We implemented a questionnaire survey targeting mothers who are in child care and had participated in consultations regarding drugs and diseases. We examined the future roles of community pharmacists by exploring the mothers’ concerns and, anxieties about child cares and their backgrounds, and their expectations for profession of community pharmacist. Mothers have listed anxiety and concerns of child care about “dermatitis such as rash and atopic eczema”; “food allergies”; “infectious diseases such as measles, chicken pox, and mumps”; and “side effects of vaccination”. In addition, most of them indicated their own concerns and anxiety about “solutions to children’s illnesses.” Despite their anxieties and concerns, however, approximately 60% of the mothers have never consulted with community pharmacists. Among them, approximately a half of them indicated the following three reasons why they have never consulted with pharmacists: “I have nothing to talk about,”, “I do not know what I should talk about,”, and “I was not sure if it was alright to talk about my concerns.”. From these results, we concluded that community pharmacists in the future should improve their communication skills and inform their availability to consult about medicine and disease to local residents.

After we organized a community-based home medical care (HMC) training workshop composed of presentations by welfare and care workers in addition to patients’ family and targeting community pharmacists in collaboration with a regional pharmacy association, we carried out a questionnaire survey to the pharmacists in attendance to take hold on pharmacist’ opinions for the workshop and attitudes for HMC. The participants had a relatively high level of satisfaction regarding the workshop, rating an average of 7.81 out of 10.0. Among the participants, 77.5% had experience of HMC such as visiting pharmacy services at patient’ home, with the most widely practiced activity being “drug administration guidance for patients at home.” However, activities such as “accompany at the time of rounds” and “participation in conferences” were not widely practiced (less than 50% of the most activity) among the participants. Many participants responded that the key factors of HMC were the cooperation system between different professionals and its environmental arrangement. Overall, positive feedback from participants regarding this workshop was reported, through statements such as “I was able to rediscover the need for cooperation in a diverse team with differing job functions” and “the care of patients and their family was important.” We found many opinions that cooperation with other professionals is important in deepening pharmacists’ involvement in HMC, and that this workshop serves as a bridge to establish greater communication between care workers and pharmacists.

Placebo is commonly used in clinical trials but it is also used in clinical practice. However, it is difficult to get informed consent for the placebo use from patients in clinical practice. Thus, placebo use in such occasion can lead to an ethical problem. We have previously reported the current use of placebo and the psychological conflict of doctors and nurses about the use of placebo treatments in the actual medical settings. However, to date, no study has reported the perception of pharmacists, some of whom prepare and explain the use of placebo medicines to patients, as their medication. We conducted a survey of the current use of placebo in clinical practice and the perception of pharmacists regarding its use in 3 hospitals based on anonymous self-report questionnaire as a pilot study prior to a nationwide survey. We administered the questionnaire to 92 pharmacists belonging to 3 hospital pharmacies between July 2 and 24, 2012. Of the 92 pharmacists, 84 (91.3%) returned the questionnaire, then we analyzed the data by descriptive statistics and a contingency table. Our results showed that 54.8% of the pharmacists had experience of placebo use in medical settings, and 9.9% of them had instructed on the use of placebo ; while 43.2% of them (answered that they feel uncomfortable with the use of placebos in clinical settings) tended to feel antipathy toward the use of placebos in clinical practice. While the use of placebos in medical setting may have a potential ethical issue, 66.7% of the pharmacists considered that placebo use in clinical settings is not necessarily unethical. Pharmacists who had previously discussed this issue with other staff thought that placebo use in clinical practice is unethical (p=0.009).

In 2012, the external prescription rate was 66.1% of the national average. A dispensing doctor is recognized by the escape clauses of Article 22 of the Medical Law, Article 21 of the Dentist Medical Law, and Article 19 of the Pharmacists Act. In this study, the medicine inventories of dispensing doctors and pharmacies were compared. The medicine supplies of 7 dispensing doctors and 11 pharmacies in Saitama were classified according to the medicinal effects. We also investigated the conditions in which high-risk medicines, poisons, or drugs were stored. The average number of medicines in the medicine inventory of a dispensing doctor was 262.3 (range : 99, 439), whereas the average number of medicines in the medicine inventory of a pharmacy was 1179.7 (minimum, 275 ; maximum, 1980). Further, among these medicines, there were an average of 41.0 high-risk medicines (minimum, 18 ; maximum, 76) in the inventory of a dispensing doctor and an average of 176.7 high-risk medicines (minimum, 5 ; maximum, 299) in the inventory of a pharmacy. In addition, poisons (average, 0.3) and narcotics (average, 0.9) were found to be stored by dispensing doctors. The study results revealed that pharmacies as well as dispensing doctors stored high-risk medicines. The dispensing doctor may be indirectly associated with critical medical accidents to need cross-check by pharmacist exceedingly high-risk medicine. Thus, for patients to use medicines appropriately and for them to be reassured of the safety of medicines, only professionals such as pharmacists, rather than doctors, should dispense medicines.

A “Okusuri-techo,” which refers to a prescription record, is utilized in medical institutions as an information tool necessary for the prevention of side effects, drug-interaction, and/or in case of emergency medication. However, the record is not shared sufficiently between patients and pharmacists and thus remains underutilized. To explore how the record could be used more effectively, a survey was conducted with the pharmacists of 73 community pharmacies that were members of the Otaru Pharmaceutical Association, and patients who received prescriptive medicines from these pharmacies. A total of 148 pharmacists and 157 patients responded. Approximately 80.9% of the patient group responded that the prescription record was useful, whereas 100% of the pharmacists answered that it was helpful. In the cross-analysis of the patients’ responses, the presentation of the prescription record to medical staff was correlated with the degree of helpfulness of the record. It was suggested that the function and effective usage of the record were rarely shared between patients and pharmacists. Sufficient understanding of the value and effective usage of such a record will therefore promote its active utilization among patients and pharmacists.

As of 2011, about 12.8 million people in Japan had osteoporosis, which is a social issue in that it increases the number of patients who are bedridden or require residential care and its poor prognosis increases the mortality rate. We delivered a presentation on osteoporosis prevention to residents;the presentation consisted of ultrasound bone densitometry readings followed by a slide show, based on the readings. This report summarizes the presentation and considers whether it was effective in enhancing awareness of osteoporosis. The presentation was given to 39 residents, who were asked to complete a questionnaire before and after it. Of the 31 female respondents, 16.1 percent had bone density in the “low or caution” range;for all eight male respondents, it was “sufficient” or “average.” After the presentation, over 90 percent of respondents selected “agree” or “slightly agree” in response to the statements “I want to have regular exercise and participate in sports,” “I want to eat nutritionally sound meals,” and “understanding one’s bone density is useful for the prevention of osteoporosis.” In addition, 97.4 percent of the residents responded that the lecture was “useful” or “somewhat useful,” and 94.9 percent indicated satisfaction with it. The presentation was considered to be useful in enhancing residents’ awareness of osteoporosis.

The Medication Guides for Patients (MGPs) are being offered as information on prescription drugs for patients by the Ministry of Health, Labour and Welfare (MHLW). The MHLW published the Risk Management Plan in April, 2012, and it noted that the MGPs should be utilized in usual risk minimization activities. It is not clear, however, whether the MGPs are efficiently utilized in actual settings. Hence, we conducted a questionnaire survey of the pharmacists in the pharmacies with dispensing and the hospitals in Mie and Yamagata prefectures to investigate the actual circumstances of MGPs utilization and to understand the existing barriers associated with the use of the MGPs as medication instructions for patients. We sent the questionnaires by mail and obtained responses from 444 facilities (33.9%) of 1,309 facilities. The recognition level of the MGPs was about 30 percent in the dispensing pharmacies, and about 50 percent in the hospitals. The MGPs were utilized as a common communication tool with the patients in approximately 20 percent of the facilities. Many respondents requested that the frequency of important and other adverse reactions should be described in the MGPs, and wider ranges of MGPs should be further implemented.Moreover, our data suggests the problem is that the present MGPs are mainly applied to special types of patients, such as those with higher literacy level or those who requested a detailed explanation. Thus, it is apparent that it is necessary to review the MGPs contents again to improve their practical benefits and disseminate them more widely.
