It is obvious that pharmacists play a critical role as risk managers in the healthcare system, especially in medication treatment. Hitherto, there is not a single multicenter-survey report describing the effectiveness of clinical pharmacists in preventing medical errors from occurring in the wards in Japan. Thus, we conducted a 1-month survey to elucidate the relationship between the number of errors and working hours of pharmacists in the ward, and verified whether the assignment of clinical pharmacists to the ward would prevent medical errors between October 1-31, 2009. Questionnaire items for the pharmacists at 42 national university hospitals and a medical institute included the total and the respective numbers of medication-related errors, beds and working hours of pharmacist in 2 internal medicine and 2 surgical departments in each hospital. Regardless of severity, errors were consecutively reported to the Medical Security and Safety Management Section in each hospital. The analysis of errors revealed that longer working hours of pharmacists in the ward resulted in less medication-related errors; this was especially significant in the internal medicine ward (where a variety of drugs were used) compared with the surgical ward. However, the nurse assignment mode (nurse/inpatients ratio: 1 : 7-10) did not influence the error frequency. The results of this survey strongly indicate that assignment of clinical pharmacists to the ward is critically essential in promoting medication safety and efficacy.

2018年8月より4カ月おきに、Google AI スピーカー(Google Home)へ、新潟大学医歯学総合病院(以下、当院)での採用医薬品各銘柄名、当院薬剤部医薬品情報管理室(以下、DI室)に寄せられた医薬品に関する問い合わせについて質問を行い、その回答の記録を続けている。当院採用医薬品銘柄名を質問した時、薬効等を回答する割合は、2018年8月の43.93%から、2020年8月には82.13%へと向上していた。DI室への問い合わせについては2018年8月では意味がある回答は0.81%であったのに対して、2020年8月には9.77%で回答が得られた。2019年12月よりAmazon AIスピーカー(Alexa Echo Show)でも当院採用医薬品各銘柄名の質問を行っているが、2020年8月の回答割合は9.95%であった。

Renal transplant recipients are administered immunosuppressive therapy to prevent acute rejection. In particular, new immunosuppressive agents have helped to improve the allograft survival rate and reduce the rate of rejection in renal transplant recipients. The optimal dose of calcineurin inhibitors is determined by therapeutic drug monitoring. However, the pharmacological efficacy of cyclosporine and tacrolimus should be estimated using both pharmacokinetics and pharmacodynamic parameters. We therefore employed the lymphocyte immunosuppressant sensitivity test(LIST) with the 3-(4,5-dimethylthiazol-2-yl)-2,5diphenyltetrazolium bromide(MTT)assay procedure to evaluate renal transplant recipients. The LIST with the MTT assay procedure can predict the pharmacological efficacy of immunosuppressive drugs using peripheral blood mononuclear cellsMoreover, renal transplant recipients must be correctly treated with immunosuppressive agents and another medicines. Therefore, the pharmacist must provide instructions for all medications to maintain adherence in renal transplantation. Therefore, transplantation therapy must be based on the pharmaceutical care given by pharmacists.
