This study describes the effect of all single-room wards on hospital-bed management and how it differs from mixed multi-bed-room wards from a case of redesigned hospital building. Surveys were conducted for 32 days by nurses in these two types of wards with a recordkeeping of each patient's bed location, patient's nursing need degrees, and the reasons of each patient-bed transfer. It showed that both types of wards have a tendency to place patients who need high level of nursing care, around the staff station. The study concludes that the all single-room wards reduce the number of patient-bed transfers especially based on patient's request for favorable therapeutic environment.

In the current laws and ordinances, the matter to accommodate the tuberculosis patient who is an airborne infectious disease is not exhibited. In this study, we discussed the building standard of the medical institution which accommodate tuberculosis patient, for the purpose of showing recommendations. The medical institution to accommodate tuberculosis patients needs to keep building facilities corresponding to the airborne infection, and it is necessary to perform use based on the latest scientific evidences.

本研究は新型インフルエンザが海外で発生した際に,水際対策の一環として濃厚接触者を対象としてホテルでの停留を行うために求められる「常識的かつ必要な」基本的空調設備の条件と,停留施設として活用する際の運営上の課題をとりまとめることを目的として実施した.そして本研究の成果物として「停留施設の空調設備リスクチェックリスト」を作成したのでここに報告する.研究の過程においては成田国際空港,中部国際空港,関西国際空港の周辺のホテルで調査を実施してまずホテルの空調システムの現状を把握した.その後に施設内での感染リスクを軽減するための空調システムの評価方法と管理手法を検討してそのまとめとしてチェックリストを作成した.最後に作成したチェックリストを利用して再度ホテルにおける調査を行い,ホテルを停留施設に転用する際の評価手法としてのチェックリストの有効性を確認した.

The study aims at finding out potential capacity for admitting casualties suffering from devastating disasters occurred in the vicinity of hospitals within a limited time frame during ordinary day to day hospital services. Three stages of analysis on admitting casualties were carried out. As the results, quite significant implications were obtained, inter alia, number of CTs / X-rays as well as the treatment space are found to be critical factors in order to complete the treatment of casualties in limited period of time.

The paper aims at clarifying how new concept of "Personal Nursing Base (PNB)"influences nursing activities in acute hospital wards in Japan. The PNB is defined as nursing base closely located to patients' beds either in the form of fixed setting or on wheels, stemming from dissemination of computerized medical and patients' record system in a hospital. The followings are the results of the surveys carried out in four hospital wards; 1) The length of direct nursing time is around 50% of total nursing duties and it has not become longer by computerization. 2) The current PC on a wheel system does not work efficiently at this moment. 3) Nurses stay longer in close to patients in PNB wards despite of the fact that direct patient care time does not increase. 4) Walking distance of nurses becomes shorter in wards which have fixed setting type of PNB.

Starting a full-scale operation of computerized information system on medical treatment and patient care, it is possible to exchange information anywhere in wards through computer terminals. It is therefore that the location and the number of nursing bases are regarded as with small limitation to distribute near to patients' beds. Personal Nursing Base (PNB) is defined as a nursing base closely located to patients' beds either in the form of fixed setting or on wheels. The paper aims at clarifying the efficiency of PNB concept compared with ordinary nursing bases in terms of shortening walking distances of nursing staff. The followings are the results of the surveys carried out in four hospital wards; 1) In three wards of the wards surveyed, PNB has been introduced. Moreover, the team nursing has been introduced in all wards, and each team is composed of one leader nurse and some member nurses. 2) Member nurses often taking records and references at PNBs located in ward passages. 3) In patients' rooms, activities of records and references are not frequently happened in the case of wards introducing computerized information system. 4) Member nurses tried to communicate with a team leader or matron. 5) Confirmation and discussion among nursing staff are not carried out by the mobile phones but by direct conversation. 6) Computer simulation shows that one PNB in each working area is effective for shortening the walking distance of nurses while more numbers of PNB have no effect.
