[Purpose] The effect of physical activity on systemic inflammation remains unclear and might be negative in patients with chronic obstructive pulmonary disease (COPD) and lower weight. We investigated the amount of physical activity as the time spent in posture and movement and its association with systemic inflammation. [Participants and Methods] In this retrospective cross-sectional pilot study, we evaluated 11 patients with COPD (age, 73 ± 7 years; body mass index, 18.9 ± 2.9 kg/m2). A recently developed triaxial accelerometer was used to measure the time spent in posture and movement. We also evaluated body composition, physiological indexes, and serum levels of inflammatory cytokines. Single correlation coefficients were calculated as the association between physical activity and other outcomes. [Results] The walking time was 36 ± 32 min/d, and the standing time was 151 ± 118 min/d. The time spent walking significantly correlated with the fat-free mass index (r=0.73) and interleukin (IL)-8 level (r=0.76). The time spent standing significantly correlated with the C-reactive protein (r=0.80) and IL-6 levels (r=0.74). [Conclusion] These data indicate that increased physical activity is associated with higher systematic inflammation. We should consider that the systemic inflammation may have been affected by the increased physical activity of the patients with COPD and lower weight in this study.

[Purpose] The aim of this study was to test the hypothesis that Lee Silverman Voice Treatment-BIG decreases the negative impact of hypokinesia on dual task performance in persons with Parkinson’s disease. [Subjects and Methods] The records of 114 patients with Parkinson’s admitted to outpatient rehabilitation at a suburban hospital were reviewed. Demographics and data for 8 outcome measures were extracted for subjects that completed 14 of 16 sessions of BIG. 93 of these subjects had records of pre and post-test Timed Up and Go, Timed Up and Go Motor, and Timed Up and Go Cognitive scores. Average age was 68.4 years (SD=10.6) and average disease duration was 4.9 years (SD=5.3). [Results] Subjects demonstrated statistically significant improvements for Timed Up and Go (3.3 SD=4.5), Timed Up and Go Motor (4.4 SD=5.8) and Timed Up and Go Cognitive (4.7 SD=5.4). Concurrent motor and cognitive performance remained stable. Dual task cost decreased at a statistically significant level for Timed Up and Go Cognitive (7% SD=31%) but not Motor (4% SD=32%). [Conclusion] These findings suggest that cueing strategies associated with LSVT BIG become internalized and decrease the negative impact of hypokinesia on mobility and cognitive performance while performing two tasks simultaneously in persons with Parkinson’s.

[Purpose] This study examined postural stability and muscle activity when limitations were placed on the ankle and knee joints. [Subjects] Forty-eight healthy young adults were randomly assigned to one of three groups: a fixed-ankle group (6 males, 9 females), a fixed-ankle-and-knee group (9 males, 7 females), and a non-fixed group (7 males, 10 females). [Methods] The fixed-ankle subjects were limited using orthopedic fiberglass casting tape and elastic bandages and the fixed-ankle-and-knee subjects were additionally limited using a knee immobilizer. There were no limitations on the ankle and knee joints of the non-fixed group subjects. We measured postural stability and muscle activities of the ankle and knee muscles. [Results] There were differences in the muscle activities of the tibialis anterior, median gastrocnemius, and rectus femoris between the groups. The greatest muscle activity in the tibialis anterior was shown by the non-fixed group, and the fixed-ankle group showed the greatest rectus femoris and gastrocnemius activities. There were significant differences in the overall stability index, anterior-posterior stability index, and medial-lateral stability index. All three indices were the lowest for the non-fixed group. [Conclusion] A fixed ankle or knee reduces muscle activity which in turn lowers postural stability.

[Purpose] The Asian Working Group for Sarcopenia (AWGS) criteria were recently updated. However, whether these changes would result in a difference in sarcopenia prevalence was unclear. We therefore focused on Japanese older adults who required long-term care/support and determined the differences in sarcopenia prevalence between the new and old criteria. [Participants and Methods] This cross-sectional study included 161 Japanese older adults aged ≥65 years who required long-term care/support along with ongoing daycare. Handgrip strength, usual gait speed, and skeletal muscle mass index were measured. We analyzed the difference in sarcopenia prevalence between the 2019 and 2014 AWGS criteria using the McNemar test. [Results] The overall sarcopenia prevalence rates were 60.2% and 53.4%, and the prevalence rates of sex-specific sarcopenia were 63.6% and 55.7% among males and 56.2% and 50.7% among females when the 2019 and 2014 AWGS criteria were used, respectively. Overall, males exhibited a significantly higher prevalence with the new than criteria than with the old. [Conclusion] With the 2019 AWGS criteria, more older males who required long-term care/support were diagnosed as having sarcopenia. Conversely, the sarcopenia diagnosis in females statistically remained unchanged. Thus, a mismatch might exist between the two criteria regarding sarcopenia prevalence in males.

[Purpose] The aim of this study was to examine the effects of exercise to strengthen the muscles of the hip together with lumbar segmental stabilization exercise on the lumbar disability index, lumbar muscle strength, and balance. [Subjects and Methods] This study randomly and equally assigned 40 participants who provided written consent to participate in this study to a lumbar segmental stabilization exercise plus exercise to strengthen the muscles of the gluteus group (SMG + LES group) and a lumbar segmental stabilization exercise group. [Results] Each evaluation item showed a statistically significant effect. [Conclusion] Clinical application of exercise in this study showed that lumbar segmental stabilization exercise plus exercise to strengthen the muscles of the gluteus resulted in a greater decrease in low back pain disability index and increase in lumbar muscle strength and balance ability than lumbar segmental stabilization exercise in chronic low back pain patients receiving the exercise treatments during the same period.

[Purpose] This study investigated the effects of manual manipulation therapy on the pain and dysfunction of patients with lumbar spinal stenosis. [Participants and Methods] In this study, 30 patients with chronic back pain were evenly divided into an experimental group, who received manual traction therapy, and a control group, who received intermittent traction therapy. Both groups received therapy three times a week for eight weeks. A visual analogue scale was used to measure participants’ back pain, and the Oswestry disability index (ODI) was used to check the functional impediment they experienced as a result. [Results] The intragroup comparison showed that the visual analog scale and the ODI significantly decreased in the control group and the experimental group, respectively. The intergroup comparison after treatment showed that the visual analog scale and the ODI of the experimental group were significantly lower than in the control group. [Conclusion] The results of this study suggest that manual manipulation therapy is an effective intervention for treating pain and dysfunction in patients with lumbar spinal stenosis.

〔目的〕競技経験が異なる群間の動作分析実験から,動作の差異を考察し,初心者指導を検討する.〔対象〕ボクシング経験者(26.2±6.1歳)と未経験者(31.3±1.2歳)各5名とした.〔方法〕実験では光刺激を合図に右ストレートで,すばやくターゲットを打つことを要求した.測定装置は筋電計と床反力計を用い,定量化した特徴量を基準に動作を4期間に分割した.測定指標は,6期間時間,床反力作用点(COP)軌跡変位量,および筋活動の各特徴量とし,群間で統計学的解析を行った.〔結果〕経験群でパンチモーション期間の短縮,COP軌跡変位量に準備期の減少と減速期の増加,2筋における筋活動開始時点の変化,および3筋に筋活動時間の短縮が確認された.〔結語〕競技初心者に対して構えの設定に着目し指導教示を検討した. <br>

[Purpose] To find the physical activity level and fall risk among the community-dwelling Malaysian older adults and determine the correlation between them. [Subjects and Methods] A cross-sectional study was conducted in which, the physical activity level was evaluated using the Rapid Assessment of Physical Activity questionnaire and fall risk with Fall Risk Assessment Tool. Subjects recruited were 132 community-dwelling Malaysian older adults using the convenience sampling method. [Results] The majority of the participants were under the category of under-active regular light-activities and most of them reported low fall risk. The statistical analysis using Fisher’s exact test did not show a significant correlation between physical activity level and fall risk. [Conclusion] The majority of community-dwelling Malaysian older adults are performing some form of physical activity and in low fall risk category. But this study did not find any significant correlation between physical activity level and fall risk among community-dwelling older adults in Malaysia.

[Purpose] Aging is associated with a progressive decline in overall muscle strength. Loss of lower limb strength leads to an increased risk of falls and a sedentary lifestyle. The purpose of this study was to investigate whether lower limb strengthening exercise leads to improved lower limb strength and balance function for the elderly. [Subjects] From a total of 74 respondents, 50 subjects were randomly assigned to either a training group (n = 30) or a control group (n = 20). The subjects ranged in age from 65 to 82 years. A randomized controlled trial compared the effects of strengthening exercise and balance function. [Methods] Leg extension and lower curl exercises were performed during the 12-week study. [Results] After training, the lower limb strength and balance of the individuals in the training group had significantly improved compared to the baseline. [Conclusion] Improvement in lower limb strength may lead to balance enhancement in neurologically intact older persons.

[Purpose] Improvement in the smoothness of movement is a motor learning outcome. This study sought to clarify the relationship between motor skills and smoothness of movement in motor learning. [Participants and Methods] We subjected 12 healthy adults to a task in which they had to learn the sensation of a load while standing up and sitting down. We attached triaxial accelerometers to the seventh cervical spine and the third lumbar spinous process of the participants prior to measurement. We took the measurements over two successive days and used absolute error and variable error as indicators of motor learning outcomes. In addition, we used entropy, calculated from the results of the power spectrum analysis of acceleration changes, as an indicator of the smoothness of the movement. [Results] In the test sessions, absolute and variable errors showed a significant reduction. Entropy also showed a similarly significant decrease, although the change in errors and entropy showed different transitions. [Conclusion] Qualitative indicators of motor learning captured an aspect of motor learning that one cannot capture by quantitative indicators. In the future, qualitative indicators will be necessary to judge the outcomes of motor learning.

筋の時間的・空間的な収縮特性は、表面筋電図学的な解析によってある程度観測することが可能である。しかし、測定から信号処理までの過程における方法論的な手続きに対する配慮が不十分であると、抽出された情報の実際的な利用価値は低いものになってしまう。そこで、本稿では、表面筋電図学的解析に関する基礎的な情報に関して整理した上で、筋収縮力、筋疲労、筋カトレーニングといった筋収縮特性と表面筋電図との関連性に関して概説する。

[Purpose] The purpose of this research was to analyze the efficacy of extracorporeal shock wave therapy for the treatment of stroke patients with plantar fasciitis. [Subjects and Methods] This study included 10 stroke patients diagnosed with plantar fasciitis who were administered 3 sessions of extracorporeal shock wave therapy per week. After the last session, they performed stretching exercises for their Achilles tendon and plantar fascia for 30 min/day, 5 times a week for 6 months. The following parameters were measured and compared prior to therapy, 6 weeks after therapy, and 6 months after therapy: thickness of the plantar fascia, using an ultrasonic imaging system; degree of spasticity, using a muscle tension measuring instrument; degree of pain, using the visual analogue scale; and gait ability, using the Functional Gait Assessment. [Results] Decreased plantar fascia thickness, spasticity, and pain and increased gait ability were noted after therapy. These changes were significantly greater at 6 months after therapy than at 6 weeks after therapy. [Conclusion] These results indicated that extracorporeal shock wave therapy reduced tension in the plantar fascia, relieving pain and improving gait ability in stroke patients.

[Purpose] Cervical vertigo as a common complaint is associated with some musculoskeletal disorders. However, to date, ultrasonographical parameters of cervical muscles in patients with cervical vertigo have not been investigated. This study was conducted to investigate size of cervical muscles in patients with cervical vertigo compared to healthy controls. [Participants and Methods] Thicknesses of cervical flexor and extensor muscles were evaluated through ultrasonography and results were compared between the patients and healthy controls by Independent Samples t-test or Mann-Whitney U nonparametric test. [Results] Results showed that, thickness of Longus Colli muscle was significantly different between the patients and healthy controls. [Conclusion] According to findings of the study, size of Longus Colli muscle is likely to be associated with etiology of cervical vertigo.

[Purpose] The purpose of the study was to assess the muscle activity change of the tibialis posterior, flexor digitorum longus, and peroneus longus during gait with orthoses. [Subjects and Methods] Sixteen healthy males participated in this study. Activity of each muscle was measured by using fine-wire and surface electromyography. Gait task was performed by the participants barefoot, with footwear and with orthoses. The electromyography data from a stance phase of each gait trial were used for analysis. The stance phase was divided into contact, midstance, and propulsion phases. The data from ten participants were extracted for final analysis, as electromyography measurements were unsuccessful for the other six. [Results] The results demonstrated that orthoses significantly reduced the tibialis posterior muscle activity in the propulsion phase compared to that in the barefoot condition. Although there was a significant difference in the midstance phase, post hoc analysis did not indicate significant differences among the phases. No significant electromyography amplitude change was detected in flexor digitorum longus and peroneus longus. [Conclusion] Orthothes reduced the tibialis posterior activity level during gait. This result may be beneficial for patients with injuries related to excessive activity of tibialis posterior.

[Purpose] Although shortwave diathermy has been widely used by physiotherapists, there are a few studies assessing the performance of the equipment in use. The aim of the present study was to evaluate the procedures adopted by physiotherapists as users of shortwave diathermy continuous (CSWD), as well as to measure the power output and frequency of CSWD equipment. [Subjects and Methods] Twenty-three physical therapists were interviewed and 23 CSWD equipment were evaluated. Admeasurement was carried out by using a standard phantom to simulate the electrode-skin distance, which ranged from 0.5 to 3.0 cm. Data analysis was performed by using descriptive statistics, ANOVA, and a post-hoc Tukey’s test or Pearson’s correlation coefficient. [Results] The questionnaires showed that 48% of the interviewees use the correct electrode-skin distance, 70% use a single electrical outlet, and 35% use a grounded electrical outlet, and that 48% of the physiotherapy tables and 61% of the plinths were made of wood. However, only 13% of the interviewees perform yearly preventive maintenance. The highest power (95.56 W) was achieved at electrode-skin distances ranging from 1.0 to 1.5 cm, with distances of 2.5 cm and 3.0 cm being null in four and eight equipment, respectively. There was a negative correlation between power output and electrode-skin distance as well as between power output and purchase date. [Conclusion] The physiotherapists involved in this study had inadequate knowledge about the correct use of CSWD equipment, which may adversely affect its performance and patient safety.

The purpose of this study was to determine the relationship between the changes in intramuscular blood volume (IMBV) under continuous shortwave diathermy (CSWD) and compare them with the electric hot pack (EHP). The subjects consisted of 41 healthy adults, who received one of three interventions: CSWD (n=17), EHP (n=12), or no intervention (Controls: n=12). Analyses were done of the values obtained five minutes preceding the intervention (T1), during the intervention (T2), and after the intervention (T3). CSWD was applied with an output of 80 W. Both CSWD and EHP were applied for 10 min. Changes in IMBV of the right medial gastrocnemius muscle were evaluated using near-infrared spectroscopy (NIRS). Oxy-hemoglobin (oxy-Hb), deoxy-hemoglobin (deoxy-Hb), and total-hemoglobin (total-Hb) were measured using NIRS. At T2 in the CSWD trial, the integration values of oxy-Hb and total-Hb were significantly increased compared with those of the controls (p<0.01), and they were significantly increased compared with those of both the controls and the EHP trial (p<0.01) at T3 of the CSWD trial. The study results verified that IMBV increases as a result of the local thermal effect of CSWD, and also suggest that CSWD is more effective at enhancing IMBV than EHP.

[Purpose] The purpose of this study was to identify the effects of trunk stability exercise using proprioceptive neuromuscular facilitation with changes in chair heights on the gait of stroke patients. [Subjects and Methods] The subjects of this study were 11 stroke patients. The intervention method was trunk stability exercise using proprioceptive neuromuscular facilitation with different chair heights (50, 60, and 70 cm). These exercises were performed 5 times per week for 6 weeks. Gait velocity, cadence, stride length, gait cycle, and stance phase duration were used to measure gait function. [Results] Significant changes in gait velocity, cadence, and stride length were observed on the affected side. However, no significant changes in gait cycle and stance phase were observed on the affected side. [Conclusion] These results indicate that trunk stability exercise using proprioceptive neuromuscular facilitation with change in chair heights were effective in improving gait velocity, cadence, and stride length on the affected side. However, in this study, no significant changes were observed in gait cycle and stance phase on the affected side. Therefore, various interventions for stroke patients should be investigated in further studies.

[Purpose] The aim of this study was to investigate the effect of neuromuscular electrical stimulation on facial muscle strength and oral function in stroke patients with facial palsy. [Subjects and Methods] Nine subjects received the electrical stimulation and traditional dysphagia therapy. Electrical stimulation was applied to stimulate each subject’s facial muscles 30 minutes a day, 5 days a week, for 4 weeks. [Results] Subjects showed significant improvement in cheek and lip strength and oral function after the intervention. [Conclusion] This study demonstrates that electrical stimulation improves facial muscle strength and oral function in stroke patients with dysphagia.

[Purpose] The purpose of this study was to investigate the effects of interferential current therapy on the waist circumference and visceral fat length in obese women. [Subjects and Methods] In this study, we selected 30 patients whose body mass index was over 25 kg/m2. The subjects were randomly assigned to receive interferential current therapy three times a week for four weeks (n=15) and transcutaneous electrical nerve stimulation three times a week for four weeks (n=15). Waist circumference was measured in cm using a tape measure with the participant in an upright posture, and the length of visceral fat was measured using a high-resolution, B-mode ultrasound machine. [Results] There was a statistically significant difference between the group of waist circumference and visceral fat length. The change of waist circumference and visceral fat length was larger the experimental group than the control group. [Conclusion] These results can be used as a basis for reducing the risk factors that increase mortality due to diseases and can prevent cardiovascular and other adult diseases caused by obesity.

[Purpose] The aim of this study was to investigate the availability, usage, and factors affecting usage of electrophysical agents by physical therapists in Nagano Prefecture, Japan. [Subjects and Methods] Questionnaires were sent to all 1,571 physical therapists working in 245 institutions within Nagano Prefecture. A total of 1,110 questionnaires were returned, out of which 1,099 (70%) questionnaires containing valid responses were analyzed. Frequencies and percentages were calculated for 22 modalities with regards to availability, usage, rate of usage, and confidence level in usage. Factors affecting usage and the relationship between rate of usage and confidence level (Spearman’s rho) were also determined. [Results] The top three responses for the various outcome measures were as follows: (1) hot packs (88%), low frequency stimulators (76%), and ultrasound (68%) for availability; (2) hot packs (72%), ultrasound (61%), and cold packs (59%) for usage; (3) hot packs (75%), cold spray (49%), and ultrasound (44%) for confidence in usage; and (4) equipment availability (80%), past experience (79%), and research evidence (78%) for factors affecting usage. There was a significant positive relationship between confidence and usage for all modalities, except for ultraviolet radiation, iontophoresis, and magnetic field. [Conclusion] Usage was strongly correlated with confidence, with the top three used modalities also being the ones with the highest confidence in usage.
