An alveolar bone graft, followed by the insertion of threaded titanium endosseous (Brnemark System) implants, was performed in two patients with cleft lip and palate.<br/> One patient, with bilateral cleft lip and palate, underwent an iliac bone graft after orthodontic treatment. An oronasal fistula was simultaneously closed by bilateral labial flaps and a tongue flap. Brnemark System implants were placed for treatment of congenital missing lateral incisors, 17 months after the grafting. The other patient, with unilateral cleft lip and alveolus, underwent a chin bone graft. Although necrotic bone resulting from exposure of the palatal side of the grafted bone was removed, most of the grafted bone survived. Six months later, occlusal reconstruction using Brnemark System implants was performed. The postoperative course was uneventful in both cases. Moreover, these patients were satisfied with both esthetic and functional results, which improved the patient's QOL.<br/> These results indicate that endosseous implants combined with bone grafting are more reliable than conventional procedures for occlusal reconstruction in patients with alveolar cleft.<br/>

目的:インプラント用従来型既成マグネットアタッチメントのキーパー吸着面はインプラント長軸と垂直に設定されている.したがって,もしインプラントが傾斜埋入された場合には,キーパー面は咬合平面と垂直とならない.このような場合にキーパーは大きなアンダーカットを生じることになり,維持力も減少してしまう.本試作キーパーはインプラント上のアングルアバットメントに接続するように設計開発された.また,角度補正型マグネットアタッチメントの吸引力を従来型のものと比較した.<br/>方法:マルチユニットアバットメント(Replace RP, Nobel Biocare, Sweden)に適合するように,試作キーパー(直径4.8mm,高さ3mm)をステンレス合金(72.0%Fe-26.0%Cr-2%others)より切削加工して製作した.試作キーパーは17度あるいは30度の角度付きマルチユニットアバットメントにスクリュー固定により接続した.比較試料としてPhysio Magnetキーパー#40(日立金属)を用いた.磁石構造体としてPhysio Magnet #40を両キーパーに装着し,デジタルフォースゲージ(Nidec-shimpo Corp)を装備した引張試験機に取り付けた.キーパーから磁石構造体が離脱するときの最大荷重量を吸引力(N)として測定した.測定値はt検定により危険率5%で統計解析を行った.<br/>結果:インプラントオーバーデンチャー用として,試作キーパーは容易に角度付きアバットメント上にスクリュー固定できた.試作キーパーと従来型キーパーとの吸引力を比較したところ,両者に有意差は認められなかった(<i>p</i>>0.05).<br/>結論:試作キーパーの吸引力は市販の従来型マグネットキーパーとほぼ同等であった.開発したキーパーは十分な維持力を有し,インプラントオーバーデンチャーのための適切な支台装置として使用できることが示唆された.

Successful prothodontic treatment is one of the important factors for long-term implantological success. In addressing the subject of implant prothodontics, one must consider not only the occlusal conditions but also the materials of the superstructures.<br/> Recently, castable ceramics has been introduced as biocompatible restorative materials. In particular, β-Ca(PO3)2, which is stable in H2O and acid, possesses similar physical features to tooth enamel. Thus, in the present study, color matching, strength, bone conditions of periimplant tissues, and influence on the oral environment when calcium phosphate derived castable ceramics crowns are used as a superstructures of implant were assessed. The results were as follows:<br/> 1. Color matching<br/> Color matching of castable ceramics to natural tooth is better than those of metal and thermosetting resin.<br/> 2. Crack<br/> No crack was found in the castable ceramics crowns during the observation period.<br/> 3. Influence on antagonist<br/> Antagonistic tooth did not have any clinical changes after constructing superstructures using castable ceramics.<br/> 4. Bone resorption<br/> No bone resorption was observed after cementing superstructures using castable ceramics.<br/> The results suggested that the calcium phosphate derived castable ceramics crown may be preferable to other materials as a superstructure of dental implant.<br/>

In general, next to Coagulasenegative staphylococci, the most frequent isolate, Corynebacterium species was isolated in the operatmg room.<br/> When an air-cleaner was used during the operation, the bacteria and fungi were considerably decreased, especially fungi decreased below the detectable level. To the results indicated that application of an air-cleaner was decreased air-contaminant microbes in the operating room for the dental implant surgery.<br/>

We evaluated the validity, using the Dental Prescale system, of implant-retained fixed prosthesis on masticatory efficiency in unilateral free-end partial edentulism, compared with a removable partial denture. The population of this study was as follows: implant-retained fixed prosthesis for unilateral free-end dentition, 7; removable partial denture for unilateral free-end dentition, 7; upper or lower single complete denture and the natural dentition on the opposite side, 5; complete edentulism with complete denture wearing, 5; natural dentition, 7.<br/> In the present study, total occlusal force, the occlusal force support rate of the rehabilitation area, the change of occlusal forces on the teeth nearest the edentulous area, and the asymmetry index were evaluated, and the results were as follows:<br/> 1. Both the total occlusal force and the occlusal force support rate of the implant prosthesis were superior to the removable partial denture.<br/> 2. The occlusal force on the teeth nearest the edentulous area was increased with the removable partial denture, while the implant-retained prosthesis decreased.<br/> 3. The implant-retained prosthesis showed more symmetrical occlusal force than the removable partial denture.<br/> In summary, it was suggested that the implant-retained prosthesis is more suitable than a removable partial denture to obtain the appropriate occlusal force and to protect the residual teeth.
