Inferior alveolar nerve and lingual nerve injury is common in the field oral surgery. Nerve injuries are induced by extraction of impacted teeth, and implant impaction, and many cases require surgical nerve repair. Partial amputation injury case and the nerves complete amputation case are adaptation of the nerves surgical repair for severe nervous part equivalent to neurotmesis of the Seddon classification, but when nerves surgical repair is not performed immediately, traumatic neuroma can develop and cause neuropathic pain; accordingly, early diagnosis and treatment are important. The gold standard of nerve surgical repair was nerve end-to-end suturing, and autologous nerve graft. Nerve repair surgery can provide a reasonable result in improving sensation in the inferior alveolar and lingual nerve. More than 80% of patients experienced some improvement in sensation, and dysesthesia did not develop after surgery in any patient who did not have it before surgery. This article will be explained not only surgical therapy but also basic research on peripheral neuropathy to clinical practice.

Schwannoma is a benign tumor originating from Schwann cells, occurring rarely in the masseter muscle. In this report, we describe a case of schwannoma in the masseter muscle. A 35-year-old man was referred to our hospital because of swelling of the left cheek. A painless elastic hard mass was palpable in the left cheek. No abnormalities were found in the skin, oral mucosa, or cervical lymph nodes. MRI T2-weighted images showed a high signal area with 43×37mm internal nonuniformity and clear boundaries. Although malignant atypical cells were not detected by fine-needle aspiration and incisional biopsy, no definitive diagnosis was obtained. The lesion was diagnosed as a benign tumor of the masseter muscle and resection was performed under general anesthesia. It was located in the masseter muscle without any adhesion to surrounding tissues. No nerves were continuous with the tumor. The tumor was 45×40×30mm in size and was covered with a capsule. Histopathological diagnosis was schwannoma. After the operation, left facial nerve palsy was recognized, but it was completely cured 3 months after the operation. In addition, neither masseter atrophy nor masticatory function was observed. Two years have passed since the operation without recurrence of the tumor.

1995年1月から2004年12月の10年間に本学会評議員が所属する61施設で,エナメル上皮腫と診断,治療された947症例に対してアンケートによる疫学的調査を実施した。性別は男性581例,女性366例であり,年代別にみると男性は20歳代で18.6%とピークを示し,女性では10歳代で23.2%とピークを示した。また部位では臼歯部が最も多く55.6%であった。臨床症状では疼痛が46.6%と最も多く,次いで腫脹が13.6%であった。エックス線所見は単房性が50.7%,多房性が40.4%であった。2005年のWHO歯原性腫瘍組織分類ではsolid/multicystic typeが74.5%と最も多く,次いでunicystic typeが17.0%,desmoplastic typeが4.1%,extraosseous/peripheral typeが3.0%であった。治療法では,顎骨保存療法(開窓145例15.9%,摘出開放創187例20.5%,摘出・掻爬289例31.8%を含む)が74.0%,顎骨切除療法が24.1%であった。
