[Purpose] To determine the effects of forward head posture on static and dynamic balance control. [Subjects and Methods] This study included 30 participants who were included into a forward head posture group (n = 14) and a control group (n = 16) according to their craniovertebral angles. Static balance control was assessed according to center of gravity sway velocity and total sway distance using an automatic balance calibration system. Dynamic balance control was assessed using the diagnosis mode of a body-tilt training and measurement system. [Results] Sway velocities on a hard surface with eyes open and closed and those on an unstable sponge surface with eyes closed were significantly higher in the forward head posture group than in the control group. Furthermore, on both the hard and sponge surfaces in the eyes open and closed conditions, total sway distances were significantly higher in the forward head posture group than in the control group. Results of dynamic balance control were not significantly different between groups. [Conclusion] Forward head posture has a greater effect on static balance control than on dynamic balance control.

痛みの評価(測定法と検査法)に関する研究報告に基づき,特に理学療法分野で応用可能であるものを紹介した。痛みを評価する目的は,患者の訴えを客観的に把握すること,痛みを引き起こしている原因を探察すること,そして痛覚機能の障害の程度を測定する(痛覚閾値の測定)ことの3つである。訴えを把握するには,痛みの強さ,性質,時間的変化,領域における4次元的に捉えておく必要がある。痛みの原因を探索するには,圧痛点の探索,姿勢の矯正,関節運動による痛みの誘発などの方法が用いられる。また,痛覚閾値の測定法には,機械的刺激法,温度刺激法,化学刺激法,電気刺激法があることを論じた。

[Purpose] To determine the effects of hydrotherapy and land-based exercises on functional mobility and quality of life among patients with knee osteoarthritis. [Participants and Methods] We conducted a randomized controlled trial with knee osteoarthritis patients randomly allocated into land-based (n=17) and hydrotherapy groups (n=17). The Time-Up and Go (TUG), Five Times Sit-to-Stand (5STS), Stair Climbing Test (SCT), and Quality of Life by questionnaires including the Modified Western Ontario and McMaster Universities Osteoarthritis Index questionnaire Thai version (Thai WOMAC) were assessed at baseline and 6 weeks. The World Health Organization Quality of Life BREF Thai version (WHOQOL-BREF-THAI) questionnaire were assessed at baseline and six weeks and 6 months. [Results] There was no significant difference in outcomes between the groups after 6-weeks and 6-months of follow-up. After 6 weeks, Thai WOMAC score improved in both groups. Only 5STS was improved in the land-based group, while the hydrotherapy group showed significant TUG, 5STS, and SCT improvement. Furthermore, only hydrotherapy showed significant improvement in WHOQOL-BREF-THAI scores in the mental, social, quality of health, and total domains after six months. [Conclusion] Both exercises equally improved functional mobility and quality of life. Hydrotherapy and land-based exercise could improve functional mobility and quality of life in patients with knee osteoarthritis.

[Purpose] The effectiveness of intrinsic foot flexor strength training performed in the plantar flexion position was examined using needle electromyography. [Subjects] The subjects of this study were 18 healthy men. [Methods] We used needle electromyography to measure the muscle activities of the flexor hallucis brevis (FHB), and the flexor digitorum brevis (FDB) in maximum plantar and an intermediate position. [Results] Significant increases in muscle activities were observed for both FHB and FDB, and the rates of increase from the intermediate position to the plantar flexion position were 43% for FHB and 46% for FDB. [Conclusion] This study demonstrated that it is possible to evaluate intrinsic foot flexors, in addition to the numerous reports on treatment methods focusing on extrinsic foot flexors. Furthermore, the results suggest that toe flexion exercises performed during plantar flexion of the ankle joint are an effective method for intrinsic foot flexor strength training.

[Purpose] This study aimed to evaluate the effectiveness of pre-operative physical rehabilitation on the postoperative course of the patients with gastrointestinal cancer undergoing surgery. [Participants and Methods] A rehabilitation physician examined and educated 33 patients (42% of whom were male with a mean age of 65.2 ± 10.9 years) who were scheduled to undergo elective surgery for gastrointestinal cancer. They received instructions for performing exercise from a physical therapist 17.0 ± 7.3 days prior to surgery. We divided the participants into three groups (improvement, maintenance, and deterioration) based on the changes in their ability to walk prior to surgery. This study compared the results of the 6-min walk test, hospital anxiety and depression scale, and 36-Item Short-Form Health Survey version 2 for the three groups at baseline, following rehabilitation prior to surgery, and 4 weeks following surgery. [Results] In the improvement group, the decrease in the ability to walk between the baseline and 4 weeks after surgery was not significant. Conversely, the deterioration group exhibited a significant decrease in the ability to walk 4 weeks after surgery. [Conclusion] Improvement in walking ability by rehabilitation training prior to surgery leads to the preservation of physical function in the patients with gastrointestinal cancer undergoing surgery.

[Purpose] Smoking has a direct effect on the respiratory system. The rate of cigarette smoking among young people has continued to increase steadily. The present study quantified and compared the respiratory function of smoking and non-smoking youths. [Subjects] Smoking and non-smoking male participants aged between 15 to 18 years were recruited (n=34 per group). [Methods] Participants were asked to complete a questionnaire relating to smoking habits and the Fagerström test for nicotine dependence questionnaire, and their respiratory function was tested (measurement of chest expansion, lung function test with a spirometer, and assessment of respiratory muscle strength). [Results] All respiratory function tests demonstrated significant differences between the smoking and non-smoking groups. Smokers initiated cigarette smoking between the ages of 15 to 18 years. The most common duration of cigarette smoking was 1-3 years and the degree of nicotine dependence among the youths was at a low level. [Conclusion] This study’s findings show that the early effects of cigarette smoking found in youths can lead to problems with the respiratory system. Such information can be used to illustrate the harm of smoking and should be used to encourage young people to quit or avoid cigarette smoking.

[Purpose] The respiratory function in patients with cervical spinal cord injury is influenced by inspiratory intercostal muscle function. However, inspiratory intercostal muscle activity has not been conclusively evaluated. We evaluated the inspiratory intercostal muscle activity in patients with cervical spinal cord injury by using inspiratory intercostal electromyography, respiratory inductance plethysmography, and ultrasonography. [Participants and Methods] Three patients with cervical spinal cord injury were assessed. The change in mean amplitude (rest vs. maximum inspiration) was calculated by using intercostal muscle electromyography. Changes in intercostal muscle thickness (resting expiration and maximum inspiration) were also evaluated on ultrasonography. The waveform was converted to spirometry ventilation with respiratory inductance plethysmography, and the waveform at the xiphoid was considered to determine the rib cage volume. Each index was compared with the inspiratory capacities in each case. [Results] Intercostal muscle electromyography failed to measure the notable myoelectric potential in all the patients. The rib cage volume was higher at higher inspiratory capacities. The changes in muscle thickness were not significantly different between the patients. [Conclusion] The rib cage volume (measured with inductance plethysmography) was greater in the patients with cervical spinal cord injury when inspiratory intercostal muscle activity was high. Respiratory inductance plethysmography can capture inspiratory intercostal muscle function in patients with cervical spinal cord injury.

[Purpose] To determine the potential factors for difference in metabolic profiles between metabolically healthy obesity and metabolically unhealthy obesity, we investigated the difference in abdominal fat volume, metabolic characteristics, and physical activity levels between metabolically healthy obesity and metabolically unhealthy obesity identified with cardiovascular disease risk factors in Japanese males. [Participants and Methods] Of 305 volunteers recruited, 130 obese males (age: 46.9 ± 8.9 years; body mass index: 29.6 ± 3.5 kg/m2) met the criteria for the study. They were divided into two groups; metabolically healthy obesity and metabolically unhealthy obesity according to cardiovascular disease risk factors including low-density lipoprotein cholesterol. Abdominal fat volumes were measured using magnetic resonance imaging. Cardiovascular disease risk factors and metabolic characteristics were evaluated by blood pressure and blood parameters. Physical activity levels were measured using an accelerometer. [Results] Despite the fact that metabolically healthy obesity had a more favorable metabolic profile than the metabolically unhealthy obesity, no significant differences in visceral and subcutaneous fat volumes were found between the two groups. Moreover, the metabolically healthy obesity had a significantly greater physical activity expenditure and moderate-to-vigorous physical activity level than the metabolically unhealthy obesity. [Conclusion] A more favorable metabolic profile in metabolically healthy obesity may be associated with physical activity level rather than abdominal fat volumes in Japanese males.

[Purpose] The purpose of this study was to examine the effect of kinetic chain exercise using EMG-biofeedback on balance and lower extremity muscle activation. [Subjects and Methods] For this study, 30 stroke patients participated in this study and they were divided into closed kinetic chain exercise using EMG-biofeedback group (CKCE+EB) and open kinetic chain exercise using EMG-biofeedback group (OKCE+EB), each group consisting of 15 patients. The kinetic chain exercise using EMG-biofeedback was performed by the patients for 20 minutes once a day, 5 days a week, for 6 weeks using an Myo-Ex. BioRescue was used to measure balance ability, while surface EMG was used to measure the lower extremity muscle activation. [Results] According to the results of the comparison within the groups, after the intervention, both groups showed significant increases in the balance ability and lower extremity muscle activation. In the comparisons between the groups, after the intervention, balance ability and lower extremity muscle activation were significantly higher in the CKCE+BE than in the OKCE+EB. [Conclusion] This study showed that closed kinetic chain exercise using EMG-biofeedback is effective for improving balance ability and lower extremity muscle activation in stroke patients.

[Purpose] The purpose of this study was to determine the concurrent validity of Wii Fit center-of-pressure parameters with Clinical Test of Sensory Integration for Balance and to evaluate the discriminant validity of the Wii Fit center-of-pressure parameters for adults and the elderly for the assessment of postural sway. [Participants and Methods] This study used an observational cross-sectional correlational design. All 70 participants were required to complete a questionnaire for eligibility screening, followed by a center-of-pressure assessment using three Wii Fit balance activities (snowboard slalom, ski slalom, and balance bubble) and Clinical Test of Sensory Integration for Balance using a force platform. [Results] For center-of-pressure assessment, our results showed there was concurrent validity of Wii Fit ski slalom and balance bubble with Clinical Test of Sensory Integration for Balance conditions 6 and 4, respectively. Our results also demonstrated that the three Wii Fit balance activities selected in this study could be used to discriminate between adults and the elderly. [Conclusion] Our results suggest that Wii Fit is a viable and affordable alternative method for center-of-pressure assessment.

[Purpose] The present study examined the effects of treatment using PNF extension techniques on the pain, pressure pain, and neck and shoulder functions of the upper trapezius muscles of myofascial pain syndrome (MPS) patients. [Subjects] Thirty-two patients with MPS in the upper trapezius muscle were divided into two groups: a PNF group (n=16), and a control group (n=16) [Methods] The PNF group received upper trapezius muscle relaxation therapy and shoulder joint stabilizing exercises. Subjects in the control group received only the general physical therapies for the upper trapezius muscles. Subjects were measured for pain on a visual analog scale (VAS), pressure pain threshold (PPT), the neck disability index (NDI), and the Constant-Murley scale (CMS). [Results] None of the VAS, PPT, and NDI results showed significant differences between the groups, while performing postures, internal rotation, and external rotation among the CMS items showed significant differences between the groups. [Conclusion] Exercise programs that apply PNF techniques can be said to be effective at improving the function of MPS patients.

健常人の反応時間に対する学習効果を筋電計とパーソナルコンピューターを用いて検索した。刺激パターンは学習前後で同一とした。また、対象は男性9名、女性11名の20名とし、平均年齢は26.8歳であった。測定回数を重ねるにしたがって、反応時間も短縮される傾向にあった事から、測定の中間に反応時間の変動要因を置き、反応時間の短縮が認められても、その全てを促通効果と見なすには一考を要すると思われた。

[Purpose] The present study aimed to determine the effects of eccentric calf raise exercise, which has the characteristics of plyometric training, on the fascicle length and muscle thickness of the gastrocnemius medialis muscle and range of motion of the ankle using ultrasonography. [Participants and Methods] Twenty-one healthy volunteers were randomly assigned to the eccentric calf raise exercise group or normal calf raise exercise group. Measurements were performed before training and at 3, 6, 9, and 12 weeks after training. [Results] In the eccentric calf raise exercise group, the fascicle length significantly increased after 6 weeks compared to that at baseline and at 3 weeks after training. The dorsiflexion angle and muscle thickness after three weeks significantly increased compared to that at baseline, but the pennation angle was not significantly different. The fascicle length, pennation angle, dorsiflexion angle, and muscle thickness showed no significant difference at all time points in the NCR group. [Conclusion] The results of this study showed that continued stretching of the gastrocnemius medialis muscle during eccentric calf raise exercise enhanced the morphological structures, such as the a fascicle length and muscle thickness. Eccentric calf raise exercise training may aid in injury prevention.

[Purpose] It is important to facilitate intestinal motility in patients with reduced bowel movement through physiotherapy. The purpose of the present study was to compare the effects of passive exercise of the lower limbs and trunk (PELT) and combination therapies (COM) with those of conventional thermotherapy (TT) on bowel sounds (BSs) in healthy adult subjects. Since autonomic activity (AA) significantly influences intestinal motility, we also investigated the relation between intestinal motility and AA by measurement of BSs. [Subjects] The subjects were 16 healthy adult males. [Methods] The subjects were randomly assigned to 3 different physiotherapies, and BSs and sympathetic nerve activity were measured before and after the physiotherapies. [Results] While BSs significantly increased following all physiotherapies, the temporal changes in BSs were different among the physiotherapies. AA measurement showed that PELT and TT significantly decreased the heart rate. While the high-frequency (HF) component was increased in all physiotherapy groups, the increases in HF did not reach statistical significance. There were no significant correlations between BSs and AA. [Conclusion] We found that all of the tested physiotherapies increased BSs, suggesting that they are clinically useful for treatment of patients with reduced intestinal motility due to limited spontaneous movement or inability to rise up from bed.

階段昇降時の動作特性を明らかにする事を目的として,F-SCANを用い足圧中心の移動軌跡を健常人・患者25名に対して測定し,以下の結果を得た。健常人の階段昇降動作での足圧中心の移動軌跡は,前足部から踵方向へ移動後,再び前方へ移動する戻り型(Aタイプ)と,平地歩行と類似したの前方向型(Bタイプ)に大別された。健常人の降段動作での足圧中心の軌跡は全てAタイプで,昇段に比較して左右への変位を認めた(p<0.01)。一方,昇段ではA・Bの両タイプがみられた。患者では,降段動作の障害がより顕著で,機能改善とともに健常人の足圧中心の軌跡波形に近付く傾向が観察された。また,杖は立脚期を安定化させる役割があることが客観的に示された。階段昇降時の動的足圧を測定する事により,降段動作の姿勢制御の複雑さの一端が明らかとなった。

The purpose of this study was to clarify the postural control of the initiation of lateral step and step-up motions in healthy young adults (24.3 ± 1.8 years: mean ± SD). The tasks involved the lateral step (step lengths, 10 cm and 20 cm) and lateral step-up motions (10-cm high stool; step lengths, 10 cm and 20 cm). The variables for analysis included motion duration (weight-shift phase and swing phase), shifts of the center of pressure (CoP) and the center of gravity (CoG), displacement and inclination of the shoulder and the pelvis, and root mean square electromyographic amplitude of the erector spinae, gluteus medius (GM) and adductor longus (AL) bilaterally. The CoP shift toward the stepping side was larger in the step-up task than in the step task, and it was smaller when both tasks were performed in the long lengths. The CoG shift toward the supporting side and the displacements of the shoulder and pelvis were larger in the step-up task than in the step task. However, the magnitude of GM activity of the supporting leg was larger in the step task than in the step-up task, and increased when both tasks were performed in the long lengths. In the stepping leg, the magnitude of AL activity was larger in the step-up task than in the step task, and decreased when both tasks were performed in the long lengths. These results suggest that GM activity of the supporting leg and AL activity of the stepping leg control the shift of CoG in the frontal plane. Our results indicate that the increase in GM activity does not depend on the height of stepping but on the length of stepping in lateral step and step-up motions.

[Purpose] Understanding the essential mechanisms in post-stroke recovery not only provides important basic insights into brain function and plasticity but can also guide the development of new therapeutic approaches for stroke patients. This review aims to give an overview of how various variables of Magnetic Resonance-Diffusion Tensor Imaging (MR-DTI) metrics of fractional anisotropy (FA) can be used as a reliable quantitative measurement and indicator of corticospinal tract (CST) changes, particularly in relation to functional motor outcome correlation with a Fugl-Meyer assessment in stroke rehabilitation. [Methods] PubMed electronic database was searched for the relevant literature, using key words of diffusion tensor imaging (dti), corticospinal tract, and stroke. [Results] We reviewed the role of FA in monitoring CST remodeling and its role of predicting motor recovery after stroke. We also discussed the mechanism of CST remodeling and its modulation from the value of FA and FMA-UE. [Conclusion] Heterogeneity of post-stroke brain disorganization and motor impairment is a recognized challenge in the development of accurate indicators of CST integrity. DTI-based FA measurements offer a reliable and evidence-based indicator for CST integrity that would aid in predicting motor recovery within the context of stroke rehabilitation.

[Purpose] The purpose of this study was to examine the effect of a dance music jump rope exercise on changes Pulmonary Function and body mass index in female overweight subjects in their 20’s. [Subjects and Methods] The subjects were randomly assigned to the dance music jump rope exercise group and the stationary cycle exercise group. All subjects have conducted the exercises three times a week for four weeks. Pulmonary function was evaluated using a spirometer, and body mass index was evaluated using an InBody 3.0. [Results] The findings of this study showed significant improvements in the voluntary capacity and body mass index of the experimental groups. Vital capacity was higher in the music jump rope exercise group than the stationary cycle exercise group, and body mass index was lower in the music jump rope exercise group than the stationary cycle exercise group. [Conclusion] This study showed that the dance music jump rope exercise can be used to improve vital capacity and body mass index.

[Purpose] The incidence of chronic obstructive pulmonary disease is rapidly increasing worldwide. In Nepal, it has the highest mortality rate among all noninfectious diseases. Since 2015, we have been involved in a project that aims to facilitate chest rehabilitation for patients with chronic obstructive pulmonary disease in Nepal. We compared the Nepali version of the St. George’s Respiratory Questionnaire with the Chronic Obstructive Pulmonary Disease Assessment Test, the latter of which was translated into Nepali for this project. We also evaluated the extent to which patient quality of life improved after the rehabilitation program. [Participants and Methods] The Nepali St. George’s Respiratory Questionnaire and Chronic Obstructive Pulmonary Disease Assessment Test were used to assess the health status of patients both before the intervention’s initiation and one year after it. Between May and September of 2016, 122 patients with chronic obstructive pulmonary disease participated in this program. [Results] We collected valid responses from 57 patients both before and after the intervention. The scores of both screening tools were significantly lower after the intervention than before and showed a significant correlation with one another. [Conclusion] These results suggest that the Nepali version of the Chronic Obstructive Pulmonary Disease Assessment Test is a reliable tool for the evaluation of chronic obstructive pulmonary disease and that the intervention used in the project might be effective for patients afflicted with the disease. However, there are limitations to the research design, such as the limited number of participants used in the study.
