〔目的〕ノルディック・ウォーキング(NW)とT字杖での歩行が歩容や姿勢に及ぼす影響を明らかにする事を目的とした.〔対象と方法〕対象は,NWの経験がない健常若年成人20名とした.NW,T字杖での歩行,杖なしでの歩行の3つの条件での歩行をトレッドミル上で実施した.歩行の速度は快適歩行速度とした.モーションキャプチャシステムを使用し,身体各部の角度(頸部・体幹・股関節・膝関節)を算出し比較を行った.〔結果〕通常歩行に比べ,T字杖歩行で歩行中の頸部最大屈曲が有意に大きな値となった.また,NWでは歩行中の膝最大屈曲が有意に小さい値となった.〔結語〕頸部疾患や膝関節疾患を有する症例に対して,力学的負荷を軽減したいような場合にはNWの方が有効となる可能性を示唆する結果となった.

[Purpose] To assess the current state-of-the-art and the prevailing trends regarding the global use of blood flow restriction (BFR) in the past 20 years. [Participants and Methods] We retrieved literature relating to BFR from 1999 to 2020 using Web of Science. We conducted a bibliometric analysis of countries/institutions, cited journals, authors/cited authors, cited references, and keywords using CiteSpace. An analysis of counts and centrality was used to examine publication output, countries/institutions, core journals, active authors, foundation references, hot topics, and frontiers. [Results] Seven hundred seventy five references were included and the total number of publications has been continually increasing over the investigated period. Representatives of important academic groups are the Japanese scholars from the University of Tokyo as represented by Takashi Abe. Jeremy Paul Loenneke’s article (centrality: 0.15) was the most representative and symbolic reference with the highest centrality. The three topics identified were intervention (intensity resistance exercise, IRE), physiology (ischemia and muscular function) and behavior (adaptation and increase). The four frontier topics were phosphorylation, reduction, low intensity and arterial occlusion. [Conclusion] This study provides an insight into BFR and offers valuable information for BFR researchers to identify new perspectives for potential cooperation with collaborators and their related cooperative institutions.

本研究は,健常若年者11名と高齢者19名を対象に,椅子からの立ち上がり動作について運動学的分析を行った。また高齢者は,その動作能力に基づいて動作自立群11名と動作低下群8名に分けて比較した。その結果,動作低下群で1)動作時間の延長と第1相の比率の増加,2)離殿時期における支持基底面と身体重心との間の距離の減少,3)身体重心の最大水平速度の低下,4)離殿時期における体幹屈曲角度の増加が認められた。このことから,動作能力の低下した高齢者は力学的に安定した姿勢調節を行って立ち上がることが示唆された。

[Purpose] This study aimed to compare the muscle activity around the foot and ankle joints, notably of the abductor digiti minimi, between affected and unaffected sides of individuals with chronic ankle instability. [Participants and Methods] Twelve adult males with chronic ankle instability in one ankle (age, 27.7 ± 5.4 years; height, 172.5 ± 8.1 cm; weight, 67.5 ± 8.1 kg) were included and underwent surface electromyography assessments in multiple positions on both affected and unaffected sides. Measurements were obtained for eight muscles including the abductor digiti minimi. Each measurement included a 5-s segment of the stable waveform, with the root mean square-processed and normalized to the resting position set to 1. [Results] Abductor digiti minimi activity on the affected side was significantly reduced during maximal toe extension/abduction with both ankle dorsiflexion and plantarflexion. Peroneus longus activity on the affected side was significantly greater during maximal toe extension/abduction with ankle plantarflexion; peroneus longus and tibialis anterior muscle activities were significantly greater on the affected side during maximal toe extension/abduction with ankle dorsiflexion. [Conclusion] In the absence of load, muscle imbalance in the intrinsic and extrinsic muscles of the foot was suggested. However, no significant differences were observed under loading conditions.

〔目的〕本研究は,女性柔道選手の月経期間別の月経随伴症状と生活習慣との関連性について明らかにすることを目的とした.〔対象と方法〕18歳から21歳までの女性柔道選手169名を対象とし,プロフィール,柔道実施状況,生活習慣で構成された質問紙と月経随伴症状に関する質問紙(MDQ)調査を実施した.〔結果〕月経前および月経中のMDQの総得点と平日の睡眠時間との間に負の相関が認められた.平日および休日の睡眠時間が短い女性柔道選手は月経随伴症状の得点が高値を示す関係性が明らかとなった.〔結語〕女性柔道選手において月経前や月経中は月経随伴症状が悪化することから,睡眠時間をはじめとした生活習慣への注意が必要である.

〔目的〕脳卒中片麻痺患者の歩行開始時の体幹運動特性について検証した.〔対象と方法〕対象は健常高齢者10名と脳卒中片麻痺患者30名とした.重心動揺計,表面筋電計,加速度計を用い,歩行開始時の体幹運動を計測した.筋電図は左右の中殿筋と脊柱起立筋の4筋を導出筋とし,加速度は頸部(C7),腰部(L3),骨盤(S1)に貼付し,測定した.〔結果〕脳卒中片麻痺患者の筋活動潜時は,麻痺側先行ステップ時の麻痺側中殿筋,非麻痺側先行ステップ時の麻痺側脊柱起立筋が非麻痺側より遅延していた.加速度はC7からL3を引いた差の値(dCL)の比較において,健常高齢者と脳卒中片麻痺患者の麻痺側先行ステップで立脚側方向へdCLが変化していたのに対して,非麻痺側先行ステップで遊脚側方向へ変化がみられた.〔結語〕脳卒中片麻痺患者の歩行開始において,先行肢別に体幹が傾斜する代償制御と動き出しが遅延する遅延制御の異なるパターンの運動戦略を呈した.

〔目的〕本研究の目的は,足部感覚トレーニングが健常若年者の足底の体性感覚に与える影響を明らかにすることである.〔対象と方法〕対象は健常若年者26名とした.介入は,閉眼の座位姿勢にて多軸不安定板上に裸足で足底部を乗せ,前後左右の1ヵ所にランダムに重錘を置き,どこに重錘があるか回答を求めた.介入前後で足底(母趾,小指球,踵)の触圧覚および二点識別覚を計測し比較した.〔結果〕介入後は,触圧覚は変化を認めなかったが,足底(母趾,小指球,踵)の二点識別覚の向上が認められた.〔結語〕本研究で考案した足部感覚トレーニングを実施することで,即時的に足底の感覚機能が向上することが示唆された.

[Purpose] The aim of this study was to compare hamstring-to-quadriceps ratio between the accelerating and decelerating sections for anterior cruciate ligament protection during squat exercise. [Subjects and Methods] Nine asymptomatic males were enrolled in this study. The hamstring (medial part) and quadriceps (rectus femoris) muscle activities during squat exercise were measured, and the squat exercises were classified into two sections (accelerating and decelerating) by using an accelerometer. [Results] The hamstring-to-quadriceps ratio was significantly higher in the decelerating section than in the accelerating section during the squat exercise. [Conclusion] Application of an increasing decelerating section strategy during the squat exercise can prevent damage in patients with a weakened anterior cruciate ligament due to sports activities.

[Purpose] To investigate how different ankle-foot orthosis functions with the same dorsiflexed setting of initial ankle joint angle affect the walking ability in individuals with chronic stroke. [Participants and Methods] In this randomized crossover study, participants underwent a 10-m walking test and walked on a WalkWay MW-1000 three times under these conditions: (1) without ankle-foot orthosis; (2) with ankle-foot orthosis with an adjustable posterior strut at 5° of fixed dorsiflexion; and (3) with ankle-foot orthosis with an adjustable posterior strut at 5–20° of restricted dorsiflexion. The primary outcome was walking speed on the 10-m walking test. The secondary outcomes were walking speed and spatiotemporal factors measured by the WalkWay MW-1000. [Results] Fifteen individuals (mean [standard deviation] age, 60.9 [8.6] years; male, 12) were enrolled. Walking speeds of the ankle-foot orthosis with fixed and restricted dorsiflexion groups were significantly higher than those without the orthosis; however, no outcomes differed significantly between ankle-foot orthosis with fixed versus restricted dorsiflexion groups. [Conclusion] In individuals with chronic stroke, ankle-foot orthosis function may be less important than the dorsiflexed setting of initial ankle joint angle in the ankle-foot orthosis.

[Purpose] The aim of this study was to determine whether the consumption of a leucine-enriched essential amino acid mixture (LEAA), which is known to increase protein synthesis in muscles, alleviates muscle damage and accelerates recovery by ameliorating muscle damage. [Participants and Methods] A double-blind, randomized crossover trial was conducted over a 5-week period. Ten untrained males (age, 23.0 ± 1.6 years) were asked to repeatedly flex and extend their elbows for 10 counts/set × 5 sets at full power while using a dynamometer. The participants took 3.6-g supplements (LEAA mixture or placebo) 3 times daily on day 0 and for the next 7 days. Changes in serum creatine phosphokinase (CPK) activity and myoglobin concentration as markers of muscle tissue damage were evaluated prior to and after exercise and on days 1, 2, 3, 5, and 7. [Results] The relative ratio of the changes in peak serum CPK activity measured on day 5 was significantly lower after taking LEAA than after taking the placebo. [Conclusion] LEAA consumption suppressed exercise-induced elevation of muscle damage markers in blood, which suggests that LEAA could attenuate muscle damage and aid muscle recovery.

〔目的〕学習記録用紙を用いた対話的かつ協働的な学習記録活動が,学生の学業的援助要請形態に及ぼす影響について検討した.〔対象と方法〕リハビリテーション専門学校の学生37名を対象に,日々の学習内容を学習記録用紙に記入し,その内容を3~4名のグループで共有して協議する学習記録活動を8ヵ月間毎日実施した.また,学習記録活動前,4ヵ月後,8ヵ月後に学生の学業的援助要請形態を調査し,回避型・依存型・適応型の得点を算出した後,その経時的な変化について検討した.〔結果〕回避型は4ヵ月後に,依存型と適応型は8ヵ月後に有意な改善が認められた.〔結語〕対話的かつ比較的容易に短時間で実践可能な学習記録活動を継続的に実践することにより,学生の援助要請スキルの修得が促される可能性が示唆された.

〔目的〕間質性肺炎増悪が胸郭出口症候群発症の要因になった可能性のある症例に対して行った介入が,症状改善をもたらした例を報告する.〔対象と方法〕間質性肺炎により常時0.5 L/分の在宅酸素療法を行っていたが,呼吸困難感と左上肢の疼痛,感覚障害を呈し, 間質性肺炎増悪による経皮的動脈血酸素飽和度低下で入院となった80代女性を対象とし,左斜角筋群ストレッチ,リラクゼーション,胸郭ストレッチなどの呼吸リハビリテーションを中心に介入を行った.〔結果〕その結果,疼痛消失と感覚障害消失がみられた.〔結語〕呼吸機能に着目して行った介入が,間質性肺炎増悪が発症の要因になった可能性のある胸郭出口症候群の症状を緩和にしたことから,呼吸器疾患による胸郭出口症候群発症の可能性が示唆された.

〔目的〕人工呼吸器(MV)管理中から早期歩行とセルフケアを獲得した重症肺炎の症例を経験した.〔対象と方法〕82歳男性,重症肺炎と敗血症性ショックによりICUへX日に入室した.X+5日よりMV管理中に歩行を含めた早期離床,および口腔ケアや整容などセルフケア指導を多職種で開始した.歩行練習は,理学療法士1名と看護師2名で5 mから開始し,50 m × 2セットまで漸増した.また,医療スタッフが必要物品を準備し,口腔内吸引,歯磨き,うがいを模倣して指導した.〔結果〕挿管下MV管理中のX+9日にセルフケアが自立し,歩行はX+12日に独歩見守りまで可能になった.〔結語〕本疾患例のように挿管下MV管理中においても,早期歩行およびセルフケアを獲得することは可能である.

[Purpose] The characteristics of heart failure in hospitalized patients with poor self-care behaviors are unknown. We investigated factors associated with self-care behaviors by using the European Heart Failure Self-Care Behaviour Scale (EHFScBS) in heart failure patients based on three comprehensive concepts. [Participants and Methods] This was a cross-sectional single-center study of heart failure patients hospitalized at a tertiary-care hospital. We investigated age, gender, family living together/apart, employment, and the Specific Activity Scale (SAS). A physical therapist provided the EHFScBS one time to determine the patients’ pre-hospital self-care behavior status. The 12 items of the EHFScBS were classified into the following three categories: Maintenance, Monitoring, and Management. [Results] The median age of the 39 consecutive patients was 81 years. A multiple regression analysis revealed that the factors exhibiting significant associations were the SAS score (β=0.504) for Management and age (β=−0.403) for the total EHFScBS score (adjusted by the number of hospitalizations for heart failure). Maintenance and Monitoring were not significantly associated with the survey items. [Conclusion] These data indicate that self-care education for hospitalized patients with heart failure leads to individualized approaches based on characteristics such as age and physical activity capacity.

[Purpose] The purpose of this study was to compare the strength and endurance between open and closed kinematic chain exercises after anterior cruciate ligament reconstruction. [Subjects] Sixty-two subjects actively participating in rehabilitation for anterior cruciate ligament reconstruction for the last 3 month were chosen. Twenty-six subjects were excluded and consequently, the data of 36 subjects were analyzed. The participants were randomly divided into 2 groups for open or closed kinematic chain exercise. [Methods] Open or closed kinematic chain exercises were performed under direct supervision of 3 physical therapists for 30 minutes per session, 3 times a week for 12 weeks. Before and after the intervention, isokinetic strength, endurance, and squat strength were tested. [Results] In the comparison of the before and after exercise results, all variables showed significant differences. Open kinematic chain exercises resulted in a significantly greater gain in isokinetic strength and endurance of the extensor muscles than the closed kinematic chain exercises. [Conclusion] Open kinematic chain exercise is helpful for the development of strength and endurance of the knee extensor mechanism after anterior cruciate ligament reconstruction, and can be used effectively in a rehabilitation program.

[Purpose] The aim of this study was to investigate the relationship between gait speed and various factors in ambulatory patients with idiopathic Parkinson’s disease. [Subjects] Fifty ambulatory patients with idiopathic Parkinson’s disease who were admitted to an outpatient clinic were included in this cross-sectional study. [Methods] The Hoehn and Yahr Scale was used for measurement of the disease severity. Gait speed was measured by the 10-Meter Walk Test. Mobility status was assessed by Timed Up and Go Test. The Hospital Anxiety and Depression Scale was used for evaluation of emotional state. Cognitive status was examined with the Mini-Mental State Examination. The Downton Index was used for fall risk assessment. Balance was evaluated with the Berg Balance Scale. Comorbidity was measured with the Cumulative Illness Rating Scale. The 36-Item Short Form Health Survey was completed for measurement of quality of life. [Results] The mean age was 66.7 (47–83) years. Twenty-eight (56%) patients were men. Gait speed was correlated positively with height, male gender, Mini-Mental Examination score, Berg Balance Scale score and physical summary scores of the 36-Item Short Form Health Survey. On the other hand, there was a negative correlation between gait speed and age, disease severity, TUG time, Downton Index, fear of falling, previous falls and the anxiety and depression scores of the Hospital Anxiety and Depression Scale. There was no correlation between gait speed and comorbidity. [Conclusion] The factors related with the slower gait speed are, elder age, clinically advanced disease, poor mobility, fear of falling, falling history, higher falling risk, and mood disorder.

[Purpose] Handgrip strength is a surrogate indicator for assessing disease-related and age-related skeletal muscle loss. Clinical utility as such a surrogate can be at least partially explained by the close relationship between handgrip strength and whole-body skeletal muscle mass. The handgrip strength is related to hand muscle size. Thus, the present study examined whether hand muscle thickness is associated with whole-body skeletal muscle mass. [Subjects and Methods] Thirty healthy male adults participated in this study. All subjects were right-hand dominant. Two muscle thicknesses (lumbrical and interosseous muscles) in the right hand were measured using ultrasonography. Whole-body and appendicular skeletal muscle masses were assessed using dual-energy X-ray absorptiometry. [Results] Although lumbrical muscle thickness was not correlated with whole-body skeletal muscle mass, there was a significant correlation with appendicular skeletal muscle mass. Furthermore, interosseous muscle thickness was significantly correlated with both whole-body and appendicular skeletal muscle masses. [Conclusion] The present findings suggest that two muscle thicknesses in the hand are related to whole-body and/or appendicular skeletal muscle mass in healthy adults. Therefore, we propose that despite being smaller than other limb muscles, hand muscle thickness may be useful as surrogate indicator for assessing disease-related and age-related skeletal muscle loss.

[Purpose] The purpose of this study was to investigate whether a cranio-cervical flexor exercise and thoracic mobilization are effective for muscular endurance (Endurance), visual analog scale (VAS) pain, and neck disability index (NDI) of patients with chronic neck pain. [Subjects] The subjects in this study were 53 patients who had chronic neck pain. [Methods] The experimental group (n=27) did both cranio-cervical flexor exercises and thoracic mobilizations and the control group (n=26) did cranio-cervical flexor exercises. [Results] A paired t-test analysis revealed significant pre-post score changes within both groups. The 2-way group time interaction for the repeated measures ANOVA was statistically significant for Endurance, VAS and NDI. The experimental group showed greater improvement in Endurance, with a between-group difference of 14.26 seconds, VAS pain, with a between-group difference of 2.02 points, and NDI, with a between-group difference of 2.07 points. [Conclusion] These results suggest that the combination of thoracic mobilization with cranio-cervical flexor exercise was more effective at increasing Endurance and reducing VAS pain and NDI.

[Purpose] To investigate the effects of thoracic manipulation and deep craniocervical flexor training on the muscle strength and endurance, range of motion, and the disability index of the neck of patients with chronic nonspecific neck pain. [Subjects and Methods] Forty-six patients with chronic neck pain participated. They received an intervention for 35 minutes a day, three times a week for 10 weeks. Subjects were randomly assigned to one control and two experimental groups: group A (thoracic manipulation combined with deep craniocervical flexor training, n=16), group B (deep craniocervical flexor training, n=15), and group C (active self-exercise as a control group, n=15). Muscle strength and endurance, pain, neck disability index, and range of motion of the cervical and thoracic spine were measured before and after the intervention. [Results] Group A showed significant increases in muscle strength, endurance, and cervical and thoracic range of motion, and significant decreases in the pain and neck disability index, compared with groups B and C. [Conclusion] Although deep craniocervical flexor training is effective at improving neck function, thoracic manipulation combined with deep craniocervical flexor training was a more effective intervention for pain relief and improving the range of motion, muscle function, and neck disability of patients with nonspecific chronic neck pain.

[Purpose] To investigate whether thoracic spine mobilization added to stabilization exercises increases the muscular strength and range of motion of the thoracic vertebrae of chronic low-back pain patients. [Subjects] This study enrolled 20 patients with chronic low back pain, who were divided into two groups. Ten subjects were randomly selected for the stabilization exercise group and the remaining 10 subjects received thoracic spine mobilization in addition to performing the stabilization exercises. [Methods] The patients performed stabilization exercises and received thoracic spine mobilization for 12 weeks. The range of motion and isometric muscular strength of the vertebrae of all subjects were measured before and after the intervention. [Results] In the comparison of muscular strength before and after the intervention, the change in muscular strength of the trunk flexors in the stabilization exercise group was 16.0±7.4 Nm, and that of the thoracic spine mobilization group was 34.2±7.6 Nm, a significant difference in each group. In the post-intervention intergroup comparison, the muscular strength of trunk flexors in the stabilization exercise group was 111.1±16.9 Nm, while that of the thoracic spine mobilization group was 125.9±11.3 Nm, a significant difference. Also, the muscular strength of the trunk extensors in the stabilization exercise group was 148.9±31.8 Nm, while that of the thoracic spine mobilization group was 182.9±37.2 Nm, a significant difference. The thoracic spine flexion in the stabilization exercise group was 29.8±9 degrees, while that of the thoracic spine mobilization group was 38.7±6.9 degrees, a significant difference. However, there was no significant difference in lumbar flexion values between the two groups. [Conclusion] Thoracic spine mobilization added to a stabilization exercise increased the muscular strength of patients with chronic low back pain.
