In this study, we investigated the muscle activity that causes spinal and pelvic movements and changes inspinal and pelvic alignment when performing forward reaching in the sitting position at 0.5 seconds, which issimilar to the speed of the flexion phase of standing up movements. We measured the muscle activities of thelongissimus, multifidus and ilicostalis muscles, and the angular changes of the spinal column and hip joint of20 healthy men performing anterior reach motion in the sitting position. The results show that the activity ofall muscles weakened at the time of 20% from the start of movement and tended to increase significantly after40%. In addition, the angle change of the spinal column and hip joint showed flexion of the lower thoracicspine from the time of 20% to 40%, and after 40%, the anterior reach distance increased significantly as theflexion angle of the hip joint increased. The results of this study show that there is a difference in the purposeof exercise between the phase up to 40% of the time from the movement start and the phase that follows.

Patients with shoulder joint dysfunction may perform compensatory motion using the glenohumeral joint,scapula, or trunk to accomplish the intended movement. It is necessary to consider which treatment strategyto pursue, improvement of normal movement or acquisition of compensatory function, depending on theintended goal. In this paper, compensatory movement caused by functional impairment of the glenohumeraljoint and scapula are used as examples. In addition, electromyographic data are presented and details of thekind of compensatory movement occurring are explained.

It is necessary to interpret the behavior in order to improve the behavior of a paralyzed upper limb in patients with cerebrovascular disorders. If the motor analysis is incorrect, the behavior will not be improved by physical therapy. Therefore, motion analysis is an important first step in physical therapy. In addition, muscle tone can be adjusted by stimulating the skin and muscles in physical therapy. This paper outlines the content of physical therapy based on kinematic and physiological interpretations of the characteristic movements of paralyzed upper limbs of patients with cerebrovascular disorders.

In fishing, the act of moving the rod while winding in the line is called the rattling operation. Here we present a case of shoulder stiffness that occurred during rattling in a patient who earlier had had a rotator cuff repair by arthroscopy. The rattling movement is performed by mild external rotation of the shoulder joint. Our case had difficulty performing the movement due to a decrease in the function of the supraspinatus muscle, and stiffness was occurring on the upper surface of the left shoulder. This also negatively influenced our case’s fishing results. Physical therapy involving stimulation of the supraspinatus muscle was performed, paying attention not to over exercise the upper trapezius muscle fibers. With improvement in the function of the supraspinatus muscle, the rattling movement became possible, the stiffness disappeared, and our case’s fishing results improved. The rattling movement is highly dependent on rotator cuff function.

Arm elevation movement is used in daily living. We have performed physical therapy for patients with arm elevation difficulty due to problems with the shoulder joint and trunk function. We think self-exercise is important for effective rehabilitation. Therefore, we examined the concepts behind self-exercise for patient with arm elevation difficulty and describe them in this report.

Physiotherapists often have opportunities to perform physical therapy for patients with hemiplegia due to cerebrovascular disorders (among patients with central nervous system diseases), during the acute, recovery and chronic phases of the condition. Physical therapy is performed for patients who have difficulties in maintaining a sitting posture or performing activities while sitting, or who have walking difficulties or difficulties in performing activities while standing, due to trunk or pelvic dysfunctions. The authors consider that physical therapy, focusing on the major causative dysfunction, should be performed for patients at any phase of the disease, and that this requires a proper knowledge of kinesiology and anatomy. In this special topic, we will discuss the main measures for reducing the trunk dysfunction of patients with hemiplegia due to cerebrovascular disorders.

Foot function deteriorates after varus sprain of the ankle joint, and impairs smooth body movement in some cases. Dysfunction of the peroneal muscles, such as muscle weakness and delayed reaction time, occurs after varus sprain, and great importance has been attached to strengthening of the peroneal muscles in exercise therapy. Our exercise therapy targets stable posture control in weight-bearing on the little toe side, and we consider that not only the peroneal muscles but also varus muscles of the foot are important with regard to muscle activity in this position. In this study, we investigated muscle activity of the varus muscles of the foot and plantar pressure distribution on the supporting side in inward step positions as an exercise menu. Compared to the baseline position regarded as 0 cm, muscle activity increased at an inward step distance of 10 and 20 cm, and significantly increased at 30 and 40 cm (p<0.01). It is suggested that the inward steps induced weight-bearing on the little toe side, and forced tilting of the lower leg outward, during which the varus muscles may have acted to brake the outward tilting of the lower leg. This suggests that the varus muscles of the foot are important for posture control in weight-bearing on the little toe side.

Lateral shift using one hand while sitting on a seat is an important ability in sitting up, for locomotion, to reach the other side. We sometimes control muscle tone of the trunk to change lateral weight bearing on one hand in hemiplegia caused by cerebrovascular diseases in which movements are difficult to perform because of trunk collapse. In this study, we analyzed the activity of the trunk muscles (obliquus abdominis, obliquus externus abdominis, obliquus internus abdominis and lumbar back) with EMG in changing the weight bearing in lateral shift with one hand while sitting on a seat. The results show that the supporting hand is necessary in sitting on the seat. The supported obliquus abdominis muscles, obliquus externus abdominis muscles and lumbar back muscles acted to maintain and fix the trunk as the weight increased. The supported obliquus external abdominis muscles acted to stabilize the upper trunk and rib cage, and the unsupported obliquus abdominis muscles, obliquus internus abdominis muscles and lumbar back muscles elevated pelvis as the weight decreased. Therefore, it is suggested that in the change of weight bearing in lateral shift the supporting hand serves an important purpose in maintaining trunk stability.

We studied the relationship between the stand-up test using standing steps of different heights and isokinetic muscle strength of knee extension. In the stand-up test, whether or not a subject could stand-up on both legs or one leg from steps of different heights was assessed. As a result, the peak torque/body weight ratio was significantly greater in subjects who could stand on one leg than those who could not (p<0.05). In the subjects who could stand on one leg, the angle of peak torque (p<0.01) and total work (p<0.05) were significantly greater when using a step of lower height. It was revealed that while quantitative muscle strength (peak torque/body weight ratio) could be roughly evaluated by determining whether the subject could stand on one leg, the height of the step used in the stand-up test did not contribute to detailed evaluation of knee extensor muscle strength. However, using a step of lower height, it was found that the knee extensor muscle could function from the large flexion of the knee joint, thus making it possible to evaluate the qualitative muscle strength (angle of peak torque).

The daily routine of changing clothes and underclothes for patients undergoing physical therapy can sometimes be a problem. Clothes of the patients need to be changed several times a day, for example, when going outdoors and for other functional and social purposes. Changing clothes can be a problem for patients, depending on the kind of clothing, the patients' posture, and the method for changing clothes. Detaching, removing, or raising and lowering of clothes can also be a problem for patients, depending on whether the patient is lying down, sitting, or standing. This poses the question: How do physical therapists guide patients regarding changing clothes and underclothes? This study examined the operations of detaching, removing of clothes, and raising and lowering of underclothes. The study observed the normal operation of changing underclothes, using healthy people as subjects. The participants were videotaped. Additionally, the participants' centers of pressure (COP) were determined using a stabilimeter, and electrical muscle activity of the supporting leg was monitored by surface electromyography.

Changing underclothes is an activity that everyone performs every day and there is a lack of objective information derived from observations of people performing this activity. Therefore, a physical therapist must rely on his or her own experience while guiding a person who is changing their underclothes. However, the physical therapist should observe and understand the movements involved. In this study we examined the activity of the abdominal muscle group in healthy persons during a change of underclothes. Observations on the angle of inclination of the pelvis in the long-sitting and chair-sitting positions were made. Integrated electromyographic values of the abdominal muscle group tended to decrease with increasing inclination angle of the pelvis. Our results suggest that the angle of the pelvis during a change of underclothes affects the activity of the abdominal muscle group.

In the physical therapy field, physicians encounter some patients who cannot maintain their posture in the standing position because of decreased activity of the lumbar spine. In such cases, forward weight shifting in the standing position may be performed. The purpose of this study was to examine postural changes and muscle activity during forward weight shifting while standing. We observed that during forward weight shifting, the activity of the multifidus muscle (L4/S1) increased from the initial stage, with the anteversion of the lower legs and pelvis. To maintain the posture of the lower lumbar spine, the muscle activity of multifidus (S1) must start prior to that of multifidus (L4).

Manual muscle testing is used to assess movement during arm elevation. Prior studies of movement during arm elevation have not assessed concurrent elbow joint motion. However, arm elevation in activities of daily living is performed not only with movement of the shoulder joint and shoulder blades, but also with elbow joint movement. Therefore, arm reach movement and arm elevation are different actions. This paper discusses research to date on arm reach movement and describes assessment and exercise therapy methods.

Visual observation and electromyographic evaluation of the movement from the sitting position to the supine position were performed. In the movement from the sitting position to the side-lying position, the forearm of the upper limb supports the lateral bending of the trunk after elevation of the pelvis. The pelvis is elevated by the muscle activity of the oblique muscle on the opposite side, which is important.

To deepen understanding of the normal for sitting from supine, 10 men and women were examined using motion analysis and electromyography. Motion was classified into three categories, namely sitting from supine with arm support on one side, sitting from supine with arm support on both sides, and sitting from supine with the sitting position with extended legs. The muscle activities of the right and left rectus abdominis muscles and right and left abdominal oblique muscle overlapping sites were examined using electromyography. A common feature of the three motions was the absence of rotation or side bending of the trunk. In addition, as segmental rotation and lateral bending of the trunk do not occur, the activities of the upper limbs are suggested to be important with regard to raising and rotating the body.

We recommend performing a top-down physiotherapy evaluation rather than a bottom-up evaluation. During an assessment, it is desirable to detect problems at the impairment level and narrow them down to one problem in terms of order and relevance. By intervention for narrowed-down problems and re-evaluation of changes, a clearer physical therapy can be developed. In this study, we introduce abnormal postures and movements that are characteristics of patients with musculoskeletal diseases involving lumbar and hip joint impairment. Furthermore, we present one problem focusing on muscle weakness and decreased muscle tone as the possible underlying cause of impairment. We explain intervention methods for this impairment using kinematic and anatomical viewpoints and electromyography.

The efficacy of physical therapy for a patient with impaired hair-washing motion due to dysfunction of the inferior fibers of the trapezius was assessed. Surface electromyography was performed to investigate the clinical implications, and to elucidate the relationship between changes in the speed of elbow joint flexion-extension during simulated hair-washing motion and the corresponding activities of the superior, intermediate, and inferior fibers of the trapezius and of the caput longum of triceps brachii. In a sitting position, the subject moved the shoulder joint approximately 110 degrees in flexion and the elbow joint approximately 45 degrees horizontally on the measured side, and performed flexion-extension of the elbow joint in which the middle finger on the measured side reciprocated between the torus occipitalis and vertex in a hair-washing motion-simulating task. The motion speed was set at 40, 80, 120, and 160 motions per minute using a metronome and electromyograms of the superior, intermediate, and inferior fibers of the trapezius and the caput longum of triceps brachii were measured for 10 seconds 3 times during each task. Electromyogram waveforms of these fibers at each speed and the influence of changes in the elbow joint flexion-extension speed on the relative integrated electromyogram value were investigated. In the caput longum of triceps brachii, the amplitude of the electromyogram waveform in a single elbow joint flexion-extension motion was compared among the tasks at different speeds. The waveform amplitude increased and the relative integral value tended to increase as the elbow joint flexion-extension speed increased only in the inferior fibers of the trapezius. These findings suggest that to evaluate and improve hair-washing motion in patients with impairment of the scapulothoracic joint, the arm should be maintained in an elevated position during this motion. Furthermore, attention should be paid to the activity of the inferior fibers of the trapezius, which are assumed to be involved in stabilization of the scapula during the elbow joint flexion-extension motion, in this posture.

We report on the use of physical therapy for a patient with right hemiplegia experiencing difficulty with getting up from a bed after cerebral infarction. Based on a previous study, the aim was for the patient to get up by rolling over to the non-paralyzed side to attain the sitting position. Observation of the getting up action of the patient revealed that the trunk flexion and rotation of the non-paralyzed side were insufficient. Therefore, the patient tried to lift the upper body by pushing with the non-paralyzed side elbow on the bed. However, the patient had difficulty performing the getting up action, because the upper body fell down to the rear due to paralysis on the side of rotation of the trunk. Our assessment showed hypotonia in both parts of the rectus abdominis, and in the external oblique muscle and serratus anterior muscle of the paralyzed side. Hypertonia was also found in the posterior fibers of the deltoid muscle and upper fibers of the latissimus dorsi muscle on the non-paralyzed side. After 1 week of physical therapy, the impairments improved and the patient was able to perform the getting up movement.

We report a case in which significant edema was found in the paralyzed foot, which showed ankle plantar flexion, varus position and toe flexion in walking, disturbing the walking movement because of insufficient load to the forepart of the foot. In this case, the patient's foot grounded first with its post-outside when standing on the paralyzed foot due to abnormal alignment on the paralyzed side. Therefore the knee joint couldn't move forward, causing knee joint flexion and trunk anteversion and compensatory tone elevation of both back muscles. We performed therapy to correct the abnormal alignment of the paralyzed foot, which remedied the compensatory tone elevation of both back muscles due to loading of the paralyzed foot sufficiently. We conclude that it is necessary to ensure the foot is fully loaded when perfoming physical therapy for patients who have trouble with the trunk due to the foot during walking.

This study examined how differences in the center of foot pressure in the sagittal plane affect the activity of muscles surrounding the hip joint in healthy young male subjects. With the subjects standing on one leg, integrated electromyography values of the muscles surrounding the hip joint, the point of center of pressure in the sagittal plane (Y-axis center displacement), and the trace length of the center of pressure in the frontal plane (X-axis trace length) were measured for three different loads with the center of foot pressure in the forefoot, midfoot and hindfoot regions. There was no significant difference in X-axis trace length between the three loading conditions. The activities of the rectus femoris, tensor fasciae latae and adductor magnus muscles were higher when the load was borne on the hindfoot than in the other two loading conditions. This indicates that these muscles have a flexor effect on the hip joint in the sagittal plane, and that they control the posterior movement of the center of gravity of the body. In addition, we consider these muscles control the center of gravity of the body in the coronal plane at the hip joint.
