Purpose: An increased prevalence of patent foramen ovale (PFO) has been reported in patients with migraine with aura compared with non-migraine subjects. This study aimed to investigate the prevalence of PFO in Japanese patients with migraine. Methods: Fifty-four consecutive patients with migraine were recruited from the headache outpatient clinic of our department. Migraine was diagnosed according to the International Classification of Headache Disorders, second edition. Patients were divided into migraine with aura (MWA) and migraine without aura (MWOA) groups. Transcranial ultrasound was performed, while contrast agent was injected intravenously with the Valsalva maneuver. PFO was diagnosed if micro-embolic signals in the right middle cerebral artery were identified soon after injection of contrast agent and Valsalva load release. Results: No significant differences in patient characteristics were observed between the MWOA group (n=22) and MWA group (n=32). The prevalence of PFO was 46.3% among all migraine patients, 56.3% in the MWA group, and 31.8% in the MWOA group. Patients with MWA thus tended to show a higher prevalence of PFO compared to those with MWOA (p = 0.077). Conclusion: The tendency toward an increased prevalence of PFO in the MWA group in this study suggests a possible association between MWA and PFO.

Purpose: This study is to assess the clinical utility of jugular venous flow pattern by evaluating ultrasonography. Methods: Consecutive 438 patients who underwent carotid artery ultrasonography were enrolled. They were evaluated jugular vein flow patterns and divided into three types: orthodromic, to-and-fro and antidromic. All of them were received MRA and compared to the flow patterns of ultrasonography. The relationship of jugular venous flow pattern and dural arteriovenous fistula (dAVF)/transient global amnesia (TGA) was also assessed. Results: The to-and-fro or antidromic pattern was significantly associated with older age, but not heart failure, in 81 patients, which was more frequently found on the left side. On MRA, venous flow signals were observed in 28 patients. The to-and-fro or antidromic pattern were more frequently observed on ultrasonography and was significantly associated with venous flow signals on MRA. Four patients who were diagnosed as dAVF showed the orthodromic flow pattern. Twelve patients who were diagnosed as TGA, and five of them showed a to-and-fro or antidromic flow pattern, which was a significantly high frequency. Conclusions: Assessment of jugular flow patterns by ultrasonography and/or MRA can help the diagnosis of diseases which are supposed to jugular venous flow abnormality.

Ocular ischemic syndrome can be of either the acute type (amaurosis fugax, retinal artery occlusion and ischemic optic neuropathy) or the chronic type (venous stasis retinopathy and ischemic oculopathy). We carried out etiological studies on 35patients (19men, 16women, aged 62±16years) with retinal artery occlusion and 21patients (13men, 8women, aged 47±19 years) with amaurosis fugax. In all patients, carotid ultrasonography was performed to clarify the extent of carotid artery disease ipsilateral to the retinal artery occlusion and amaurosis fugax. All the patients underwent electrocardiography and transesophageal echocardiography. Arterial stenosis exceeding 50% of the diameter of the internal carotid artery ipsilateral to the symptomatic eye was more frequent in patients with retinal artery occlusion than in those with amaurosis fugax. Patent foramen ovale more frequent in the latter than in the former. Patients with venous stasis retinopathy and ischemic oculopathy (neovascular glaucoma) had occlusion of the ipsilateral carotid artery with reversed ophthalmic artery or severe stenosis of the ipilateral carotid artery with stenosis of the ipsilateral ophthalmic artery. Neurosonologic studies can provide valuable clinical data on ocular ischemic syndrome.

はじめに 全く思いがけないことに,2000年10月10日,ノーベル財団から『電気を通すプラスチックの発見は「有機物は電気を通さない絶縁体」という常識を覆す画期的で,独創的な研究成果である』という発表があり,20世紀最後の年のノーベル化学賞を受賞することになった. 大学院を終え助手として大学に奉職するようになってから,2000年3月に定年退官するまでの34年間,高分子合成に関するさまざまな研究を行った.一貫して行った研究はアセチレンの重合反応に関することであった.研究を始めた当初は反応機構の解明が当面の目標だったので,電気を通す有機化合物に全く興味を持たなかったわけではないが,将来そのような研究にテーマを変えることは夢にも思わなかった.ところがひょんなことから,というよりは思わぬ失敗実験のお陰により,これまで粉末でしか得られなかったアセチレンの重合体であるポリアセチレンが薄膜状で合成できることが分かった.その薄膜状のポリアセチレンを試料とすることにより,とんとん拍子で目的とする研究が進み,数年で目的とする研究を終えることができた.

We report two cases of stroke with calcified cerebral emboli (CCE). Case 1 was a 76 years old woman who experienced stroke with right hemiplegia. Non-contrast enhanced cranial computed tomography (CT) and magnetic resonance imaging on admission showed acute stroke and CCE in the branches of the left middle cerebral artery (MCA). A calcified plaque was seen as mitral annular calcification (MAC) on echocardiography. No other plaque was seen on echo and CT, so we identified MAC as the source of the emboli. Case 2 was 92 years old woman who experienced stroke with right hemiplegia and disturbance of consciousness. Non-contrast enhanced CT showed acute stroke and CCE in the branches of the MCA and a chest CT showed aortic arch calcification. We identified as aortic arch calcification as the source of the emboli. In these cases, ultrasound examination and CT were useful for the diagnosis of CCE. Since CCE is difficult to diagnose, further investigations are recommended.

A 79-year-old woman was admitted to our hospital with vertigo. Vertigo was exacerbated by head rotation. Head magnetic resonance imaging revealed no acute cerebral infarction. The bilateral posterior communicating artery was not seen on magnetic resonance angiography. Three-dimensional computed tomographic angiography revealed severe stenosis in the left subclavian artery. Carotid Doppler ultrasonography (CDUS) revealed a change in the vertebral artery (VA) blood flow with the head rotated. These findings were also confirmed using dynamic digital subtraction angiography (DSA). Stenting in the left subclavian artery was performed, and the vertigo disappeared. This is the first report of subclavian steal syndrome that appeared with head rotation. These findings suggest that evaluation of VA and subclavian artery using dynamic CDUS and DSA is required for patients presenting with vertebrobasilar insufficiency even if VA blood flow is antegrade in the neutral position.
