神経系は全身に分布し,物理的,電気的,化学的手段によって情報を伝達している。神経系は四肢,体幹に分布するため,身体運動に対して柔軟に適応するメカニズムを有している。このようなメカニズムは外傷等により障害され,痛みや可動域障害などの原因となる。従来の理学療法では神経系は主として検査・評価対象であったが,神経系モビライゼーションでは病態に応じた積極的なアプローチが行われる。本論では,神経系機能障害の病態について概説し,神経系の機能障害に対する検査,治療について記載する。

慢性閉塞性肺疾患(以下COPD)患者では歩行時の移動動作以外にも,腕を挙上位で使う日常生活活動において障害を訴えるものも多い。そこで今回我々は,COPD患者6例に対し,定量的腕エルゴメーター負荷訓練を実施した。そして,同負荷訓練がCOPD患者の自覚症状,体幹機能およびKraus-Weberテストなどの評価で検討した結果,背筋持久性,体幹の柔軟性,さらに分時換気量の改善に伴って呼吸困難度が有意な改善を示した。以上のことにより,COPD患者にとって,定量的腕エルゴメーター負荷試験は,体幹の柔軟性,背筋の機能を改善することによって,呼吸困難度を軽減する有効な方法であると考えられた。

[Purpose] This study was performed to investigate the effects of yogic eye exercises on eye fatigue in undergraduate nursing students. [Subjects and Methods] The study used a pretest-posttest design with a non-equivalent control group. Forty undergraduate nursing students were selected by convenience sampling, with 20 assigned to an exercise group and 20 assigned to a control group. The yogic eye exercise intervention was performed for 60 minutes, two days a week for 8 weeks. It consisted of 8 steps: palming, blinking, sideways viewing, front and sideways viewing, rotational viewing, up and down viewing, preliminary nose tip gazing, and near and distant viewing. Eye fatigue was measured using a questionnaire for evaluating ocular fatigue. [Results] The exercise-group measurements revealed a significantly decreased eye-fatigue score compared with that of the control group. [Conclusion] These findings indicate that yogic eye exercises could reduce the eye fatigue score in undergraduate nursing students.

胸部外科における心肺疾患患者の肺理学療法目的は、術前、術後の呼吸機能低下が原因で出現する種々の合併症予防と換気効率低下改善にある。術前において最も重要なことは、患者及び家族教育であり、呼吸訓練の重要性を理解してもらうことは肺理学療法の目的達成のポイントとなり得る。次は、術前、術後の全身状態が非常に異なることを念頭に置き、患者が普段から意識しなくても腹・胸式呼吸パターンが行える様に指導する。最後に、術前、術後の呼吸音を聴診することで、これは術後背臥位期間中の外側肺底区・後肺底区の聴診は異常呼吸音の早期発見につながるからである。

[Purpose] This study compared the effectiveness of home exercise alone versus home exercise combined with ultrasound for patients with temporomandibular joint disorders. [Subjects and Methods] This study enrolled 23 female and 15 male patients who were divided randomly into two groups. The home exercise group performed a home exercise program consisting of an exercise program and patient education, and the home exercise combined with ultrasound group received ultrasound therapy in addition to the home exercise program. Pain intensity was evaluated using a visual analogue scale. Pain free maximum mouth opening was evaluated at baseline and 2 weeks after the treatment. [Results] There was no difference between the two groups in baseline values. After the treatment, the visual analogue scale decreased and pain free maximum mouth opening scores improved significantly in each group. Additionally, both values were higher in the home exercise combined with ultrasound group than in the home exercise group. [Conclusion] The combination of home exercise combined with ultrasound appears to be more effective at providing pain relief and increasing mouth opening than does home exercise alone for patients with temporomandibular joint disorders.

[Purpose] The purpose of the present study was to examine the immediate effects of passive hamstring stretching exercises on cervical spine range of motion and balance. [Subjects] The present study was conducted with 60 healthy university students without any musculoskeletal dysfunction as subjects. They were divided into an experimental group consisting of 30 subjects and a control group consisting of 30 subjects. [Methods] Cervical spine range of motion was measured using a cervical range of motion goniometer, and the stability test was conducted to assess balance. The experimental group were administered hamstring stretching with ankle dorsiflexion for 30 seconds three times, whereas the control group received the same treatment without ankle dorsiflexion. [Results] Cervical spine range of motion and balance immediately increased in the experimental group while there was no change in the control group. [Conclusion] The results show that hamstring muscle stretching exercises the fascia of the skeletal muscles of the human body and that the fascia are connected to each other by interactions of force. The human skeletal muscles interacted with each other to increase the flexion and extension range of motion of the cervical spine. In addition, the transfer of these forces to the stabilizer muscles of the pelvis and spine were the most important factor in the improvement of the subjects’ balance.

[Purpose] The aim of this study was to determine the lower-limb muscle activities during muscle exercise using an on-the-bed muscle training device that utilizes the intermittent inflation of an oval rubber balloon placed against the sole. [Subjects and Methods] Ten healthy subjects were asked to adopt a semi-recumbent position with a flat oval chloridized polyvinyl balloon placed against the sole. The subjects were asked to extend both their legs when they felt the inflation of the balloon on their plantar surface. This exercise was defined as leg exercise against intermittent sole pressure stimulation (ExISPS). [Results] The root-mean-square (RMS)-EMG values of the vastus medialis during ExISPS in the semi-recumbent position were similar to those when standing up from 45-degree knee flexion and calf-raising. The RMS-EMG values of the biceps femoris showed no significant difference among these exercises. The RMS-EMG values of gastrocnemius medialis during ExISPS in the semi-recumbent position were higher than those when standing up from 45-degree knee flexion and standing up from a chair. [Conclusion] The findings of this study suggest that the muscle activities during ExISPS mimic or are greater than those required of the leg in daily life.

脳卒中片麻痺患者の各基本動作間の難易度は,一般的には寝返り,起座,起立,歩行の順に難しくなると言われるが,その順位の異なる者の比率とその原因について検討するため片麻痺患者40名を対象に調査した。一般的順位と同じ「同群」は29名,異なる「異群」11名で,このうち起座が不可で起立,歩行が可の「異群(1)」は9名であった。「異群(1)」は股伸展0°以下と体幹回旋制限5°以上において「同群」と有意差があり,異群(1)のうち寝返り,起座とも不可の群では体幹屈曲5°以上制限にも有意差を認め,これらの動作困難の原因として股・体幹可動域制限の影響が示唆された。対象者が高齢で,物的介助起立・平行棒内歩行と低い能力レベルで,半側無視例が多くいたことが,このような動作難易度順位の逆転現象の誘因として考えられた。

[Purpose] The purpose of this study was to measure the effect of structural components on walking ability by conducting tests using subjects who used combined rollator-wheelchairs and walking aids in their daily lives. [Subjects] Ten residents living in a health care facility for the elderly who required walking aids participated in this study. [Methods] We measured maximum walking speed (MWS), step length, cadence and conducted the shuttle stamina walk test (SSTw), and the timed up-and-go test (TUG) under three conditions (rollator with forearm support, rollator with handgrip and cart). [Results] There were significant differences among the conditions with forearm support providing the best results in terms of MWS, step length, cadence, SSTw and TUG, showing in improved walking abilities of walking speed, endurance and dynamic balance. [Conclusion] These results show that therapists should advocate using rollators with forearm support as a safe method of expanding the range of walking activities for elderly who use walking aids or rollator-wheelchairs in their daily lives.

[Purpose] The study investigated stretching for safer a golf swing compared to present stretching methods for proper swings in order to examine the effects of stretching exercises on golf swings. [Subjects] The subjects were 20 amateur golf club members who were divided into two groups: an experimental group which performed stretching, and a control group which did not. The subjects had no bone deformity, muscle weakness, muscle soreness, or neurological problems. [Methods] A swing analyzer and a ROM measuring instrument were used as the measuring tools. The swing analyzer was a GS400-golf hit ball analyzer (Korea) and the ROM measuring instrument was a goniometer (Korea). [Results] The experimental group showed a statistically significant improvement in driving distance. After the special stretching training for golf, a statistically significant difference in hit-ball direction deviation after swings were found between the groups. The experimental group showed statistically significant decreases in hit ball direction deviation. After the special stretching training for golf, statistically significant differences in hit-ball speed were found between the groups. The experimental group showed significant increases in hit-ball speed. [Conclusion] To examine the effects of a special stretching program for golf on golf swing-related factors, 20 male amateur golf club members performed a 12-week stretching training program. After the golf stretching training, statistically significant differences were found between the groups in hit-ball driving distance, direction deviation, deflection distance, and speed.

[Purpose] The purpose of this study was to determine whether exercises using proprioceptive neuromuscular facilitation (PNF) scapular and pelvic patterns might decrease the pain index and increase the lumbar flexibility of obese patients with low back pain. [Subjects and Methods] Thirty obese patients with low back pain were randomly assigned to an experimetal group (n=15) and a control group (n=15). The exercise program of the experimental group consisted of scapular patterns (anterior depression − posterior elevation) and pelvic patterns (anterior elevation − posterior depression). The control group performed neutral back muscle strengthening exercises. Over the course of four weeks, the groups participated in PNF or performed strengthening exercises for 30 minutes, three times per week. Subjects were assessed a pre-test and post-test using measurements of pain and lumbar flexibility. [Results] The results show that lumbar flexion and lumbar extension significantly improved in the experimental group, had significant improvement and that the Oswestry Disability index (ODI) significantly decreased. However, there were no significant changes in the control group. The experimental group also showed significant differences in the pain index and lumbar flexibility from the control group. [Conclusion] This study showed that PNF can be used to improve pain index rating and lumbar flexibility. The findings indicate that the experimental group experienced greater improvement than the control group by participating in the PNF lumbar stabilization program.

[Purpose] As an alternative to manual stretching, the aim of this study was to investigate the feasibility of using neural/visceral manipulation as a safe and effective intervention to increase neck range of motion of infants with congenital muscular torticollis. [Participants and Methods] Ten 4-month old infants with congenital muscular torticollis received eight sessions of neural/visceral manipulation administered for 30–50 minutes without observed pain. Specific palpation techniques addressed restricted tissue areas of neck, head, trunk and extremities. Neck rotation and lateral flexion were assessed by still photography and a computer program calculating ROM angles before, immediately following, and 4 months post intervention. Motor development and social competence were monitored over time using the Alberta Infant Motor Scale and Bayley-III Social Emotional Scale. [Results] Results of analysis of variances revealed significant improvements in passive and active neck rotation and lateral flexion. Significant increases were also found on the Alberta Infant Motor Scale and Bayley-III Social-Emotional scale. [Conclusion] Neural/visceral manipulation can be used safely in infants with congenital muscular torticollis to improve neck range of motion.

本研究の目的は、脳卒中片麻痺患者の片膝立ちにおいて麻痺前と、非麻痺側前では、どちらが容易であるかにっいて検討することである。対象は、両膝立ちが監視レベル以上で可能な脳卒中片麻痺患者69例である。結果は、発症後1年未満と1年以上、発症後6か月未満と6か月以上1年未満との比較、および麻痺側別の比較でも非麻痺側前の片膝立ちが容易であることを認めた。下肢ブルンストローム・ステージ別の比較ではステージIII、VIにおいて非麻痺側前の片膝立ちが容易でありIV・Vでは麻痺側前と非麻痺側前の間には難易度に差を認めなかった。

[Purpose] The aim of this study was to evaluate the immediate effects of Mulligan fibular taping on static and dynamic postural balance in healthy subjects using computerized dynamic posturography (CDP). [Subjects and Methods] Forty-four volunteers (26 males and 18 females) aged 21 ±2 years participated in the study. The Mulligan tape was applied by a specialist in this technique. The placebo group received a treatment with a similar tape but with several cuts to avoid the fibular repositioning effect produced by Mulligan tape. Main outcome measures: The Sensory Organization Test (SOT) and the Motor Control Test (MCT) were performed by each subject at baseline and after the interventions. Outcome measures included equilibrium and strategy scores from each trial and condition of the SOT, and speed of reaction (latency period) from the MCT. [Results] Mulligan ankle taping did not have an impact on postural control during static and dynamic balance in subjects with healthy ankles when compared with placebo taping. [Conclusion] There was no difference in, equilibrium and strategy (SOT) and speed of reaction (MCT) in any of the subjects in this study. Therefore, this study suggests that Mulligan ankle taping does not have an impact on balance in healthy subjects.

[Purpose] Hippotherapy is an unusual type of treatment and has been found to be effective for diseases of the musculoskeletal system and rehabilitation. Horseback riding simulator is used as a beneficial alternative to the real horse with utilizing an optical motion capture system and EEG. [Participants and Methods] The idea is to monitor body and brain behaviour of the professional rider and non-professional rider utilizing a horse simulator, using optical motion capture system to identify differences in pelvic region activity between professional and non-professional riders and EEG to investigate the brain effect of professional rider utilizing horseback riding simulator. [Results] For the monitoring body and brain behaviour of the professional rider and non-professional rider, two types of experiment were handled, the first experiment represents body behaviour and the second experiment represents brain behaviour. [Conclusion] The study shows, that inexperienced rider may make mistakes of pelvis movements that leads to the asymmetry in hip external rotation and back region. Also, the study of EEG provides that while horseback riding mostly frontal lobe is active, that refers to concentration, body movements and intelligence. Moreover, temporal and parietal lobes are highlighted that relates to sensor-motor cortex and moving which are needed during riding.

片麻痺患者の動作分析について、運動分析を含めた概念でそれぞれの障害レベルにおける運動・動作分析について述べた。いづれの分析においても、それらを片麻痺患者の障害像の把握と理学療法施行上のプログラム計画、更に理学療法効果の指標に役立てることである。これまで筆者らが行ってきた運動・動作分析に関連した研究の一部を紹介し、片麻痺患者の何を対象に、そして何の目的で行う必要があるかを提示した。

[Purpose] The purpose of this study was to examine the relationship between physical function and postural sway during local vibratory stimulation of middle-aged subjects in an upright position. [Subjects] The subjects were 25 healthy community-dwelling middle-aged people. [Methods] We measured postural sway using a Wii board while vibratory stimulations of 30, 60, or 240 Hz were applied to the subjects’ lumbar multifidus or gastrocnemius muscles. Physical function was evaluated by 5-m usual gait speed and grip strength. [Results] Gait speed was strongly correlated to the anteroposterior body sway in the upright position during 30 Hz gastrocnemius muscles vibration (GMV). [Conclusion] Postural sway during 30 Hz GMV was strongly associated with gait speed and showed a posterior displacement. These findings show that the lower leg’s response to balance control under 30 Hz proprioceptive stimulation might be a good indicator of declining gait function.

[Purpose] The purpose of this study was to examine the relationship between the paraspinal muscle cross-sectional area and the relative proprioceptive weighting ratio during local vibratory stimulation of older persons with lumbar spondylosis in an upright position. [Subjects] In all, 74 older persons hospitalized for lumbar spondylosis were included. [Methods] We measured the relative proprioceptive weighting ratio of postural sway using a Wii board while vibratory stimulations of 30, 60, or 240 Hz were applied to the subjects’ paraspinal or gastrocnemius muscles. Back strength, abdominal muscle strength, and erector spinae muscle (L1/L2, L4/L5) and lumbar multifidus (L1/L2, L4/L5) cross-sectional areas were evaluated. [Results] The erector spinae muscle (L1/L2) cross-sectional area was associated with the relative proprioceptive weighting ratio during 60Hz stimulation. [Conclusion] These findings show that the relative proprioceptive weighting ratio compared to the erector spinae muscle (L1/L2) cross-sectional area under 60Hz proprioceptive stimulation might be a good indicator of trunk proprioceptive sensitivity.

[Purpose] This study aims to determine the specific proprioceptive control strategy used during postural balance in older patients with low back pain (LBP) and non-LBP (NLBP) and to assess whether this strategy is related to proprioceptive decline and LBP. [Subjects and Methods] Pressure displacement center was determined in 47 older persons with LBP and 64 older persons with NLBP during upright stance on a balance board without vision. Gastrocnemius (GS) and lumbar multifidus muscle (LM) vibratory stimulations of 60 and 240-Hz, respectively, were applied to evaluate the relative contributions of different proprioceptive signals (relative proprioceptive weighting ratio, RPW) used in postural control. Age, height, weight, back muscle strength, L1/2 and L4/5 lumbar multifidus cross section area ratio, skeletal muscle mass index, sagittal vertical axis, and Roland-Morris disability questionnaire (RDQ) were evaluated. [Results] Compared with older patients with NLBP, those with LBP showed a lower RPW 240-Hz, lower L4/5 lumbar multifidus cross-sectional area ratio, and a significantly higher age and RDQ. Logistic regression analysis showed that RPW 240-Hz and age were independently associated with LBP, after controlling for confounding factors. [Conclusion] Older patients with LBP decreased their reliance on GS (RPW 240-Hz) proprioceptive signals during balance control.

[Purpose] The aim of this study was to evaluate the effect of pulsed high intensity neodymium-doped yttrium aluminum garnet (Nd: YAG) laser on staphylococcus aureus (S. aureus) and pseudomonas aeruginosa (P. aeruginosa) bacterial growth, which cause many health problems and establish which doses are effective in bacterial inhibition. [Materials and Methods] Five samples of S. aureus and five samples of P. aeruginosa were prepared in the microbiology lab, one used as control sample and the other 4 samples acted as experimental samples. The experimental samples received pulsed high intensity Nd: YAG laser with a total dose of 500, 600, 700 and 800 joules. The primary measures are colony count and the percentage decrease in colony count, the colony count was assessed at baseline and after 24 h of laser application. [Result] There was significant decrease in colony count and the percentage decrease in colony count after pulsed high intensity Nd: YAG laser application in all experimental samples of S. aureus and P. aeruginosa after 24 h of application for all doses (500, 600, 700 and 800 j) as compared with the control sample, with the most effect in higher doses of pulsed high intensity Nd: YAG laser than lower doses in both types of bacteria. [Conclusion] pulsed high intensity Nd: YAG laser was found to be an effective modality for inhibition of S. aureus and P. aeruginosa growth after a single application.
