We conducted a randomized, controlled study to evaluate whether pharmacists' advice on smoking cessation would result in a higher smoking cessation rate using Nicorette (nicotine gum preparation). Fourteen pharmacies in Tokyo, Kanagawa, and Nagano participated. Smokers who visited pharmacies to buy Nicorette from March 1, 2002, through August 31, 2002, were recruited and randomly assigned to two groups. For the intervention group (A), pharmacists provided both regular instructions on Nicorette use and smoking cessation advice at the first sale and then gave follow-up advice just before starting a cessation and 1, 3, and 8 weeks and 3 months thereafter. For the control group (B), pharmacists provided regular instructions alone. The primary outcome measure was the self-reported smoking cessation rate and the secondary outcome measure was the relationship between the smoker's egogram and effectiveness of intervention. Twenty-eight smokers were enrolled and randomized into group A (n=11) or group B (n=17). The absolute abstinence rate in groups A and B at 3 months was 45.5% and 31.2%, respectively. The odds ratio was 1.83, which was not statistically significant. There was no difference in egogram score between absolute abstinence subjects and nonabstinence subjects in group A. The egogram scores in Adapted Child of absolute abstinence subjects in group B were significantly higher than in nonabstinence subjects. In conclusion, instructions and advice given by pharmacists may improve the smoking cessation rate in smokers receiving nicotine replacement therapy.

【Objective】 Nowadays, the pharmacist’s work changed from product-centered to patient-centered care. In this research, we make it clear through patient’s talk what a patient expects of a family pharmacist and a pharmacy. We think it is utilized to construct a communication standard required for family pharmacists. 【Method】 Focus group interviews were conducted with 3 groups of 11 patients who utilize pharmacies, recruited through snowball sampling. Interview data were categorized using qualitative analysis method. 【Results】 As a result of analysis, 7 categories and 43 subcategories were generated. Based on these, a diagram was created representing the relationships between categories. 【Discussion】 The generated categories indicated “factors influencing patient’s relationship with pharmacists” and subcategories indicated “requests to or expectations from pharmacists by patients.” Many of them overlapped with the image of “family pharmacist/pharmacy” depicted in the “Pharmacy Vision for Patients” indicated by the Ministry of Health, Labour and Welfare in 2017. Meanwhile, patients wanted attitude, ethics, and sense of mission as basic qualifications originally required in a medical professional. Moreover, the research showed broader viewpoints and expectations for advanced communication skills such as support of patient’s self-determination of medication, relationship between the patient and the physician, and consideration of social and economic background of the patient.

本研究は、医療者-患者間のコミュニケーションギャップに注目し、臨床現場での有意義なインフォームド・コンセントの実現に還元すると共に、医療スタッフや薬学部生を対象とした医療コミュニケーション能力養成のための教育プログラム構築にも寄与することを目的として「がん患者への医療者からの説明」について質的検討を行った。その結果、同じ内容の説明をしたとしても、個々の患者の治療に対する感じ方や理解度などにより受け取り方は異なり、医療者が患者の個別性を尊重する重要性が示唆された。

During our efforts to develop a training program for new pharmacists, we have come to the opinion that it is essential to understand the personality characteristics of each individual pharmacist. The present study was undertaken to examine the effects of various personality characteristics of new pharmacists regarding their work by psychological tests with existing education programs. The ultimate goal of obtaining basic data would be useful for devising effective training plans for new pharmacists. The subjects consisted of 16 new pharmacists who participated in the training program for new pharmacists conducted at the Q Pharmacy in April through June of 2001. A psychology test, the Tokyo University Egogram (TEG), was administered at the beginning and at the end of training, and the State-Trait Anxiety Inventory (STAI) was administered at the beginning and at the end of training and once a month for two months.An analysis of changes in egograms taken both before and after the training suggested that the effects of the training were manifested in their personality even after a short period of training.Some personality characteristics that seem desirable for pharmacists were derived from the data obtained for pharmacists with highly appraised performances. These include moderately high Critical Parent (CP) and Free Child (FC), high Nurturing Parent (NP) and Adult (A), and moderately low Adapted Child (AC). It is possible that more effective training can thus be provided if not only the trainee but also the trainer understands one's own personality haracteristics.
