症例は55歳女性.19歳時,下垂体腺腫摘出術を施行.術後,ホルモン補充療法が行われたが,数年後に治療中断.48歳時,糖尿病を指摘されたが放置.55歳時,再度HbA1c 10.8%とコントロール不良の糖尿病を指摘され当科入院.入院時,高脂血症,肝機能障害の合併および,内分泌検査で成長ホルモン分泌不全,中枢性性腺機能低下症,甲状腺機能低下症を認めた.画像診断では,肝の変形,脾腫,脾腎シャントを認めた.肝機能障害の原因は,ウイルス,自己免疫,アルコール性は否定的であり,肝生検を施行し非アルコール性脂肪肝炎(non-alcoholic steatohepatitis: NASH), 肝硬変と診断した.血糖コントロールは超速効型インスリンの投与で改善した.本症例は下垂体腺腫摘出術後GH分泌不全状態にあったが,長期間にわたりホルモン補充療法が行われなかったため,NASHを発症し,肝硬変まで進展したと推測された.

症例は65歳,男性.2008年6月21日,関節痛に対して近医でプレドニゾロンを処方された.その後口渇,多飲,多尿が出現.8月10日に意識レベルの低下あり当院受診.JCSI-2,血糖値1023 mg/dl,尿ケトン体(4+),血中ケトン体高値,血液ガス分析でpH 7.269, Base Excess -20 mmol/lであり,糖尿病ケトアシドーシス(DKA)の診断で即日入院となった.輸液とインスリン持続静注でアシドーシス,高血糖は改善.翌日に血清アミラーゼ,リパーゼ値の上昇を認めた.腹部CTで急性膵炎,腹部大動脈血栓,右腎梗塞の所見.膵炎に対して絶飲食としメシル酸ガベキサートを投与,血栓・腎梗塞に対してヘパリン,アルガトロバン,ワーファリンを投与し軽快した.経過中腹痛は認めなかった.退院約9ヵ月後に施行した75gOGTTは正常型であった.

Brainstem disorders are often characterized by abnormal eye movements, however, the type of abnormal movements vary depending on the localization and severity of the damage. Here, we report a case of pontine hemorrhage who exhibited horizontal conjugate gaze palsy and disorder of speech discrimination. The 34-year-old man presented with eye pain and dull feeling malaise?, disorder of speech articulation and torpor paralysis ?? of the left side of the body of sudden onset, that developed while he was driving a car. Clinical examination revealed horizontal conjugate gaze palsy, both rightward and leftward, while vertical gaze and convergence were normal. Although the pure-tone audiogram and DPOAE were within normal range, ABR showed prolongation of latency, and reduction of speech discrimination was recognized. MRI revealed evidence of hematoma and edema extending from the mid-dorsal aspect to the right side of the pons. Based on the findings, the patient was diagnosed to have pontine hemorrhage due to hypertension affecting the right facial nerve nucleus, right pyramidal tract, bilateral abducens nuclei, MLF, PPRF and the auditory neural pathway. Although the symptoms of pyramidal tract and auditory disorder resolved by day 19 when he was?? moved to another hospital, the other neurological symptoms were persistent.

The purpose of this paper is to make a empirical analysis of the adjustment speed of the bank loan rate. The crucial changes occurred in recent Japanese loan market. The previous studies showed that the adjustment speed of the bank loan rate increased. They estimated the parameters of a linear regression system obeying different regimes. They have a problem in that the regressions arn likely to be contaminated with observations from the other regimes. This paper circumvents this problem by a finding the switching points in time using the simulation method.
