We evaluated heparinoid-containing soft ointments and heparinoid-containing creams to develop information useful to select drugs. Spreadability, consistency and the water content of stratum corneum was measured. A selection table focusing on impression of the use was prepared. Spreadability of the semi-original soft ointment and cream were lower than those of all generic drugs. Regarding hardness, one generic soft ointment was softer than the semi-original drug, but the semi-original cream was harder than all generic creams. Regarding the impression of the use, a significant difference was noted in smell in soft ointments, and 6 items in creams. Correlation was found between the spreadability and consistency test results and impression of the use. A significant difference was noted in the water content of stratum corneum in creams. The selection table prepared based on the impression of the use may provide information that helps selection of preparations in clinical practice.

Seven products, currently on the market as acetaminophen suppository 100 mg, are commonly used for pyrexia in children, and yet there have been no reports on their solubility and divisibility. We examined the hourly solubility rate of each suppository using a dissolution tester for suppositories. In addition, we conducted a hardness test using the EZ-test, a questionnaire-based investigation on the accuracy and divisibility when the suppositories were divided. There was a significant difference between each suppository (P < 0.01). While 4 types dissolved completely within 60 minutes, one type took 120 minutes to dissolve completely. Another two had only 51 and 67%, respectively, at 180 minutes. The bases are hard fat in all suppositories. The types and additives of hard fat may have had an influence on solubility. The hardest suppository was assessed as showing marked differences in the weight ratio and solubility between the distal and tail portions when divided in half, due to which division was considered inappropriate, and there were differences in accuracy and divisibility among suppositories.From the above, it was suggested that the acetaminophen suppository 100 mg products may show differences in the solubility, time to action onset, and duration of the action. In addition, we should instruct physicians on important points of caution regarding the characteristics of each preparation when dividing the suppositories so that they can be given safely to infants.

先発医薬品と後発医薬品では添加物が異なるため,溶解性が異なる可能性が考えられる.そこで本研究ではコールター原理に基づいた自動細胞計数装置を用いて,バンコマイシン塩酸塩点滴静注用5製剤をそれぞれ生理食塩液に溶解し振とうした際の不溶性微粒子数を測定し,各製剤の溶解性を比較検討した.各振とう時間において5製剤の不溶性微粒子数には有意差が認められ,1製剤は先発医薬品よりいずれの時間においても有意に低値を示した.他の4製剤は振とう時間を長くすることで,有意に不溶性微粒子数は低値を示した.したがって,製剤毎の溶解性には差があり,溶解時間が異なることが確認された.バンコマイシン塩酸塩点滴静注用を適正に使用するには,溶解性の差を理解し,適切な溶解時間を現場の医療スタッフに周知徹底する必要がある.

慢性疾患や急性疾患により、治療薬を服用しながら自動車運転をしている患者は多いが、服薬による交通事故の実態は不明である。今回、医薬品医療機器総合機構の医薬品副作用データベース(JADER)を用いて医薬品による交通事故の実態について調査した。2004年4月〜2016年3月において、JADERの総登録件数は390,669件で、有害事象としての交通事故は342件であった。被疑薬176薬品のROR(Reporting Odds Ratio)および95%信頼区間(CI)下限値を算出した結果、82薬品においてCIの下限値が1を超えており因果関係が示唆された。被疑薬としては、プラミペキソール塩酸塩水和物が47件と最も多く、そのうち36件が突発的睡眠が併発有害事象であった。次いでゾルピデム酒石酸塩46件、プレガバリン35件、バレニクリン酒石酸塩19件、スルピリド13件等であり、添付文書に自動車運転等に関する警告や禁止が記載されている医薬品が主であった。高齢化が進み、自動車運転に対する療養指導、服用すべき医薬品に関する適切な服薬指導は益々重要となる。自動車運転をする患者に対しては、医師や薬剤師が適切にアドバイスする必要があり、その際に、本調査結果は有用な資料の一つとなると考える。

Stomatitis frequently occurs during chemotherapy and radiotherapy for cancer. Because of its pharmacological properties including anti-inflammatory activity and stimulatory effects on endogenous prostaglandin synthesis, rebamipide has been suggested as a potentially effective treatment against stomatitis. In the present study we tested the stability of oral rebamipide solutions prepared in our hospital pharmacy using sodium alginate as a thickener to increase retention of this agent in the oral cavity, and the addition of different flavoring mixtures intended for use in enteral diets to reduce the bitterness of rebamipide and sodium alginate. Samples of oral rebamipide solution prepared with 13 kinds of flavoring and sodium alginate were evaluated in terms of their appearance, redispersibility, pH, viscosity, and rebamipide content immediately after preparation and 1, 3, 7, and 10 days after storage at room temperature under ambient light or in a cool, dark place. After 10 days of storage, favorable stability was observed in four sample solutions supplemented with green apple, pineapple, yogurt, and tomato flavoring mixtures intended for use in Elental® diets. These oral solutions may have potential clinical application.

陸前高田地域再生支援研究プロジェクトは、東日本大震災において岩手県で最も甚大な被害にあった陸前高田市において、被災住民自身が地域の再生、生活再建に向けてその課題を話し合い、主体的な取り組みを行うことを支援してきている。そして、仮設住宅および被災地域におけるコミュニティの形成のあり方を共に模索しながら、今後の復興における地域再生のモデルづくりに寄与することを目的として、今日まで活動を続けている。本プロジェクトは、上記に関する活動の一環として、2015年8月に、2011年から引き続き5回目となる市内・外合わせて48の仮設住宅団地の自治会長等へのインタビュー調査を行っている。本稿は、仮設住宅自治会長等に対するインタビュー調査結果等についての概要を記したものである。内容としては、居住5年目を迎えた仮設住宅団地における①転出・転入、空き住戸等の居住状況、②高齢者や子どもなど配慮が必要な人の状況、③住環境、生活環境の問題と対応、④自治会活動とコミュニティ形成の状況、⑤外部支援団体の関与の状況、⑥住宅再建・復興まちづくりに関する情報や意見等についてであり、それらの全体的な概要と各9地域の特徴について整理している。調査時点において震災発生から約4年半が経とうとしており、仮設住宅での暮らしが長期化する中、2014年末から一部災害公営住宅への入居が始まり、また、高台への移転が開始されてきており、住宅再建が目に見えてきた地域と、大規模な土地のかさ上げによる区画整理事業の完成時期が明確でなく、なかなか将来の展望が目に見えない世帯が少なからずあり、昨年度に比べて世帯・地域間格差の広がりが見られ、今後の支援のあり方が問われる。本稿で記した概要に加えて、各仮設住宅団地のデータの詳細を報告書としてまとめ、仮設住宅団地自治会長、行政、市議会、支援団体等広く関係者に送付し、今後の復興施策へのフィードバックを図っている。
