The influenza virus causes seasonal influenza epidemics and, from time to time, worldwide pandemics. Seasonal influenza epidemics in winter cause increased morbidity because vaccination is aimed at preventing influenza. However, influenza treatment is limited because oseltamivir cannot be used in children. Recently, Kampo medicine containing maoto was reported to have anti-viral and antipyretic effects. In Japan, maoto is generally administered as concentrated extract-granules. However, children do not like to take the maoto extract-granules because of their distinctive smell and bitter taste. Therefore, masking the smell and bitter taste is necessary for administration to children who are unable to take oral medications. In this study, we developed and characterized a new chocolate formulation for masking the maoto extract, using human sensory tests.The chocolate bases A to J (10 forms) were selected from commercial products. The total evaluation score for the maoto extract-containing chocolate formulation-D (Mao/nC) was the highest of the 10 forms according to the human sensory test results. The maoto extract-containing adjusted chocolate formulation (Mao/OrC) was based on an adjusted version of the Mao/nC formulation. The Mao/OrC exhibited improved smell and sweetness and overall evaluation scores compared with the Mao/nC, according to the human sensory test results. The results of a dissolution test, drug release test, and viscosity and osmotic pressure of the Mao/OrC were improved compared with those of the Mao/nC, and those of bioequivalence were similar. These results suggest that the Mao/OrC could be used as a new masking chocolate formulation for preparation in hospitals.

A family history of diabetes and genetic predisposition are established as risk factors for diabetes mellitus, lifestyle factors also play an important role in the etiology of diabetes. Alcohol consumption may be related to risk for type 2 diabetes mellitus (type 2 DM) through effects on insulin secretion and sensitivity. Several large-scale epidemiological studies have suggested an inverse association between moderate alcohol consumption and reduced risk for type 2 DM. We assessed whether or not moderate alcohol consumption is associated with DM.Among 2, 338 men, 150 cases of incident DM were newly identified by means of an oral 75 g glucose tolerance test. The newly diagnosed DM percentages of drinking 0 g/day, 1-9.9 g/day, 10-29.9 g/day, 30-49.9 g/day and ≥50 g/day were 8 .3, 6.3, 5 .1, 5.2, 7.2, respectively. The newly diagnosed DM percentage of drinking 1-9.9 g/day had a significantly lower risk than 0 g/day. The frequency of alcohol consumption was significantly inversely associated with diabetes ; a frequency of greater than 6 times per week showed a significantly lower risk than 0 times per week. HbA1c of drinking 10-29.9 g/day, 30-49.9 g/day, and 5≥0g/day were significantly lower than that of 0 g/day.These data suggest that light moderate and frequent alcohol consumption have a decreased subsequent risk of diabetes mellitus.

野菜サラダ(キャベツ,オリーブ油,酢)と米飯の摂取順序を変えた時に食後の血糖値とインスリン値のプロファイルがどのように変化するのかを確認するため,10名の健常成人において試験を行った.その結果,米飯摂取後に野菜サラダを摂取した場合と比較して,米飯摂取前に野菜サラダを摂取した場合には,食後20, 30, 45分での血糖上昇値(ΔC)は有意に低下し(p<0.01),最高血糖値(ΔCmax)に到達する時間は約40分遅延した(p<0.01).ΔCmaxは平均21%低下し,食後0∼120分までの血糖値上昇曲線下面積は,平均39%低下した.血清インスリン値は血糖値とパラレルに推移し,食後のインスリン分泌が節約できる可能性が示唆された.以上より,野菜サラダは米飯よりも先に摂取するほうが食後の血糖上昇を抑制するために有効であることが示された.

The purpose of this study was to evaluate the physicochemical properties of piperine (PP) in ground mixtures (GMs) of PP with α-, β-, or γ-cyclodextrin (CD) under conditions of humidity, heat, and humidity-heat. In solid-state fluorescence measurements, the fluorescence maxima for GM (PP/αCD = 1/2), GM (PP/βCD = 1/1), and GM (PP/γCD = 1/1) were observed at 463, 472, and 469 nm, respectively. On the other hand, the humidified GMs exhibited maxima at 454, 460, and 465 nm, while the humidified-heated samples displayed fluorescence maxima at 455, 455, and 469 nm, respectively. Therefore, the molecular behavior of PP with α, β, and γCD was concluded to vary upon the coordination of water molecules. NIR and solid-state fluorescence measurements revealed that the molecular behavior of PP inside the α, β, and γCD cavity changed by water and heat factors depends on the mobility of the methylenedioxyphenyl group.This is an open access article distributed under the Creative Commons Attribution License, which permits unrestricted use, distribution, and reproduction in any medium, provided the original work is properly cited.

本研究は, 異なる配合率のモチ性の大麦品種「キラリモチ」混合米飯の摂取が米飯と比較して食後血糖値の上昇を配合率依存的に抑制するかとGlycemic Index (GI) が配合率依存的に低下するか検証するため実施した。対象は健常な成人10名 (男性6名, 女性4名) とした。糖質50 g分の米飯 (基準食) とモチ性大麦の配合率が30, 50, 100%の大麦混合米飯 (試験食) のそれぞれを摂取し, 摂取前および摂取開始から15, 30, 45, 60, 90, 120分後の血糖値を測定した。血糖値, 血糖値上昇曲線下面積 (IAUC) , Glycemic Index (GI) について解析した。大麦の配合率が増加するほど血糖値とIAUC0-120が低下した。GIは, 基準食を100とした場合, 大麦配合率30, 50, 100%でそれぞれ, 79.7, 67.5, 48.3であった。モチ性大麦混合米飯には, β-グルカン量に依存して米飯と比較して血糖値の上昇を抑制する効果が示唆された。

In 2012, the external prescription rate was 66.1% of the national average. A dispensing doctor is recognized by the escape clauses of Article 22 of the Medical Law, Article 21 of the Dentist Medical Law, and Article 19 of the Pharmacists Act. In this study, the medicine inventories of dispensing doctors and pharmacies were compared. The medicine supplies of 7 dispensing doctors and 11 pharmacies in Saitama were classified according to the medicinal effects. We also investigated the conditions in which high-risk medicines, poisons, or drugs were stored. The average number of medicines in the medicine inventory of a dispensing doctor was 262.3 (range : 99, 439), whereas the average number of medicines in the medicine inventory of a pharmacy was 1179.7 (minimum, 275 ; maximum, 1980). Further, among these medicines, there were an average of 41.0 high-risk medicines (minimum, 18 ; maximum, 76) in the inventory of a dispensing doctor and an average of 176.7 high-risk medicines (minimum, 5 ; maximum, 299) in the inventory of a pharmacy. In addition, poisons (average, 0.3) and narcotics (average, 0.9) were found to be stored by dispensing doctors. The study results revealed that pharmacies as well as dispensing doctors stored high-risk medicines. The dispensing doctor may be indirectly associated with critical medical accidents to need cross-check by pharmacist exceedingly high-risk medicine. Thus, for patients to use medicines appropriately and for them to be reassured of the safety of medicines, only professionals such as pharmacists, rather than doctors, should dispense medicines.
