症例は65歳の2型糖尿病男性で経口薬・インスリン併用中だが,高血糖,肥満が是正されないためSGLT2阻害薬を導入すべく入院となった.メトホルミン,利尿薬内服を中止の上でイプラグリフロジン50 mg開始.血糖改善は良好で2日目から約10 %のインスリン減量を実施.3日目に全身性皮疹,腎機能障害(Cr 1.6 mg/dl),ケトーシス(3-OHBA 626 μmol/l),軽度代謝性アシドーシス(PH 7.348, HCO3- 18.3 mmol/l)に加え高度高K血症(7.3 mEq/l)を来した.症例は高K既往,K高含有食品(昆布)常用,ARB内服など高Kを呈しやすい素地があった.これに利尿薬中止,SGLT2阻害薬開始後の腎機能低下,アシドーシス,インスリン作用不足によるK細胞外シフトが加わり高Kを発症したと推察する.高Kを呈しやすい背景の患者へのSGLT2阻害薬導入ではK値に注意を要する.

In this study, a new education teaching material using a steam locomotive model to acquire technical skills of high accuracy and high-density assembly is proposed. The proposed teaching material is processed by hand finishing, a turning process, and welding. In this teaching material, the machining and assembling accuracy are evaluated by pressure values, which are required for running, as well as the travel distance of the steam locomotive model. Thus, this teaching material can be used for introductory education for acquiring manufacturing technologies, because the importance of the machining and assembling accuracy can be understood.

筆者は2008 年8月26 日に上海博物館所蔵青銅兵器の調査を実施した。上海博物館は銘文を持つ戦国時代(紀元前5世紀半ばから紀元前221 年)製造の青銅器を多数所蔵する。 今回、総合的な金文著録である『殷周金文集成』(全18 冊、中華書局、1984 年~ 1994 年)に収録されている15 件の有銘青銅兵器について調査を実施した。本稿はその調査報告である。この15 件は戦国七雄の中でも中原に位置した戦国三晋諸国(韓・魏・趙)において製造された青銅兵器(以下、三晋兵器と略称)である。 2007 年4月、『殷周金文集成(修訂増補本)』(全8冊)が刊行された。本書は『集成』の修訂増補版であるが、とりわけ戦国青銅兵器部分について面目を一新する内容となっている。本調査は新版の性格と出版の意義を改めて示すことになった。本稿では実見した15 件の青銅兵器銘文の分析を基礎に、『集成』収録の三晋兵器全体(141 件)について旧版との移動を確認した。 検討の結果、新版では60% を超える新模本の作成が見られ、近20 年の戦国青銅兵器研究の成果をふまえた時期区分の精密化や新しい研究の積極的な採用など見られることを指摘した。本書は今後の戦国文字・戦国史研究の展開にかかせない必備の金文著録といえる。This article describes our investigation report which was carried out on bronze weapons at the Shanghai Museum, on 2008/08/26. The Shanghai Museum has a collection of numerous bronze weapons (of the San Jin 三晋), created during the Warring States Period in China. With the provision of scientifi c research expenses and also of support from all concerned, we were able to gain access to collections normally inaccessible. As a result of our investigation, we succeeded in confi rming the erroneous/missing world and phrases in Shiwen 釈文(The Chinese University of Hong Kong Institute of Chinese Studies,2001),which is supposed to contain the same contents as Yinzhou Jinwen Jicheng (Zhong Hua Book Co.1983 ~ 1994).The result of our investigation reveals the characteristic features of Yinzhou Jinwen Jicheng (revised and expanded edition)(Zhong Hua Book Co,2007), and also the signifi cance of its publication.
