The effect of acupuncture in sensory paralysis was evaluated in 43 patients. Thiry-two cases in the study group were after mandibular third molar extraction and 11 cases by postoperative injury of orthognathic surgery. Treatment was performed in four forms : needles only (A), A with moxibustion (AK), electrical needle stimulation (ESA) using LEP 4000 OhmPulser Ra direct current 6 V 5-100 Hz, and ESA+A : exercise therapy was also given 1 or 2 times weekly. There were, however, some variations depending on the patient's conditions.Visual analog scale (VAS) measurement was carried out in addition to the estimation of effectiveness.The group receiving ESA+A seemed to improve better than those receiving other forms of treatment.The results indicated that young patients less than 40 years old tend to improve with fewer treatment times of shorter duration.With respect to the relationship between the effect and the time of starting treatment, we noticed that the earlier the treatment is started, the better the improvement.In summary, the study indicated that acupuncture is useful in treating sensory paralysis.

The purpose of this study was to clarify the following : 1) effect on the elasticity of the musculus trapezius caused by psychological load, and 2) relationship between psychological load, muscle elasticity, and the autonomic nervous system (ANS) by frequency analysis of heart rate variability.The experiment was conducted on a group of 30 healthy volunteers, consisting of 16 males and 14 females with an average age of 31.2 years old. We investigated and compared the changes in muscle elasticity and ANS in the case of control (resting), phonating and mental calculation as psychological load. Psychological status and stress response were measured by using State-Trate Anxiety Inventory (STAI), Stress Checklist (SCL), and Stress Tolerance Checklist (STCL) .As a result, the subjects experienced effective acute emotional stress during mental calculation, and muscle elasticity increased significantly.Heart rate (HR), low frequency (LF) component, and LF/HF increased significantly during mental calculations. The high frequency (HF) component tended to decrease during mental calculations.The rate of elevation of muscle elasticity and HR (difference between at rest and when calculating) was significantly related to STAI and STCL. STCL was significantly related to state of anxiety. The results of this study suggest that anxiety and stress tolerance affect muscle elasticity.

乳歯環状う蝕とその歯垢中のレンサ球菌, 特にStrep. mutansとの関連性を追求するために3歳児の無う蝕児29名と環状う蝕歯牙保有児30名について歯垢中のレンサ球菌並びに歯垢の酸産生力の比較を行ない, また両者の関連性について検討した。その成績は, 歯垢中の全レンサ球菌数の平均値は環状う蝕児群は無う蝕児群より極めて高い値を示した。また, Strep. mutansの検出率並びに全レンサ球菌に対する比率において, 環状う蝕児群は有意に高い値を示し, Strep. sanguisでは逆に無う蝕児群の方が高い値を示した。歯垢を糖加培地に培養した場合に環状う蝕児群の方が強い酸産生力を示した。またう蝕の程度並びに歯垢の酸産生力とStrep. mutansの比率および菌数との間に正の相関々係がみられた。歯垢を培養した糖加培地にはStrep. mutansの増殖とともにpHの低下がみられた。

In 1999, we gave a Questionnaire on “having a family dentist” to 414 workers (300 males, 114 females) at a government office. Oral health examination was also provided to the same subjects.The following results were obtained:1. Female workers (43%) had a family dentist more often than male workers (29%), and the difference was significant (p<0.01)2. About 45% of the subjects received regular dental checkups at least once a year; however, half of them did not have a family dentist.3. Convenience, comfort, and communication factors were the main reasons for choosing a particular dentist, while continuity, comprehensiveness, and specialty factors were not taken into consideration in choosing a dentist.4. The persons who had family dentists had more FT and DMFT, and fewer total numbers of teeth than those without family dentists.From these results, it was suggested that people visited dentists with dental problems and received treatment repeatedly, and then they came to have family dentists. The habit of visiting a dentist regularly seeking checkups and prevention is not yet popular among Japanese adults.To improve oral health consciousness and to promote oral health, it should be recommended to receive regular checkups and oral health education by a family dentist. However, the present condition does not allow this system. Therefore in Japan, it should be encouraged to provide oral health examination and oral health education regularly at work sites, to improve the oral health of the adult population.

The threshold levels of taste perception on the right and left side of the tip, root of tongue and soft palate in 60 healthy volunteers were measured by means of an electrogustometry test and a filter-paper disk taste test to confirm the following hypotheses : 1) the threshold on the tip of the tongue was lower than those of the root of the tongue and soft palate, 2) there was a different threshold level between sweet, salty, sour and bitter tastes on the tip of the tongue, 3) some personal properties influenced the person's threshold. Measured threshold levels were compared between these six locations with multiple comparisons. Factors influencing the threshold level on the tip of the tongue, thereafter, were investigated using multivariate logistic regression models. Those factors included sex, age, serological, psychological and behavioral factors. As a result, the thresholds levels on the tip of the tongue were significantly lower than the root of the tongue and soft palate. The thresholds for sweet and bitter taste on the tip of the tongue were also lower than those for salty and sour taste. Increase of age, male, serological increase of zinc and usual alcohol drinking were identified as factors of increase for several thresholds, and increase of anxiety was identified as a factor of decrease for two thresholds of four tastes (sweet, salty, sour and bitter) on the tip of the tongue in the logistic regression models. Increase of depression, mouth breathing, habitual tonus of the tongue and smoking did not appear to be influential factors.

A simple, broadly applicable, standardized dysphagia severity scale would be useful to standardize dysphagia evaluations, to monitor recovery and efficacy of treatment and to study the consequences of dysphagia. We developed a global Dysphagia Severity Scale (DSS) from videofluorographic swallowing studies which included subjective clinical ratings of functional swallowing. We rated laryngeal penetration/aspiration (P/A) and pharyngeal retention (PR), and then assessed methods for scoring severity of P/A and PR and for combining them in a global scale. Each method was tested by correlation with overall clinical severity ratings. The highest correlations were found by : 1) scoring P/A based on which foods were aspirated (no aspiration or penetration ; penetration only ; thin liquid aspiration ; thick liquid, pudding, or chewed solid food aspiration ; 2) scoring PR based on how much food was retained (none ; minimal ; moderate ; severe) ; and 3) combining P/A and PR by taking the higher of the two scores as the final DSS rating. The final global DSS score and the clinical severity rating correlated well (r=0.71) . This proposed scale shows promise for rating global dysphagia severity.

The clinical analysis of 462 patients (154 men and 308 women) with problems of implant therapy done at other clinics is reported. They visited the clinic for oral implant of the Tokyo Medical and Dental University Hospital during the period from January 1996 to March 2002. The results were as follows1) The majority of the patients (76.6%) were 40 to 69 years of age.2) Of the total patients, 26.8% had been referred from the other outpatient clinics in our hospital, 14.7% from private dental offices and 2.6% from attending doctors.3) There were 361 patients with complications related to the implant therapy. Seventy-six patients asked for second opinions concerned about a diagnosis or treatment recommendation. Thirty patients required maintenance of the implants.4) The most common complications observed were peni-implantitis (184 cases) . Prosthodontic compli-cations (72 cases) included framework/resin/veneering material fractures, screw loosening and fractures. There were implant fractures in 21 cases, sensory disturbance in 20 cases, problems related to sinus in 13 cases, problems of connected teeth in 30 cases and phonetic/esthetic complications in 10 cases.5) The types of problem implants were osseointegrated implants (196 cases), alumina-ceramic implants (74 cases), metallic blade type implants (35 cases), pin type implants (10 cases), subperiosteal implants (6 cases) and other implants.

歯の硬組織とくにエナメル質が人体組織の中で最も硬い組織であることはすでに周知のことであるが, その微細構造と硬さの関係についてはまだ充分明かでない問題が多い。本研究は最新の超微小硬度計であるハネマン式微小硬度計を使用して, 歯の硬組織の微細構造と硬さの関係をしらべたものである。本実験では, 新鮮な健態抜去歯約200例について, その肉厚切片標本をつくり, その標本の微細構造を顕微鏡下に直視しながらビッカース・ダイヤモンド圧子を圧接して, その構造部分の硬さ (VHN) を測定する方法を採用した。その場合, ダイヤモンド圧子への負荷々重が小さい (59) ので圧痕も小さく (約6μ程度) , したがって従来は測定できなかったエナメル叢自体の硬さの測定も可能であった。実験の結果, 次のような知見が得られた。1) 人の歯のエナメル質の硬さ値 (VHN) は, 200~307, エナメル叢の硬さ値 (VHN) 80~180, 象牙質の硬さ値 (VHN) 43~68であった。これらの硬さ値に差異があるのは歯質の石灰化度に差があるためであり, 同一歯牙のエナメル質でも部位によって硬さの差があった。例えば唇側より舌側が, 近心側より遠心側が硬い値を示す例が多かった。これは桐野のいう同一組織でも低石灰化帯と過石灰化帯があることを硬さの面からも実証したことになる。なおレチウス条の構造上からくる切断面の硬さの差異も明かにすることができた。従来, 不明であったエナメル叢自体の硬さを測定した結果, 他のエナメル質より軟かく, ここが低石灰化部分であるという説を裏付けた。歯質の硬さと個人差, 性別差, 歯種差との関係では特性的な有意差は認められなかった。しかし歯質の硬さと年齢差の関係では注目すべき現象が見出された。はじめ低年齢では増齢とともに硬さが高くなるが, 図12, 13, 14で示したように, エナメル質では30歳台, エナメル叢では40歳台, 象牙質では50歳台で一度硬さが減じ, 再び増齢とともに硬さが高くなる傾向が見られた。この現象が人体の代謝と関連があるかどうかはまだ明かでないが, 極めて興味深い問題である。

The influences of cusp inclination on the stability and the function of removable partial dentures were evaluated on 5 subjects with the maxillary unilateral free-end saddle. An experimental removable partial denture was made with six pressure sensors fixed on to the basal surface. The artificial teeth were replaceable by attachment. Several metal maxillary canine risers were made to change the inclination of the lateral gliding movement. During lateral gliding, the canine risers and the artificial teeth were contacted with the opposing mandibular teeth simultaneously. The pressures exerted upon the supporting tissues and the mandibular movements were measured during mastication with each artificial teeth of different cusp inclination. The results were as follows: 1. The steeper cusp caused the reduction of the peak pressures as well as the total pressures in most measuring spots. 2. The time differences of the first and the peak pressure detection between 6 measuring spots extended as the cusp became steeper. 3. The mandibular movements were influenced by the cusp inclination especially in the subjects with tear-drop type masticatory strokes. 4. It was suggested that the moderate cusp of artificial teeth contributed to the denture stability in function.

The purpose of this study was to investigate the changes of the activity of the sternocleidomastoid (SCM) muscles during sustained voluntary clenching. Ten healthy male subjects without any occlusal functional problems were asked to clench as long as possible in the intercuspal position while keeping the electromyographic activity of the masseter muscle at the 50% maximum voluntary contraction. Frequency analysis was carried out by computer using a fast Fourier transform algorithm to obtain the power spectrum of the SCM muscle during the fatiguing process and the recovery process. The results were as follows:1. Sustained activities of the SCM muscles were observed during sustained voluntary clenching.2. The power spectra of the SCM muscles significantly shifted to a lower frequency as time elapsed.3. The power spectra of the SCM muscles obtained three minutes after relaxation recovered to those of the beginning of clenching.These findings indicated that muscle fatigue may be induced in the SCM muscle during sustained voluntary clenching and that electromyographic power spectral analysis can be used as a noninvasive, objective, and quantitative index of SCM muscle fatigue.

顎口腟機能と密接に関連した舌は, 咀嚼は勿論のこと発音運動においても重要な役割を果たしている。そこで舌の機能的進化の実態を解明する目的で, 舌運動との関連において, ヒトを含む霊長類の舌の筋構築と筋紡錘分布の変遷を比較調査した。ツパイでは, 舌筋構築は比較的単純で, 舌に筋紡錘は存在しない。スローロリスでは, 上縦舌筋が舌の正中部に限局した小線維束として配列し, 筋紡錘が頤舌筋に1個出現した。ニホンザルでは, 舌筋構築はより複雑となる。上縦舌筋の発達は悪いが, 頤舌筋はよく発達している。筋紡錘は, 頤舌筋に94個, 茎突舌筋に8個, 舌骨舌筋に6個と外舌筋に多く分布し, 上縦舌筋には6個, 横舌筋には8個と内舌筋にも出現し, 総数61個の筋紡錘が出現した。ヒトでは舌筋構築はきわめて複雑で, 上縦舌筋は舌背部から舌外側縁に広く発達し, 筋紡錘は上縦舌筋に159個, 横舌筋に79個, 下縦舌筋に22個, 垂直舌筋に8個と内舌筋に268個と多く出現した。頤舌筋には121個, 茎突舌筋には75個, 舌骨舌筋には37個と筋紡錘は外舌筋に233個分布し, 舌全体では501個の筋紡錘が片側で存在した。

The author constructed a new attachment utilizing the undercut area of the mesial or distal neck of anchor teeth (Fig. 1. 2. 3.) and applied it to the small unilateral dentures attached arteriorly and posteriorly with favorable results.Advantages of this attachment are as follows:1. Partial denture with this attachments looks naturally, because of reducing conspicuity of metal.2. Comfortable, because of no clasp arm and small denture plate.3. The denture is retained in the mouth with sufficient firmness and without injury to the remaining structures.
