顎関節症の治療は,その原因を明確にした上で実施することが大切である。理学療法の実施にあたっては,顎関節の解剖学や運動学を理解した上で評価が完全になされていることが重要となる。本論文では,欧米諸国の顎関節症の治療理論と実際をふまえながら,顎関節症の理学療法と生活指導について述べる。理学療法としては,物理療法の他に,マイオフェイシャルリリース,軟部組織モビライゼーション,アクティブ・ストレッチ,リラクセーション,下顎下制リリース,関節モビライゼーション,関節包内運動再教育訓練,筋力増強及び協調性訓練などについて概説する。

[Purpose] We focused on skeletal muscle mass index, one of the biomarkers of sarcopenia, and investigated the association between skeletal muscle mass index and the parameters of lung function and respiratory muscle strength. [Participants and Methods] After applying the exclusion criteria, we included, in this cross-sectional study, 120 community-dwelling older adults aged ≥65 years who required long-term care/support and underwent ambulatory rehabilitation under the long-term care insurance system in Japan. We measured the skeletal muscle mass index, forced vital capacity, forced expiratory volume in 1 second, peak expiratory flow rate, maximum expiratory pressure, and maximum inspiratory pressure. The data were analyzed using Pearson correlation coefficient and multiple regression analysis. [Results] The skeletal muscle mass index was positively correlated with only maximum expiratory pressure for both male and female participants by Pearson’s correlation coefficient. With the skeletal muscle mass index as a dependent variable, only the maximum expiratory pressure was significant for both male and female participants by the multiple regression analysis. [Conclusion] Therefore, the findings of this study suggested that compared with lung function tests, maximum expiratory pressure, which is an indicator of respiratory muscle strength, is related to muscle mass. Maximum expiratory pressure might be the most useful indicator for sarcopenia.

[Purpose] The foundations of the scoliosis specific and evidence-based physiotherapy program according to Schroth is the original the Lehnert-Schroth (LS) classification which is still in use today. The purpose of this paper is to test the reliability of the LS classification system, using clinical and radiological images of scoliosis patients as classified by specialist experienced clinicians. [Participants and Methods] A list of 40 pictures of X-Rays and a list of 40 clinical pictures (all posterior trunk images) of patients with idiopathic scoliosis were provided by the second author. Three specialist professional physiotherapists or orthotists rated all clinical and radiological pictures according to these two patterns of the LS classification. [Results] The intra-observer Kappa value was 0.90 (clinical) and 1.00 (x-rays). The inter-observer Kappa values at average was 0.65 (clinical) and 0.71 (x-rays). [Conclusion] For the application of classifying the patients when prescribing postural advice and exercises from the Schroth program the LS-classification seems an easy to use and highly reliable tool. This test demonstrated sufficient reliability with respect to the x-rays, but the tests of the clinical pictures alone, demonstrated fair levels of reliability, which indicates that it is an appropriate tool for physiotherapists when an x-ray is not available.

[Purpose] This study used electromyography to measure the effective treatment duration of interferential current therapy for muscle fatigue. [Subjects and Methods] Fifteen healthy adult men volunteered to participate in the study (age: 24.2 ± 1.3 years; weight: 67.6 ± 4.92 kg; height: 176.4 ± 4.92 cm). All subjects performed 5 min of isometric back extension exercise to produce muscle fatigue, and were then treated with interferential current therapy for 15 min, with electromyography monitoring (treatment group). After sufficient rest, the exercise was repeated for 5 min and an electromyography signal was acquired for 15 min with no treatment (control group). [Results] In the treatment group, the median frequency shifted to a higher level; the root mean square decreased over time, and then maintained a minimum amplitude. However, there were few changes in the electromyography signal after exercise in the control group. [Conclusion] Electromyography signals can provide information about the effective duration for muscle fatigue treatment as well as the muscle characteristics during treatment. This study should be helpful for clinicians by demonstrating the appropriate duration of therapy for relief of muscle stiffness.

[Purpose] No previous studies have confirmed whether the effects of visual feedback on motor learning vary according to learner skill level for a learning task. The purpose of this study was to clarify whether differences in skill influence the effects of visual feedback on motor learning. [Participants and Methods] Sixty-four participants were assigned to one of four different feedback groups (concurrent-100%, concurrent-50%, terminal-100%, or terminal-50%). The learning task was to adjust the load amount continuously to the left lower limb in accordance with sound stimulation at intervals of 1 Hz. The four groups performed a pretest, practice sessions, and a retention test 24 hours after practice. After completing these measurements, the participants were classified as either high- or low-skilled based on the results of the pretest. [Results] Only the groups of low-skilled participants who used concurrent feedback showed lower root mean square errors in the retention test compared to in the pretest. [Conclusion] Differences in skill level for the same task influenced the effects of visual feedback on motor learning. Furthermore, concurrent visual feedback can help improve motor learning in low-skilled learners for the same task.

[Purpose] Although there is evidence that non-specific chronic pain can be influenced by physical therapy, some patients with scoliosis and chronic pain may benefit from additional brace treatment. The purpose of this review is to answer the question whether there are studies on the use of brace treatment in patients with scoliosis and pain and to investigate whether brace treatment does positively influence chronic pain. [Methods] A PubMed review has been undertaken using the key words (1) scoliosis and pain and brace treatment and (2) scoliosis and pain and orthotics. From both searches the studies were extracted that included a patient group with the diagnosis of a scoliosis and with additional chronic non-specific low back pain, treated with a brace. [Results] One hundred forty two items have been found for search (1) and 111 for search (2). The total number of relevant items found in both searches was 10. According to the studies found, bracing seems an effective treatment for this condition. [Conclusion] Brace treatment in patients with scoliosis and chronic non-specific low back pain has demonstrated to be effective. It should be used when exercise treatment is not effective. A clinical test is demonstrated to predict the most beneficial approach in brace treatment.

[Purpose] This study examined the effects of trunk exercises performed on an unstable surface on trunk muscle activation, postural control, and gait speed in stroke patients. [Subjects] Twenty-four participants with stroke were recruited in this study and randomly distributed into experimental (n = 12) and control groups (n = 12). [Methods] Subjects in the experimental group participated in trunk exercises on the balance pad for 30 min, five times a week for 4 weeks; those in the control group performed trunk exercises on a stable surface for 30 min, five times a week for 4 weeks. Trunk muscle activation was measured by using surface electromyography, and trunk control was evaluated with the Trunk Impairment Scale (TIS). Gait speed was measured with the 10-Meter Walk Test. [Results] Activity of the external and internal oblique muscles in the experimental group was significantly higher than that in the control group. The TIS score of the experimental group showed significantly greater improvement than did that of the control group. The 10-Meter Walk Test (10MWT) score also significantly improved in the experimental group. [Conclusion] Trunk exercises on an unstable surface improve trunk muscle activation, postural control, and gait speed in patients with hemiparetic stroke.

[Purpose] The aim of the present study was to evaluate whether quality of life (QOL) was improved in gynecological cancer patients with lymphedema following Complex Decongestive Physical Therapy (CDPT), and whether edema volume change as a result of treatment correlated with change in QOL. [Subjects and Methods] Forty-six patients who had unilateral lower leg lymphedema were treated with CDPT for 2-4 weeks. Percentage excess volume (PCEV) and QOL were recorded at baseline, 1 month, and 6 months after CDPT. [Results] PCEV was significantly decreased at 1 month. There was a statistically significant difference in physical functioning, role-physical, mental health, and general health in the Korean version of the Short Form-36 (SF-36) Health Survey. [Conclusion] The results of this study suggest that QOL was significantly improved for gynecological cancer patients with lower leg lymphedema during the maintenance phase, and was necessarily correlated with the PCEV.

[Purpose] The purpose of this study was to determine the effects of adjusting the scapula into its ideal position through active scapular protraction on the muscle activation and function of the upper extremity. [Subjects] Twenty female college students aged 19–21 without any physical or functional disability were the subjects of this study. They had no history of injury to their upper extremities or hands. [Methods] After the initial measurements the experimental group was asked to perform active scapular protraction; then, their grip strength and muscle activation were measured again. Every action was maintained for 5 seconds and repeated 3 times. The mean values of the measurements were analyzed. A resting of 1 minute was given between each action. [Results] The results revealed a significant change in the experimental group’s grip strength after active scapular protraction had been performed. The surrounding muscles of the scapula, such as the serratus anterior, upper trapezius, flexor carpi ulnaris, flexor carpi radialis and palmaris longus, showed significant changes in muscle activation after active scapular protraction. The muscles of the upper extremity also showed significant changes after active scapular protraction. [Conclusion] The adjustment of scapula into its ideal position through active scapular protraction increased the activations of the muscles surrounding the shoulder joint and improved the function of the upper extremity.

[Purpose] The purpose of this study was to investigate the therapeutic effects of scapular stabilization exercise on function of the upper extremity of individuals with chronic hemiparetic stroke. [Subjects] Eleven chronic hemiparetic stroke patients participated in a scapular stabilization exercise program conducted in the sitting position for 30 min per session, 5 days per week, for 4 weeks. We used 3 clinical assessments: the Manual Function Test, the Berg Balance Scale, and the Modified Barthel Index. [Results] Manual Functional Test Scores for the paretic upper limb significantly improved after the scapular stabilization exercise program. However, Manual Function Test scores for the non-paretic upper limb did not significantly improve after the scapular stabilization exercise program. The Berg Balance Scale and the Modified Barthel Index scores did not significantly improved after scapular stabilization exercise program. [Conclusion] The results of this study suggest that scapular stabilization exercise can improve the function of the paretic upper extremity of individuals with chronic stroke. However, scapular stabilization exercise did not affect balance or basic daily activities.

[Purpose] Although gum chewing while walking has been reported to increase walking speed and heart rate, its effect on energy expenditure remains unclear. The purpose of the present study was to investigate the effects of gum chewing while walking on fat oxidation, energy expenditure, and different walking parameters. [Participants and Methods] This randomized crossover study included 10 males and 5 females who walked for 15 min at their own pace while chewing 2 pieces of gum in the gum trial or while eating 2 tablets in the control trial. A wearable metabolic system, heart rate monitor, and pedometer measured fat oxidation, energy expenditure, heart rate, step count, and walking distance. Walking speed and stride length were also calculated. [Results] The energy expenditure, fat oxidation and heart rate were significantly higher during the gum trial than during the control trial. Significant increases were observed in the step count, walking distance, and walking speed but not in the stride length. [Conclusion] Our results suggest that gum chewing affects sympathetic nervous system activity and walking rhythm with a consequent improvement in the health-related effects of walking, which in turn helps to maintain weight. These findings may play a role in preventing the gradual age-related weight gain that predisposes to obesity.

[Purpose] To establish a standardized sonographic approach to quantify anterior translation of the mandibular condyle during mouth opening by examining the reliability of image acquisition and processing, and to understand how anterior condylar translation contributes to mouth opening. [Participants and Methods] Twenty-eight participants without temporomandibular disorder (TMD) participated. During day 1 of data collection, all participants performed maximal mouth opening while an examiner recorded anterior condylar translation using sonography. The mouth opening range of motion was also obtained. On day 2 of data collection, the same procedure was performed on 6 participants that participated in day 1 of data collection. To establish reliability of image processing, 3 examiners measured condylar translations on 2 separate days. To determine reliability of image acquisition, images obtained from 2 days of data collection were analyzed. [Results] Excellent intra-class correlation coefficients (ICCs) and small standard errors of measurement (SEMs) for image acquisition and processing were shown. A significant, linear model was found to describe the relationship between condylar anterior translations and mouth opening. [Conclusion] Anterior condylar translation during mouth opening can be reliably measured using sonography. The linear relation between condylar motion and mouth opening can be used to guide clinical practices.

[Purpose] This study aimed to investigate the effects of the therapeutic device combined with LED and microcurrent (MC) on muscle tone and stiffness in the calf muscle after its application during moderate aerobic exercise. [Subjects and Methods] Twenty healthy adult subjects were randomized to either the test group of the therapeutic device combined with LED and MC or the control group, and they walked on a 10%-sloped treadmill with a 5 km/hr speed for 30 minutes. Each of the subjects in the test group performed treadmill exercise with the therapeutic device attached to the edge of his or her calf muscle. After the exercise, the muscle tone and stiffness at the edge of the calf muscle were measured. [Results] With respect to the muscle tone, a statistically significant difference was found between the two groups only 5 minutes after the exercise. Concerning muscle stiffness, significant differences were shown between the two groups right after the exercise and 5 minutes after the exercise. [Conclusion] Integrated treatment with LED and MC on is considered helpful for lowering the muscle tone 5 minutes after the exercise, and for lowering the muscle stiffness right after the exercise and 5 minutes after the exercise.

[Purpose] The present study aimed to investigate the effects of repetitive transcranial magnetic stimulation (rTMS) on visual perception, depression, and activities of daily livings (ADLs) in stroke patients. [Subjects and Methods] Forty-four stroke patients were divided equally into an experimental group that underwent rTMS and a control group that underwent mock rTMS. Changes in patient visual perception, depression, and ADLs were evaluated. All subjects underwent treatment for 20 minutes, 5 times per week, for 4 weeks. Beck Depression Inventory (BDI), Motor-free Visual Perception Test (MVPT) and Functional Independent Measurement (FIM) were respectively used to assess depression, visual perception and ADLs. [Results] The experimental group showed significant improvements in depression, visual perception, and ADLs between week 1 and 4, between week 1 and 8, and between week 4 and 8. Meanwhile, the control group showed no differences between week 1 and 4, and although, like in the experimental group, a significant difference was observed in depression and visual perception between Week 1 and 8, there was no significant difference in ADLs. [Conclusion] These demonstrate that rTMS has a positive impact on visual perception, depression, and ADLs.

[Purpose] This study aimed to investigate the effects of multisensory dynamic balance training on muscles thickness such as rectus femoris, anterior tibialis, medial gastrocnemius, lateral gastrocnemius in children with spastic diplegic cerebral palsy by using ultrasonography. [Subjects and Methods] Fifteen children diagnosed with spastic diplegic cerebral palsy were divided randomly into the balance training group and control group. The experimental group only received a multisensory dynamic balance training, while the control group performed general physiotherapy focused balance and muscle strengthening exercise based Neurodevelopmental treatment. Both groups had a therapy session for 30 minutes per day, three times a week for six weeks. The ultrasonographic muscle thickness were obtained in order to compare and analyze muscle thickness before and after in each group. [Result] The experimental group had significant increases in muscle thickness in the rectus femoris, tibialis anterior, medial gastrocnemius and lateral gastrocnemius muscles. The control group had significant increases in muscle thickness in the tibialis anterior. The test results of the rectus femoris, medial gastrocnemius and lateral gastrocnemius muscle thickness values between the groups showed significant differences. [Conclusion] In conclusion, a multisensory dynamic balance training can be recommended as a treatment method for patients with spastic diplegic cerebral palsy.

[Purpose] This study was conducted to investigate the effect of exercise training using transcranial direct current stimulation (tDCS) on breathing in patients with chronic stroke patients. [Subjects and Methods] Thirty chronic stroke patients who do not show abnormal response to electric stimulation were enrolled in this study and each 15 subjects were randomized either into the study group and the sham-controlled group. The subjects performed diaphragmatic breathing exercise for 20 minutes while tDCS device was attached to them (for study group, the device was on while for the sham-controlled group, the device was turned off 30 seconds later) [Results] The results of FVC, FEF1 and FEV1/FVC in the study group and those of FVC and FEV1 in the sham-controlled group were significantly increased after the breathing exercise. The independent comparison result between the groups showed that the breathing performance of study group significantly improved based on the results of FVC and FEV1. [Conclusion] In conclusion, the results of this study confirmed that breathing exercise effectively improved FVC and FEV1 in chronic stroke patients. Also, the breathing exercise using tDCS was more effective in improving FVC and FEV1.

[Purpose] The aim of this study was to assess the effects of transcranial direct current stimulation (tDCS) on depression and quality of life (QOL) in patients with stroke, by conducting conventional occupational therapy with and without tDCS on 20 patients each. [Subjects and Methods] The experimental group (N=20) received both tDCS and conventional occupational therapy, while the control group (N=20) received false tDCS and conventional occupational therapy. The treatment was conducted 20 times over a four-week period; each session was 30 minutes long. The Beck Depression Inventory (BDI) was administered to score the depression levels in patients before and after the intervention, while the stroke-specific quality of life (SS-QOL) was measured to compare the QOL. [Result] Following the intervention, the patients in the experimental group showed a significant decrease in depression and an increase in the QOL. In contrast, the control group showed no significant changes in depression or QOL. Our findings indicate that tDCS decreased depression while increasing QOL in patients with stroke. [Conclusion] In other words, our study confirmed that the application of tDCS during stroke rehabilitation improves the depression symptoms and QOL in patients.

[Purpose] The aim was to evaluate the effect of therapeutic ultrasound on the pain, joint mobility, muscle strength, physical function, and quality of life of people with knee OA. [Subjects and Methods] One-site, one-arm, before-after study that included people with Grade II or III tibiofemoral osteoarthritis. Ten therapeutic ultrasound sessions (duty cycle=20%, ERA=10 cm2, BNR=6:1, SATP=2.2 W/cm2, SATA=0.44 W/cm2, frequency=1 MHz, time=4 minutes) were applied. Assessments of primary outcome variables: pain intensity and function, and secondary variables: joint mobility, muscle strength and quality of life, were performed at onset and end of therapy; an additional intermediate evaluation was included for the primary variables. [Results] Means of repeated measurements of pain intensity (pain at rest, pain on palpation and pain after functional activities) and function showed significant differences. There was a significant reduction in pain intensity at the end of functional activities as well as a significant increase in function and in quadriceps muscle strength. [Conclusion] Therapeutic ultrasound applied in accordance with the parameters used, could be recommended during the treatment of individuals with knee osteoarthritis, because it significantly decreased the intensity of pain after the 5th session, and this reduction was maintained until the end of the intervention.

〔目的〕大腿骨近位部骨折者のADL評価と再転倒リスク評価としてのtimed up and go(TUG)テストの有用性を検証した.〔対象〕当院回復期リハビリテーション病棟から自宅退院する大腿骨近位部骨折者40名とした.〔方法〕退院前に TUGテストとfunctional independence measure(FIM)を行い,両者の関連性を分析するとともに退院後3ヵ月間の再転倒の有無を調査した.〔結果〕TUG遂行時間とFIM得点に有意な相関が認められた.再転倒者は7名で,その中にはFIM高得点者も存在したが,TUG遂行時間20秒以下の条件を加えると再転倒者は0名であった.〔結論〕大腿骨近位部骨折者の退院時のADLおよび再転倒リスクを評価する上で,TUGテストは有用といえる. <br>

[Purpose] This study examined the muscle activation when subjects gripped a handle with a circumference equal to 0%, 25%, 50%, 75%, or 100% of their hand length to determine the size that can be gripped most stably relative to the hand length. [Subjects and Methods] The muscle activation of 34 subjects was measured using electromyography. To make a cylinder proportionate to the hand length, multiple pencils were combined and the circumference was measured with a tape measure. The cylinders were made with circumferences of 0 %, 25 %, 50 %, 75 %, and 100 % of the hand length. [Results] The largest muscle activation was observed at handle circumferences of 50% and 75% of the hand length in the wrist extensor bundle and at 75% of the hand length in the flexor digitorum superficialis. [Conclusion] Therefore, circumferences of the middle range of hand length seems to be most appropriate for hammer handles and handles with high resistance which require large strength at industrial sites.
