An external dental fistula involves formation of a fistula or granulation as an excretion pathway in the jaws or face, due to chronic purulent odontogenic inflammation. We describe a case involving a 30-year-old male patient who had an external dental fistula-like scar in the right buccal region. A diagnosis of an external dental fistula, caused by an infected right mandibular first molar, was made; endodontic therapy was performed without symptomatic improvement, and the patient was referred to our university. Tenderness in the masseter region and scarring in the right buccal region were found upon examination. Diagnostic imaging revealed a cylindrical structure suggestive of an external dental fistula in the soft tissues. Removal of the external dental fistula was performed under general anesthesia and the course was good. Patients with an external dental fistula may show symptoms for a prolonged period before a definitive diagnosis is made; however, diagnosis can be facilitated by early, accurate imaging examination.

The hyoid bone is a bone not directly connected with other bones. Rather it is surrounded by soft tissues, mandible and vertebra. These act a protector to the traumatic injury. We herein report two cases of hyoid bone fracture. A 29- year- old man was tackled during a football game. After the game, he experienced continous pain for a week while swallowing. A plain radiograph did not reveal the cause of his pain at the first examination, but CT images taken more than a week later revealed a hyoid bone injury. The 3D image shows the dislocation of a fragment of the bone. A 46- year- old man had injured his mandible in a traffic accident and thereafter experienced severe pain in his jaw and was unable to open his mouth. The fracture of his mandibular body and ramus was found by radiography. The repositioning operation of the mandibule was considered successful, but the hyoid bone fracture was not discovered at that time. After the operation, the patient complained of gradual pain upon swallowing, and a subsequent radiographic examination showed that the hyoid bone had been fractured. In both cases, it was confirmed that ordinary lateral oblique radiography should be technically modified to some extent, and CT or other images might be necessary for the detection or diagnosis of hyoid bone fracture.

オロト酸をフッ素ガスを用いて直接フッ素化し,ついで得られたトフルオロオロト酸を脱炭酸する二段反応工程からなる5-フルオロウラシルの新規合成法にっいて検討した。フッ素化反応を効率よく行なうには,溶媒は重要な役割を果すので,これについて検討した。その結果,それ自体還元性の物質でありフッ素ガスのような強い酸化力を有するフッ素化剤を用いる反応の溶媒として用いられた例のないギ酸が溶媒としてもっとも有効であることを見いだした.また反応を円滑に行なうには7~15wt%の水を含むギ酸を使用することが必要であることがわかった。本溶媒中,-5~10℃の反応温度でオロト酸とフッ素ガスを反応させることにより5-フルオロ-6-ヒドロキシ-5,6-ジヒドロオロト酸が生成し,これは100℃で加熱することによりほぼ定量的に水を脱離して5-フルオロオロト酸に変換され,その収率は70%以上であることがわかった。フッ素化反応で得られた5-フルオロオロト酸の脱炭酸方法について検討した結果,水中で5~7atmの加圧下に150℃ 以上で加熱することにより90%の収率で5-フルオロウラシルが得られることがわかった。

Denosumab is a bone antiresorptive agent for use in patients with osteoporosis or metastatic cancer of the bones. A recent meta-analysis revealed that denosumab is associated with an increased risk of developing medication-related osteonecrosis of the jaw (MRONJ) compared with bisphosphonate (BP) treatment or placebo, although the increased risk was not statistically significant between denosumab and BP treatments. This paper presents the case of an 83-year-old man with MRONJ in the left maxilla caused by the use of denosumab for prostate cancer with multiple metastases to lymph nodes, bone and lungs, which improved by minimally invasive treatment after withdrawal of denosumab. The patient was given a subcutaneous injection of denosumab every 4 weeks for a period of 17 months. He had no history of receiving bisphosphonates or radiation therapy. We performed careful examinations and treatment with antibiotics, local irrigation and removal of as many necrotic bone chips as possible every 2 weeks. Finally, the remaining sequestrum was removed 10 months after the cessation of denosumab. The affected area was epithelialized within 19 days.
