A series of α-cyanocinnamamide derivatives was synthesized and evaluated for inhibitory activity against tyrosine-specific protein kinase using intact plasma membrane fractions from an epidermoid carcinoma cell line, A-431 cells. Among these compounds, several novel α-cyano-4-hydroxy-3, 5-disubstituted cinnamamide derivatives, e.g., α-cyano-3-ethoxy-4-gtdrixt-5-phenyl-thiomethylcinnamamide (ST 638), showed potent inhibitory activity. The studies on the structure-activity relationship revealed that the presence of the hydroxy group at the 4 position and the double bond in the α-cyano-4-hydroxycinnamamide skeleton was important for potent inhibitory activity, and that the presence of hydrophobic groups at the 3 and 5 positions on the benzene ring also enhanced the inhibitory activity of α-cyano-4-hydroxycinnamamide derivatives.

胃石イレウスは比較的稀な疾患であり,術前診断が困難なことが多い.今回われわれは,胃石による小腸イレウスの1手術例を経験したので若干の文献的考察を加えて報告する. 症例は63歳男性,数日前より嘔気,嘔吐を繰り返し,症状の増悪を認めたため近医受診し,上部消化管造影を施行したところ十二指腸下行脚に陰影欠損を認め,十二指腸腫瘍によるイレウスの疑いで当院紹介入院となった.上部消化管内視鏡を施行したところ,十二指腸下行脚には病変は認めず,鉗子孔からのガストログラフィンによる造影で,空腸に体位にて移動する陰影欠損を2カ所確認した.以上により,異物(胃石)イレウスと診断し自然排出を期待して保存的治療を試みたが9日間経過しても排出されず,外科的療法(胃壁切開)により,計5個の胃石を摘出した.摘出した胃石の成分分析よりタンニン98%の結果を得,柿の常食の嗜好もあることから,柿胃石によるイレウスと診断した.

国連人権理事会を創設した2006年3月の国連総会決議60/251は、「理事会が、重大かつ組織的な侵害を含む、人権侵害の事態に対処」すべきことを定める。新設の普遍的定期審査(UPR)については、基本的に人権侵害事態対処機能とは異なる趣旨のメカ二ズムであることが確認される一方、「アラブの春」の文脈では、人権理事会の活動にその発足以来はじめて積極的なアプローチが見られた。しかし、その常態化は困難であり、特別手続や人権高等弁務官などによる注意喚起が、人権理事会の審議や決議の「引き金」となるメカニズムを確立させることが必要である。

Oxazolomycin and neooxazolomycin are structurally closely related antibiotics isolated from Streptomyces sp. by Uemura et al. The former is the parent member of a class of polyene bicyclic antibiotics, other members being oxazolomycin B and C, 16-methyloxazolomycin, and curromycin A and B. The oxazolomycins were found to exhibit wide ranging and potent antibiotic activity, including inhibitory activity against Gram-positive bacteria, antiviral activity against vaccina, herpes simplex type I and influenza A, as well as in vivo antitumor activity. The intriguing molecular architectures and the biological activities make these compounds attractive targets for synthesis. However, the total synthesis is limited to Kende's synthesis of neooxazolomycin. We report here a novel approach to neooxazolomycin, which can be also applicable to the synthesis of oxazolomycin. Our synthesis of right hand core 22 started with methyl (S)-hydroxyisobutyrate and proceeded through three major transformations involving regio- and stereoselective iodination via intramolecular Pt-catalyzed hydrosilylation, Pd-catalyzed enolate alkenylation, and stereoselective dihydroxylation accompanied by concomitant lactonization. Nozaki-Hiyama-Kishi coupling of aldehyde 23 obtained from 22 with N-Fmoc-iododienamine 24 gave 7S-isomer 25 and 7R-isomer 26 as a 1: 1 epimeric mixture. It was gratifyingly found that Dess-Martin oxidation of this epimeric mixture followed by L-Selectride reduction of the resulting ketone produced the desired 7R-isomer 26 in excellent stereoselectivity (94% de). Removal of the silyl protecting group allowed us to obtain the Kende's intermediate 27, the synthesis of which constitutes a formal synthesis of neooxazolomycin.

虫垂原発悪性リンパ腫の1例を経験したので, 本邦報告例の検討を加えて報告した. 症例は42歳, 男性. 下腹部痛を主訴として近医を受診し, 急性虫垂炎と診断され当院を紹介された. 虫垂穿孔性腹膜炎の診断で虫垂切除術を施行したところ, 術後の病理学的検索で non-Hodgkin's lymphoma, diffuse, large cell type と診断された. 根治術を目的として第10病日に R_3 リンパ節郭清を伴う右半結腸切除術を施行したが, 切除標本のいずれにも悪性リンパ腫の所見を認めなかった. 虫垂原発悪性リンパ腫はきわめてまれな疾患であり, 本邦報告例は21例にすぎない. 多くは急性虫垂炎の診断で手術がなされ, 術後の病理学的検索で確定診断されていた. 根治術がなされたものは7例 (33.3%) のみであった. リンパ節転移陽性の9例中3例, 陰性の8例中7例が生存中とされていた. 治療は癌腫と同様に根治的切除が第1であり, 症例により後療法が必要と考えられた.
