Objective: There are insufficient coherent reports on mechanical thrombectomy (MT) for occlusion of the second segment of the middle cerebral artery (M2 occlusion) in a real-world clinical setting. We evaluated the efficacy and safety of MT for M2 occlusions and compared the primary thrombectomy strategies (stent retriever, aspiration catheter, and combined technique) to analyze factors predicting good functional outcomes.Methods: We evaluated background factors, preprocedural factors, procedural factors, and procedural time for patients who underwent MT for M2 occlusions from our retrospective cohort. According to the modified Rankin Scale (mRS) score three months after MT, patients were divided into good (mRS ≤2) and poor (mRS ≥3) prognosis groups.Results: A total of 29 patients (median age, 78 years; 11 [37.9%] females) were included in the study. In this cohort, rates of successful reperfusion, thrombolysis in cerebral infarction (TICI) 3, postprocedural hemorrhage (PPH), and symptomatic PPH were 82.8, 34.5, 31.0, and 0%, respectively. Good prognoses were achieved in 13 (45%) cases. A prognostic factor of MT for M2 occlusions is TICI 3 from multivariate analysis (OR, 11.7; 95% CI, 1.003–136; p = 0.0497). There was no statistically significant difference in the functional outcome three months after MT based on the choice of the primary thrombectomy strategy.Conclusion: MT for M2 occlusions is a reliable and relatively safe procedure. The presence of TICI 3 was a prognostic factor in this cohort. Future studies are warranted to investigate the optimal thrombectomy strategy for medium vessel occlusion.

Objective: Knee valgus during jump landing is a cause of knee injuries during sports activities. Body recognition is important for maintaining the knees and other body parts in their proper positions. The aim of this study was to investigate whether knee valgus during jump landing in healthy young women is related to the inaccuracy of recognition of bilateral knee positions in the squatting position.Methods: In 39 healthy young women, the degree of knee valgus was evaluated during the drop vertical jump test using the ratio of the knee separation distance to the ankle separation distance. The accuracy of recognition of bilateral knee positions in the squatting position was evaluated by having the blindfolded subjects indicate with their index fingers the subjective positions of their bilateral patellae by placing their fingers on a horizontal bar positioned in front of them 3 cm below the navel. The difference ratio of the recognized distance to the actual distance between the bilateral patellae was measured as an inaccuracy index.Results: The degree of knee valgus during the drop vertical jump test was positively correlated with the degree of inaccuracy of the recognized knee position with the knees in the neutral position (r=0.358, P=0.025).Conclusions: In healthy young women, knee valgus during jump landing was significantly correlated with the inaccuracy in knee position recognition in the squatting posture. This finding suggests that the assessment of knee position recognition in the squatting position could be useful as a screening tool for preventing knee injuries on jump landing during sports activities.

集住化とは広範囲に分散する小規模な集落を計画的に造られた大きな町に統合する政策であり,スペイン統治下のアメリカ全土で実施された。そのおもな目的は先住民のキリスト教化を促進し,租税の徴収と賦役労働者の徴発を容易にすることだが,それに加えて,人間は都市的環境でのみその本性を発揮する,という考え方が背景にあった。第5代ペルー副王フランシスコ・デ・トレドが1570年代にアンデスで実施した政策は,約140万の人びとを800以上の町に集住させる大規模なものであり,在来の居住形態,社会組織,権力関係,アイデンティティを大きく変えたといわれている。 本論文は,2013年10月24日にリマの教皇庁立ペルーカトリカ大学で開催された国際公開セミナーの成果であり,副王トレドの集住化について近年刊行された以下の3冊の研究書の学術的意義を論じている。従来の研究では,トレドの政策はアンデスの生活様式を全面的に否定し,それをヨーロッパのもので置き換える根本的改革とみなされてきた。しかし近年,歴史学,人類学,考古学の分野において従来の見解の見直しが進んでいる。植民地事業が内包する矛盾や両義性,支配/抵抗という二項対立に還元できない植民者と被植民者の錯綜した交渉,先住民による再解釈や選択的受容など,従来見落とされてきた側面の解明が進み,集住化のイメージが刷新されつつある。この動向は,コロニアル/ポスト・コロニアル研究全体の動向とも連動している。・ジェレミー・ラヴィ・マンフォード『垂直の帝国―植民地期アンデスにおける先住民の総集住化』デューク大学出版会,2012年。・スティーヴン・A・ウィンキー『交渉される居住地―インカとスペインの植民地統治下におけるアンデスの共同体と景観』フロリダ大学出版会,2013年。・マリナ・スロアガ・ラダ『交渉される征服―1532年から1610年までのペルーのワイラスにおけるワランガ,地方権力,帝国』ペルー問題研究所/フランス・アンデス研究所,2012年。

T-1095, a derivative of phlorizin, is an orally active inhibitor of Na+ -glucose cotransporter (SGLT). We investigated the acute antihyperglycemic effect of T-1095 in streptozotocin-induced diabetic rats (STZ rats). T-1095 and its metabolite T-1095A inhibited the SGLT activity in brush border membranes prepared from kidneys of both normal and STZ rats, but the latter agent was approximately 10 times more potent than the former. Single oral administration of T-1095 (30-100 mg/kg) dose-dependently induced glycosuria in normal rats. The fed glucose levels in STZ rats were dose-dependently suppressed by single oral administration of T-1095 (3-100 mg/kg), whereas there was only marginal hypoglycemic effect in normal rats. Since there was no effect on blood glucose in nephrectomized STZ rats, inhibition of renal glucose reabsorption rather than intestinal glucose absorption mainly contributes to the antihyperglycemic effect of T-1095. In conclusion, T-1095 is the first orally active agent which has an acute antihyperglycemic action in the absence of endogeneous insulin secretion with a low risk of hypoglycemia and has therapeutic potential for treatment of diabetes mellitus.
