed with CCR was non-inferior to the trials conducted outside Japan, thus suggesting that the benefits of CRT-D are similar between Japanese and non-Japanese patients.  (Circ J 2012; 76: 1911–1919)

Background:Acute decompensated heart failure (ADHF) is often accompanied by liver congestion through increased right atrial pressure (RAP). Liver stiffness (LS) assessed non-invasively using transient elastography is related to increased RAP and liver congestion in patients with general HF. We investigated the relationship of LS with clinical and echocardiographic variables and outcomes in patients with ADHF.Methods and Results:The subjects were 105 patients with ADHF admitted to hospital between October 2016 and June 2017. Patients were divided into 2 groups based on median LS at admission (low LS <8.8 kPa [n=52] vs. high LS ≥8.8 kPa [n=53]). Death from cardiovascular disease and readmission for HF were primary endpoints. Total bilirubin and γ-glutamyl transpeptidase levels, MELD-XI score, diameters of the inferior vena cava and right ventricle, and severity of tricuspid regurgitation were greater in the high LS group (all P<0.05). During a median (interquartile range) follow-up period of 153 (83–231) days, cardiac events occurred in 29 patients (54%) in the high LS group and in 13 (25%) in the low LS group (P=0.001). After adjusting for variables that influence organ congestion, a high LS ≥8.8 kPa was still significantly associated with cardiac events (all P<0.05).Conclusions:Increased LS measured by transient elastography reflects RAP elevation, hepatic congestion, and hepatic dysfunction. LS upon admission may be a useful prognostic marker in patients with ADHF.

In the TST-2 spherical tokamak device, we carried out a fully non-inductive current startup experiment by Landau damping of the Lower Hybrid Wave (LHW). Capacitively Coupled Combline Antennas (CCCAs) were used for wave injection. The antennas are located on the outboard side and the top side of the vacuum vessel, and by reversing the toroidal magnetic field, it is possible to simulate the case of wave injection from the bottom side. The highest plasma current of 26.7 kA was achieved by top injection with the reversed toroidal magnetic field. According to numerical calculation using ray tracing and Fokker-Planck codes (GENRAY/CQL3D), the downshift of the parallel wavenumber helped the tail of the electron velocity distribution extend to higher energy than the other cases. Additionally, in order to evaluate the directionality of the wavenumber spectrum which is also important for efficient current drive, a finite element solver (COMSOL) was used. In order to avoid deterioration of the wavenumber spectrum, one limiter of the outboard antenna should be moved away toroidally by 70 mm from the current position, and the preferred distance between the antenna and the cutoff density layer is about 2 cm.
