【背景】今回, 水痘を罹患した後に脳梗塞を発症した小児の1例を経験したので報告する。【症例】4歳の男児。右上下肢脱力で当院へ救急搬送された。来院後, 救急外来で一過性の右上下肢麻痺および構音障害が確認された。頭部MRI検査拡散強調画像で左内包後脚に高信号域, MRA検査で左中大脳動脈本幹の血管狭小所見を認め急性期脳梗塞と診断し, 同日入院した。入院5週間前に水痘罹患歴があり, 入院時の髄液検査より水痘・帯状庖疹ウイルス (varicella zoster virus : VZV) DNAが検出され, 水痘罹患後の血管炎による脳梗塞と診断した。アシクロビル (Aciclovir : ACV), プレドニゾロン (Prednisolone : PSL), 抗血栓薬投与により症状は単相性に改善し, 第18病日に自宅退院となった。【考察】小児期の脳梗塞の原因は, 成人と比較し多岐に渡り, 水痘罹患も主要な原因の一つである。小児脳梗塞症例においては, 基礎疾患の検索のほか, 水痘をはじめとした感染症の罹患歴ももれなく聴取することが重要である。

塩素ガスへ暴露すると上気道粘膜の刺激症状や急性肺損傷acute lung injury(ALI)などの呼吸器系障害が生じるが,ALI発症までには数時間以上を要するという報告が散見されている。今回我々は,塩素ガス吸入後に遅発性のALIを発症した 2 症例を経験したので報告し,ALI発症までの時間と症状出現から増悪までに要する時間について検討した。症例 1 は26歳の女性で,自殺目的で 3 種類の洗剤を混合し,発生した塩素ガスを吸入した。来院時無症状であったが,受傷10時間後に低酸素血症が出現し,胸部X線撮影・胸部CTで両側肺の浸潤影が出現した。ALIの診断でsivelestat sodium hydrateを投与し,受傷 6 日目に軽快退院した。症例 2 は64歳の男性で,塩素含有化学物質を誤って混合し,発生した塩素ガスを吸入した。来院時より上気道症状や低酸素血症を認めたが,受傷35時間後に病態が増悪し,胸部CTでのスリガラス状陰影の出現を認め,ALIと診断した。ステロイドを経口投与し,PaO2/FIO2 ratioは改善した。本 2 症例の経過と,塩素ガス暴露による症例報告や動物実験の報告から検討すると,症状の出現までに10時間程度の時間を要し,受傷後48時間程度で病態が最も増悪することが予測された。以上より,塩素ガスに暴露した患者では,来院時無症状であったとしても10時間程度の経過観察を行い,有症状の患者に対しては48時間程度の慎重な経過観察が必要と考えられた。

This study clarifies the transformation process of the building arrangement and gardens on the detached housing area. The study area is the planned housing area in Kami-Kotoen, Nishinomiya. In this area has developed since about 50 years, has not been decided the district plan and the building agreement, has maintained its living environment. The main information obtained is as follows: (1) This area has the four phases of the transformation process. (2)The extended and rebuild time relate with the change of family member. (3)The site arrangement has the site use divided nine parts, based on the original house.

The population of Japan will change to a decrease. In such situation, it is important to pay attention where population had kept increasing. This study aims to clarify the feature about population increase and to clarify the feature of Boze island according to the population increase The following four points are understood. 1) the feture of population increasing at Boze island. 2) the transformation of Boze island related to the population increase 3)the folk customs, and the feature of residents' composition at home, in Boze island 4) Possibility that residents have removed frequently in this island Above, the dwelling system of this island is considered.
