A questionnaire survey of inpatients of widely varying ages was conducted to investigate the ease of taking internal medicines and desired dosage forms.A similar questionnaire survey of nurses and pharmacists working in a university hospital was also conducted to compare awareness of dosage forms by profession.The results of this survey clearly demonstrated the difference in awareness of patient preference regarding internal medicines among pediatric nurses,nurses working at other hospital wards,and pharmacists.This discrepancy seemed to reflect the difference in awareness of different health care workers concerning impairment of deglutition capability with aging.Awareness of patients' desired dosage forms also varied between pharmacists and nurses,which could be due to the difference in knowledge of pharmaceutics between these 2 professions.Based on the results of this study,it was felt that pharmacists should provide medication instruction to individual patients and provide pharmaceutical information to nurses,and that information sharing is important to ensuring that proper pharmaceutical care is conducted in the hospital.

ひと昔前,CMにも出てきたこの言葉の意味について検索してみた.英語では"honest injun"と表現し,Marc Twainの"Adventures of Tom Sawyer"で使われているが,起源は更に古いと考えられていることが分かった(www.alc.co.jp;アルクより).また,その背景としては「ネイティヴアメリカンにとって言葉は神聖なもの,絶対的なものであり,言葉をもって他人と約束をするということは神と約束を交わすのと同等で,それを破ることは神を欺くことになる.」という概念にあるとされているのが一般的な解釈である.私は学生時代,大切な恩師からこの言葉を幾度となく聞いた.それはゼミ中だったり,酒宴の席だったり様々であったが,現在でも事あるごとに思い出される貴重な言葉となっている.

Beraprost sodium (BPS) is often used for pediatric patients with pulmonary hypertension. The purpose of this study was to determine the expiration date of the powdered medicine prepared by grinding tablets. In the present study, the hygroscopicity and stability of the beraprost tablet (DORNER® tablet), ground Dorner tablet and powder formulation (Doener powder) consisting of the ground DORNER® tablet and lactose (EFC® lactose) were investigated after storage at various relative humidities (RHs) and light exposures. While the DORNER® tablets and ground DORNER® tablets were found to adsorb significant amounts of water vapor at an RHs of greater than 51.0%, Dorner powder scarcely adsorbed water. The stability of BPS in the Dorner powder decreased after storage under 3000 lux for 90 days. From these results, the expiration date and storage conditions of Dorner powder were determined to “90 days without exposure to light.” We also investigated the stability of BPS in solutions of various pH values on the assumption that Dorner powder may be given to pediatric patients after dissolving in soft drinks. Because BPS degraded significantly below pH 2, pharmacists should alert patients not to take Dorner powder with acidic soft drinks.

今夏,筆者は夏季休暇を利用してアメリカのミネソタ州に1週間滞在した.多くの方々から「なぜミネソタなの?」とたずねられたが,ミネソタは筆者が留学のため家族とともに2005年から2006年にかけて過ごした場所であり,日本語補習校を訪問すること,留学先のボスに最近の研究状況について報告すること,妻が在籍したラボが新築移転したのでそれを見学しに行くこと,そしてアメリカ国内で開設した銀行口座を閉じることなどを目的とした旅行であった.

Osteoporosis is more common in elderly people and oral bisphosphonates are widely used for the treatment of osteoporosis throughout the world. It is generally considered that elderly people decline in physical function with age and find the taking of drugs to be problematic. We conducted this survey to clarify potential problems in osteoporotic patients during treatment with oral bisphosphonates. The survey of an 8-item questionnaire was performed in 32 pharmacies in Hiroshima prefecture. A total of 684 patients were enrolled in the survey, 637 patients were analyzed and 47 patients with protocol violation were excluded from the survey. Baseline characteristics were as follows: gender (female, 581; male, 56), age (< 70 years old, 26%; 70 years old ≤, 74%), administration interval (daily, 42; weekly, 313; monthly, 282). The patients with dry mouth accounted for 14% of total patients and those with experience of discomfort during swallowing accounted for 11%. Moreover, 40% of total patients had experience of forgetting to take bisphosphonates. Our survey demonstrated that osteoporotic patients taking bisphosphonates experienced several hurdles and did not complain directly to pharmacists about it. We need to communicate better with osteoporotic patients and collaborate closely with physicians, nurses and others.

Regarding the management of blood preparations, since September 1995 the Ministry of Heath, Labour and Welfare has had to make and/or to keep documentary records such as the product name, date of dispensing, the patient's name and lot number and so on. At Yamanashi Medical University Hospital, to cope with deleterious accidents caused by blood preparations, we developed a new system for the management of blood preparations. This system consists of a printing system for confirmation labels for blood prescriptions and a computerized management system concerning the medication. In this study, the effect of this new management system on the rational use of blood preparations was evaluated. Compared with the former system, the time required to dispense blood preparations decreased from about 500 to 170 seconds per case. Furthermore, based on a questionnaire to medical doctors and/or nurses, it appeared that this system could reduce the time required to manage blood preparations and improve the rational use of injectable drugs.

The stability of powdery dosage forms of drugs prepared by unsealing the packages was investigated. For 247 articles purchased by the Yamanashi Medical University Hospital, the variables related to the evaluation of stability after unsealing these packages and investigating the contents based of an “Interview Form”. A longterm preservation test was carried out on 153 items (94%), and the mean length of the test was about 36 months. The acceleration and the severity tests were carried out using 17 items (10.4%) and 114 items (69.9%), respectively. The mean conditions of the acceleration test was 40°C/6 months, which agreed with the guidelines proposed by the “Drug Manufacturing Guidelines”. On the other hand, for the severity test, certain conditions for the evaluation could not be arranged, and testing was carried out under various conditions. In order to improve the efficiency of dispensing and the quality of life for patients, a further increased standardization of the testing conditions is recommended.

Di-2-ethylhexyl Phthalate (DEHP) was reported to dissolve from a polyvinyl chloride (PVC) administration tube during the infusion of an injection fluid containing a solvilizing agent, soybean oil or lecithin, etc. This paper describes the behavior of DEHP dissolution from PVC tubes during enteral tube feeding. The DEHP concentration was determined using HPLC. After the passage the ENSURE LIQUID® (EL) solution through the PVC tube, DEHP was dissolved in the EL solution. The EL solution is one type of enteral nutrition. This dissolution pattern was similar to that of injection fluid containing a solvilizing agent, in which the DEHP concentration in the EL solution increased linearly over time at first and thereafter reached a plateau. 170 μg of the DEHP was dissolved after 200mL of the EL solution had passed through the PVC administration set (the drip conditions were 25°C, 100mL/hr, tube length 115cm and inner diameter 0.3cm). The amount of DEHP which dissolved into the solution depended on the temperature (25-40°C), and no clear difference was observed between the PVC tubes (administration tube, feeding tube). In addition, the amount of DEHP that dissolved into the solution varied depending on the type of enteral nutrition.
