[Purpose] In order to develop a more effective treatment method for pain and function in myofascial pain syndrome, we examined the effects of ESWT, stability exercises, and combined treatment. [Subjects] The subjects were randomly divided into a stabilization exercise group (n=12), an ESWT (Extracorporeal Shock Wave Therapy) group (n=12), and a combined treatment group (n=12). [Methods] The stabilization exercise group performed shoulder joint stabilization exercises. The ESWT group received ESWT for the upper trapezius. The combined treatment group received a combined treatment of shoulder joint stabilization exercises and ESWT. Pain and function were measured using the visual analog scale (VAS), pressure pain threshold (PPT), neck disability index (NDI), and the Constant Murley Scale (CMS). [Results] The VAS Score showed statistically significant improvements in all of the groups. All of the CMS evaluation items except muscle strength in the stabilization exercise group, and all of the CMS items in the ESWT group and the combined treatment group, exhibited statistically significant improvements. The combined treatment group of ESWT and stabilization exercises showed statistically significant improvements in all VAS of CMS evaluation items, and the NDI test after the four-week intervention. [Conclusion] The combined treatment was more effective at reducing pain than ESWT, and stabilization exercise would be useful for physical therapists treating myofascial pain syndrome in a clinical setting.

[Purpose] The purpose of this preliminary study was to investigate whether young players with no history of injury, have developed early asymmetries in dynamic balance ability tested via the recommended for screening in sports, Modified Star Excursion Balance Test (MSEBT). [Participants and Methods] Twenty-four young healthy male soccer players participated in the study having at least 4 years of systematic soccer training. The Waterloo Footedness Questionnaire was used to discriminate the stability dominant leg (STAB) from the non-stability dominant leg (NSTAB). Dynamic balance was assessed via the MSEBT. Participants, after familiarization, made 3 attempts in each direction for both legs: a) Anterior (AN), b) Posterolateral (PL) and c) Posteromedial (PM). [Results] The sole statistically significant performance asymmetry was in the PL direction, in favor of the STAB (94.5 ± 13.3 cm vs. 98.1 ± 10.4 cm). [Conclusion] The results of this pilot study showed a potential for developing dynamic balance asymmetries, in soccer players at the age of 13–14 years. Since asymmetry was significant in only one direction, further long term monitoring would be helpful to evaluate whether this is a growing functional deficit, potentially involving any of the other two directions of testing or if it is alleviated with increasing training age. These asymmetries could comprise an injury risk factor.

[Purpose] To demonstrate immediate alteration in lumbar lordosis and the lumbar angle in each segment after the application of the mechanical pressure technique to the psoas major muscle (PM). [Participants and Methods] In all, 34 participants were assigned to either the PM pressure technique group (n=17) or control group (n=17). Three dimensional (3D) coordinates of the 12th thoracic spinous process and lumbar spinous processes were measured with a 3D digitizer in the prone position with 15° bilateral hip extension to compare the changes in lumbar lordosis and the lumbar extension angle in each segment in both the PM pressure technique group and control group. [Results] Mann-Whitney’s U test revealed no significant differences in lumbar lordosis in either group. However, the lumbar extension angle at L4 decreased significantly after the PM pressure technique compared with that before the pressure technique. Additionally, the lumbar extension angle at L4 also decreased significantly after the PM pressure technique compared with the control group. Conversely, lumbar extension angle at L1 increased significantly after the PM pressure technique compared with that before. There was no significant difference in the lumbar extension angle at L2, L3 and L5 after the PM pressure technique. [Conclusion] This study suggests that the PM pressure technique possibly attenuates PM stiffness while reducing lumbar extension in each segment.

[Purpose] The hybrid assistive limb was developed to improve the kinematics and muscle activity in patients with neurological and orthopedic conditions. The purpose of the present study was to examine the long-term sustained effect of gait training using a hybrid assistive limb on gait stability, kinematics, and muscle activity by preventing knee collapse in a patient with cerebral palsy. [Participant and Methods] A 17 year-old male with cerebral palsy performed gait training with a hybrid assistive limb 12 times in 4 weeks. After completion of 12 sessions of hybrid assistive limb training, monthly follow-up was conducted for 8 months. The improvement was assessed on the basis of joint angle and muscle activity during gait. [Results] The degree of knee collapse observed at baseline was improved at 8-month follow-up. Regarding muscle activity, electromyography revealed increased activation of the vastus lateralis at 8-month follow-up. Moreover, the hip and knee angles were expanded during gait. In particular, the knee extension angle at heel contact was increased at 8 months after follow-up. [Conclusion] Gait training with a hybrid assistive limb provided improvement of gait stability such as kinematics and muscle activity in a patient with cerebral palsy. The improved gait stability through prevention of knee collapse achieved with hybrid assistive limb training sustained for 8 months.

[Purpose] To present the case of a total reduction of pseudoscoliosis spinal deformity in an adult female suffering from recurrent back pains. [Participant and Methods] A 29 year old female suffering from recurrent back pains was diagnosed with lateral thoracic translation posture; aka pseudoscoliosis. The patient was initially given 12 treatments of relief care including spinal manipulative therapy, then another 24 treatments receiving the same plus mirror image® translation traction and exercises. [Results] The patient achieved a complete reduction of the lateral thoracic translation posture (pseudoscoliosis) as indicated on a post-treatment radiograph after 36 total treatments. Most orthopedic tests became normalized and the patients back pains were significantly improved after the correction of posture, but only slight improvements after the initial 12 sessions of manipulative therapy only. [Conclusion] Pseudoscoliosis is structurally reducible by use of CBP® mirror image® lateral translation traction methods and exercises and led to the resolution of back pains in this case. The diagnosis of pseudoscoliosis as opposed to true scoliosis is very important and likely underdiagnosed in common practice. Upright radiographic imaging is essential to differentiate these two spinal disorders and offers no harm to the patient. Comprehensive assessment including routine use of x-ray is recommended to differentiate between spinal disorders.

[Purpose] The towel gathering exercise is commonly chosen in the treatment of painful accessory navicular. However, the effect has not been well documented in clinical settings. So we compared the reduction in pain effect of towel gathering exercise with those of three toe flexor exercises we developed. [Subjects and Methods] Fifteen accessory navicular cases (4 males, 11 females) were chosen as subjects. Their mean age was 20.3±10.7 years old. The three exercises were almost maximal isometric contraction of first toe flexor, 2nd-to-5th toe flexors and 3rd-to-5th toe flexors. All subjects randomly performed the three toe exercises and the towel gathering exercise. Before and after each exercise we assessed the pain by provocative tests such as stepping and others using a visual analog scale (100 mm). [Results] The exercises of 2nd-to-5th toe flexors and 3rd-to-5th toe flexors showed overtly significant improvement of pain. On the other hand, the effects of both the towel gathering and first toe exercises were less significant. Especially, the towel gathering exercise showed the poorest effect of pain relief.

[Purpose] This study compared the effects of a short-term conservative physiotherapy program versus those of occlusive splinting on pain and range of motion in cases of Temporomandibular Joint Dysfunction. [Participants and Methods] This study included 112 male and female participants with ages ranging from 15–27 years. Outcome measures were pain assessed by the visual analogue scale and Temporomandibular Joint Range of Motion measured with the Temporomandibular joint opening index. Patients were randomly assigned to one of two groups. Conservative physiotherapy was provided to one group 3 days weekly while the other group received standard occlusive splinting. Splinting was used daily for the total period of treatment. Any adjustments for splints were done by the treating dentist. Both groups were treated for a total period of 6 weeks. [Results] Between group statistical analysis revealed a significant reduction in pain intensity and Temporomandibular joint opening index in favor of the conservative physiotherapy group. [Conclusion] Over a treatment period of 6 consecutive weeks, conservative physiotherapy could be a better initial treatment than occlusive splint in relieving pain and improving range of motion in cases of myogenic temporomandibular dysfunction.

[Purpose] The purpose of this study was to investigate the effect of changing the plantar flexion resistance of an ankle-foot orthosis on knee joint reaction and knee muscle forces. Furthermore, the influence of an ankle-foot orthosis with an over-plantar flexion resistance function on knee joint reaction force was verified. [Participants and Methods] Ten healthy adult males walked under the following three conditions: (1) no ankle-foot orthosis, and with ankle-foot orthoses with (2) a strong and (3) a weak plantar flexion resistance (ankle-foot orthosis conditions). The knee flexion angle, quadricep muscle force, hamstring muscle force, and knee joint reaction force during the stance phase were measured using a motion analysis system, musculoskeletal model, and ankle-foot orthosis model. [Results] The peak knee joint reaction force, knee flexion angle, and quadricep muscle force in the early stance phase significantly increased in the strong plantar flexion resistance condition in comparison with the “no ankle-foot orthosis” condition. [Conclusion] Increased knee joint reaction force with over-plantar flexion resistance suggests that over-plantar flexion resistance causes various knee problems such as knee pain and knee osteoarthritis.

[Purpose] This study developed a reciprocal hip exercise using Thera-Band for those with flat lumbar spines and observed changes in standing and movement angles during sit-to-stand. [Participants and Methods] Eleven females participated in this study. The participants performed the reciprocal hip exercise using Thera-Band. [Results] The trunk flexion angle increased significantly after reciprocal hip exercise. The sit-to-stand time after reciprocal hip exercise was significantly faster than that before exercise. [Conclusion] The reciprocal hip exercise would useful for patients with flat back who can then sit and work for long periods.

[Purpose] It is difficult for amputees to perform conventional cardiopulmonary exercise testing. Values were determined for two-legged, one-legged, and two-armed exercise testing in healthy adult males (Study 1), for comparison with preliminary measurements of endurance in amputee football players (Study 2). [Participants and Methods] In Study 1, cardiopulmonary exercise testing was performed in healthy adult males. Correlations between oxygen uptake in two-legged and one-legged/two-armed exercise were calculated and a comparison was made between one-legged exercise and two-armed exercise for each measured value. In Study 2, cardiopulmonary exercise testing was performed on male amputee football players using a two-arm-driven ergometer. The measured values obtained for healthy adult males and amputee football players were compared. [Results] In Study 1, peak work rate and peak heart rate values of healthy participants were significantly higher in two-armed exercise than in one-legged exercise. The correlation between peak oxygen uptake values for two-legged and one-legged exercise was decreased. In Study 2, peak work rate of two-armed exercise was significantly higher in amputee football players than in healthy participants. [Conclusion] Study 1 suggested that musculoskeletal factors might have greater significance for one-legged exercise than for two-armed exercise. Study 2 suggested that para-sports, including amputee football, may contribute to physical strength and health maintenance in lower leg amputees.

[Purpose] The purpose of this study was to learn about the effects of a home-oriented program based on sensorimotor activities on executive and motor functions in children with ADHD. [Participants and Methods] In this study, a home-based sensorimotor program was conducted for two male children diagnosed with ADHD. The participants performed the program with their parents four times a week, for forty minutes each time, over a 12-week period. The participants’executive functions and gross motor skills were evaluated using the Children’s Color Trails Test (CCTT) and the Bruininks-Oseretsky Test of Motor Proficiency-2 (BOT-2). [Results] After participating in the home-based sensorimotor program, the participants showed improvements in both their executive and motor functions. [Conclusion] This study confirmed that a home-based sensorimotor program was effective in improving executive and motor functions in children with ADHD. This conclusion suggests the need to develop various exercise programs that can relieve the symptoms of ADHD in children and improve their abilities to adapt at home or school.

[Purpose] This study aimed to determine the safety and immediate effect of a single training session with the Hybrid Assistive Limb (CYBERDYNE) on walking ability in patients with cerebral palsy. [Participants and Methods] This study included 20 patients with cerebral palsy (15 males, 5 females, mean age 15.0 ± 6.3 years; 19 with spastic cerebral palsy, 1 with athetoid cerebral palsy; Gross Motor Function Classification System level I: 4, II: 3, III: 9, and IV: 4). Participants completed a single 20-minute gait training session using the Hybrid Assistive Limb. The safety and immediate effect were evaluated. The immediate outcomes were gait speed and mean step length, and cadence before and after training. [Results] Two participants were excluded because they were not tall enough to use the Hybrid Assistive Limb. Eighteen participants performed the training. There were no serious adverse events during the training. Since 14 participants were able to walk on their own, walking evaluations were performed before and after training. Statistically significant improvements were observed in gait speed and mean step length. [Conclusion] Gait training using the Hybrid Assistive Limb is safe for patients with cerebral palsy and can produce immediate effects on walking ability in ambulatory patients with cerebral palsy.

[Purpose] To investigate the effects of core strength training on core stability with and without the Valsalva maneuver. [Participants and Methods] Twenty-four students were randomly assigned to the training and control groups. Students in the training group undertook a 4-week training program that included exercises for the transverse abdominis, multifidus, diaphragm, and pelvic floor muscles, whereas students in the control group performed their usual activities. Participants were required to perform four types of task with and without the Valsalva maneuver. Seated stabilometry was assessed according to the center of pressure (COP). [Results] In the training group, the rectus area in the quiet sitting position with the Valsalva maneuver was enlarged and the length of trajectory during a sudden perturbation task was decreased. No significant changes to the COP were seen in the control group. [Conclusion] Some parameters of core stability improved after participants completed a 4-week core strength training program.

[Purpose] The purpose of this study is to investigate the effects of disability awareness educational program of university students in the department of physical therapy on reducing prejudice against people with disabilities and increasing positive attitudes toward people with disabilities. [Participants and Methods] Students who participated in the disability awareness educational program were selected as experiment group and 15 students who did not take program were selected as control group. [Results] First, in the comparisons between the control and experimental group, there was no difference in the overall prejudice on people with disabilities and positive attitudes toward people with disabilities before the disability awareness educational program. However, there was a significant difference in the overall prejudice and positive attitudes between two groups after the disability awareness educational program. Second, in the comparisons between pretest and posttest, there was no significant difference in the overall prejudice on people with disabilities in control group. However, in the comparisons between pretest and posttest, there was a significant difference in the overall prejudice in experimental group. [Conclusion] It is necessary to provide disability awareness educational program for the university students in the department of physical therapy to reduce the overall prejudices against people with disabilities and increase positive toward people with disabilities.

[Purpose] The association between foot injuries and foot alignment, including the transverse arch height (TAH) and asymmetry, was examined in athletes participating in college track events. [Participants and Methods] This study included 55 male athletes participating in a college track and field club. Data including demographic information and the incidence of foot injuries within a year prior to participation in this study were obtained via questionnaires. TAH and the medial longitudinal arch height during 10 and 90% loading, leg-heel alignment, and the heel angle were measured before calculating the asymmetry of each alignment parameter measured. Participants were categorized into an injury or a normal group. Unpaired t-tests were used to perform between-group comparisons for each alignment parameter measured and asymmetry. Additionally, logistic regression analysis was performed to identify factors associated with foot injuries after adjustment for demographic data. [Results] TAH asymmetry during 10 and 90% loading was significantly greater in the injury group. Further logistic regression analysis performed showed that only TAH asymmetry during 90% loading was significantly associated with foot injuries after adjustment for demographic data. [Conclusion] With regard to track events, a greater asymmetry of forefoot TAH in a weight-bearing position was observed to be associated with foot injuries.

[Purpose] This study aimed to conduct experiments to examine the effects of wrist eccentric control exercise or shoulder stabilization exercises after a basic direct treatment of the elbow in the treatment of tennis elbow patients in terms of pain and grip strength. [Subjects and Methods] The subjects were divided into two groups: one group conducted wrist eccentric control exercise and was comprised of 5 male and 4 female subjects, and the other group received shoulder stabilization exercise and was comprised of 5 male and 4 female subjects. [Results] In the intragroup comparison, both groups showed a significant decrease in pain level and a significant increase in the measurement of the tenderness thresholds of the upper trapezius muscle, lateral epicondyle, and grip strength. In the intergroup comparison, the shoulder stabilization exercise group showed a significantly greater increase in the measurement of the tenderness thresholds of the upper trapezius muscle and grip strength, and the differences were not significant in the pain level and tenderness threshold of the lateral epicondyle. [Conclusion] Wrist eccentric control exercise and shoulder stabilization exercises can be useful as intervention methods for relief from pain due to lateral epicondylitis and for the improvement of functions impaired by tennis elbow.

[Purpose] The purpose of this study was to assess the effects of home-based knee stretching exercises on knee range of motion (ROM) and gait speed in patients with knee osteoarthritis awaiting total knee arthroplasty. [Subjects] Thirty-six patients with severe knee osteoarthritis were randomly allocated to stretching (n=17) and control (n=19) groups. [Method] The subjects in the stretching group were instructed to perform home-based knee stretching exercises once a day for about 80 days, whereas the subjects in the control group were told to maintain their current level of physical activity. Outcomes assessed percentage changes in the total range of knee ROM in the supine position (S-ROM), pain, gait speed and knee ROM during gait (G-ROM). [Results] The stretching group showed significantly greater improvement in S-ROM, gait speed and G-ROM than the control group (control vs stretching; S-ROM, 0.4 ± 8.6% vs 9.5 ± 16.2%; gait speed, 1.6 ± 11.4% vs 11.6 ± 10.7%; G-ROM, 0.6 ± 15.2% vs 14.2 ± 14.6%; p<0.05 for all). Pain was significantly decreased in the stretching group compared to the control group (median values: -15.6% and 6.5%, p<0.01).

[Purpose] Patients diagnosed with diabetes mellitus (DM) are known to experience sensory disturbances in their feet secondary to diabetic neuropathy (DN). The sole of the foot tactile pressure sensitivity threshold in patients with DM was evaluated in addition to examining age-related effects based on the sites studied. Whether the tactile pressure sensitivity threshold in the sole of the foot in elderly patients with DM was associated with the duration of DM or aging was evaluated. [Participants and Methods] Our study included 204 patients with DM (136males and 68 females). Patients were divided into the following age groups: 20–39 (20 patients), 40–49 (26 patients), 50–59 (34 patients), 60–69 (60 patients), 70–79 (45 patients), and 80–89 years (19 patients). Using Semmes-Weinstein monofilament testing, the tactile pressure sensitivity threshold at the sole of the foot was evaluated in the bilateral halluces, first metatarsal head, fifth toe metatarsal head, and the heels, and age-related effects were examined. [Results] At all the sites that were examined, we observed an age-related increase in the tactile pressure sensitivity threshold value, and this finding demonstrated a positive partial correlation with the duration of DM. [Conclusion] Aging influences DN-induced degeneration of peripheral nerves.
