[Purpose] This study aimed to evaluate the impact of lung function and peripheral muscle function on the six-minute walking distance (6MWD) in systemic sclerosis (SS) patients and, thereby, to develop an explanatory model of functional exercise capacity for these individuals. [Methods] In a cross-sectional study, 31 SS patients underwent pulmonary function testing (including spirometry, diffusing capacity for carbon monoxide [DLCO], and respiratory muscle strength), isometric dynamometry with surface electromyography, and the 6MWD. [Results] There was a significant correlation between the 6MWD (% predicted, 6MWD%) and the following parameters: height (r = 0.427) and DLCO (r = 0.404). In contrast, no other independent variable showed a significant correlation with the 6MWD% (r ≤ 0.257). The final prediction model for 6MWD% (adjusted R2 = 0.456, SE of bias=12%) was 6MWD% Gibbons = −131.3 + 1.16 × heightcm + 0.33 × DLCO% predicted. [Conclusion] In SS patients, body height and pulmonary diffusion are the main determinants of the 6MWD. Our results justify further investigation of the performance of SS patients during exercise, which may increase the understanding of the pathophysiological mechanisms involved in the disease. The impact of these findings in SS patients may be useful for evaluating the effects of rehabilitation programs.

[Purpose] This study aimed to investigate the movement of the thorax, lumbar spine, and pelvis when healthy participants sit on a chair, and to identify the kinematic characteristics due to changes in the height of the seat. [Participants and Methods] Twenty healthy participants (14 males, 6 females; mean age, 29 ± 5 years) were recruited for this study. They performed stand-to-sit motion using one seat with a height of 100% that of the lower leg length (standard) and another with a height of 60% that of the lower leg length (lower). A three-dimensional motion analysis system and four force plates were used to analyze each joint angle. [Results] The mean lumbar spine flexion angle was significantly increased in the lower versus the standard seat. As a kinematic characteristic, the pelvis tilted posteriorly while the thorax tilted anteriorly, which increased the lumbar spine flexion angle. The pelvis was tilted posteriorly when the hip joint flexed about 60° regardless of the seat height. [Conclusion] The lumbar spine flexion angle increased in the lower seat stand-to-sit motion, which suggested an increase in the load on the lumbar spine. The lumbar spine flexion angle was influenced by the characteristic movements of the thorax and pelvis.

[Purpose] To compare the toe flexor, hand grip and knee extensor strengths of young and elderly men, and to examine the association between toe flexor strength and physical activity or inactivity levels. [Subjects and Methods] Young (n=155, 18–23 years) and elderly (n=60, 65–88 years) men participated in this study. Toe flexor, hand grip, and knee extensor strength were measured. Physical activity (time spent standing/walking per day) and inactivity (time spent sitting per day) were assessed using a self-administered questionnaire. [Results] Toe flexor, hand grip, and knee extensor strength of the elderly men were significantly lower than those of the young men. Standing/walking and sitting times of the elderly men were lower than those of the young men. Toe flexor strength correlated with hand grip and knee extensor strength in both groups. In elderly men, toe flexor strength correlated with standing/walking time. In comparison to the young men’s mean values, toe flexor strength was significantly lower than knee extensor and hand grip strength in the elderly group. [Conclusion] The results suggest that age-related reduction in toe flexor strength is greater than those of hand grip and knee extensor strengths. An early loss of toe flexor strength is likely associated with reduced physical activity in elderly men.

[Purpose] There have been concerns that the availability and usage of electrophysical agents have decreased, based on data from cross-sectional surveys. The aim of this study was to conduct the first five-year follow-up longitudinal survey to determine the changes in the availability and usage of electrophysical agents in Nagano Prefecture, Japan. [Participants and Methods] This longitudinal observational study employed the same postal questionnaire survey of practicing clinicians in 2014 and 2019. A total of 22 modalities had been selected for inclusion in the questionnaire based on what is used in clinical facilities and hospitals. [Results] The response rate was 71% and 63% for 2014 and 2019, respectively. The modalities that were high in availability and usage for both 2014 and 2019 were hot packs, ultrasound, cryotherapy and low frequency. While most modalities demonstrated a decreased trend in usage, electrical stimulation devices increased from 2014 to 2019. The results also demonstrated that usage was affected by gender (males greater than females), years of experience (older greater than younger), qualifications (diplomas greater than degrees), and confidence (confident greater than non-confident). [Conclusion] Our results may assist educators with designing educational curricula that is consistent with the needs of clinicians.

[Purpose] The purpose of this study was to investigate the effects of different bridging exercise methods, varied by the position of the lower limbs, on the muscular activity of the neck, trunk, thigh and lower limbs. [Subjects] The subjects of this study were 14 males in their twenties. [Methods] The muscular activities of the longissimus capitis and sternocleidomastoid in the neck, the erector spinae and rectus abdominis in the trunk, the rectus femoris and lateral hamstring in the thigh, and the tibialis anterior and lateral head of the gastrocnemius in the lower leg were measured using a surface electromyography. [Results] The comparison of the muscular activities showed significant differences in the longissimus capitis, rectus abdominis, erector spinae, rectus femoris, biceps femoris and gastrocnemius muscles among the bridging exercise methods. [Conclusion] Action and activity of muscle groups varies according to the position of the lower limbs during the bridging exercise.

[Purpose] The aim of this study was evaluate the effect of a sling exercise on shoulder subluxation in stroke patients. [Subjects] The subjects of this study were thirty-four stroke patients with subluxation. They were randomly selected and divided into two groups. [Methods] The subject group (n=17) performed the sling exercise with traditional therapeutic exercise. The control group (n=17) performed only traditional therapeutic exercise. [Results] The results indicate that improvement of corrected vertical distance (cVD), and the ratio of the oblique distance of the affected and unaffected sides (rOD) was greater in the experimental group than in the control group. [Conclusion] We suggest that the sling exercise is effective at reducing the severity of hemiplegic shoulder subluxation in post-stroke patients.

[Purpose] This study reports the effects of the Muslim prayer, known as Salat, on heart rate (HR) and blood pressure (BP) while performing and miming the actions of Salat: standing, bowing, prostrating and sitting. [Subjects] Thirty Muslim subjects were asked to perform the actual and mime Salat. [Methods] HR and BP were measured using a Schiller AT-102 Electrocardiograph and an Omron SEM-1 Automatic Blood Pressure Monitor. [Results] The findings revealed that there was a significant difference in the HR of the subjects between performing and miming Salat. The standing and prostration positions of Salat produced the highest and the lowest HR, respectively. A lower HR may be of potential benefit to an individual’s health. The systolic and the diastolic BP decreased significantly after performance and mime of Salat, and a greater reduction in BP was observed during performance of Salat. [Conclusion] This is the first study of HR and BP in relation to Salat positions. The findings will encourage further studies to explore the benefits of Salat maneuvers for patients with cardiovascular diseases.

[Purpose] This study investigated the effects of co-contraction resistance exercises of the transverse abdominal and pelvic floor muscles in middle-aged females with stress urinary incontinence. [Participants and Methods] We included 32 females with stress urinary incontinence and divided them into two groups: the inner muscle training group and the pelvic floor muscle group. The thickness of the transverse abdominal muscle was measured during four tasks: (1) rest, (2) maximum contraction of the transverse abdominal muscle, (3) maximum contraction of the pelvic floor muscle, and (4) maximum co-contraction of the transverse abdominal and pelvic floor muscles. In the latter three tasks, measurements were obtained while the participants performed resistance movements using a Thera-band®. A home program was conducted in both groups, and the intervention lasted for 8 weeks. [Results] The cure rates for SUI were 87.5% and 68.8% in the inner muscle training and pelvic floor muscle groups, respectively. After the intervention, the thickness of the transverse abdominal muscle significantly increased in the inner muscle training groups performing maximum co-contraction of the transverse abdominal and pelvic floor muscles and maximum contraction of the transverse abdominal muscle. [Conclusion] Inner muscle training exercises are more effective than pelvic floor muscle exercises in improving inner muscle function and urinary incontinence in middle-aged females.

[Purpose] The purpose of this study was to investigate the effects of scapular stabilization exercise on neck posture, muscle activity, pain, and quality of life in individuals with neck pain and forward head posture. [Subjects and Methods] Fifteen participants were recruited according to the selection criteria and were randomly allocated to the scapular stabilization group (n=8) and the control group (n=7). The scapular stabilization group underwent training for 30 minutes a day, 3 times a week for 4 weeks; the control group performed relaxation exercises for 4 weeks. [Results] After training the scapular stabilization group showed significant improvement on the craniovertebral angle, upper trapezius muscle activity, serratus anterior muscle activity, Neck Disability Index scores, Visual Analog Scale scores, and World Health Organization Quality of Life Assessment-BREF scores compared to those in the control group. [Conclusion] Scapular stabilization exercise can help improve the head posture and pain in the patients with neck pain and forward head posture. Controlling the muscular activities through scapular stabilization exercise also improves the patients’ quality of life.

[Purpose] Isolated finger flexion associated with function of the flexor digitorum superficialis has been qualitatively assessed using standard and modified tests. The purpose of this study was to quantify isolated finger flexion in healthy participants. [Participants and Methods] We assessed 100 volunteers (mean age: 44.6 years) without upper limb dysfunction using the standard and modified flexor digitorum superficialis tests. The sum of the isolated active flexion angles of the metacarpophalangeal and proximal interphalangeal joints of the test finger was also calculated, with the other three fingers held in an extended position with our original jig. [Results] The mean isolated flexion angles were, respectively, 152.4° and 154.8° for the right and left index fingers, 161.1° and 160.4° for the middle fingers, 160.6° and 158.2° for the ring fingers, 129.4° and 134.6° for the independent flexor digitorum superficialis function, 85.8° and 74.7° for the common flexor digitorum superficialis function, and 75.8° and 71.2° for absent flexor digitorum superficialis function in the small finger. The functional variations of the flexor digitorum superficialis of the small fingers showed symmetry in 65.0% of the fingers but asymmetry in 35.0%. [Conclusion] The data obtained in this study provide normal reference values for the examination of independent movement disorders of the fingers.

[Purpose] This study investigates whether cognition, balance and dual task performance in institutionalized older adults improves by a virtual reality dual task training. [Subjects and Methods] Randomized controlled trial; Twenty institutionalized older adults with mild cognitive impairment (13 female, 7 male; average age, 87.2 ± 5.96 years) were randomized to the intervention (i.e. Virtual reality dual-task training using the BioRescue) or control group (no additional training). The intervention group took part in a 6-week training program while the elderly in the control group maintained their daily activities. Balance was measured with the Instrumented Timed Up-and-Go Test with and without a cognitive task. The Observed Emotion Rating Scale and Intrinsic Motivation Inventory were administered to evaluate the emotions and motivation regarding the exergaming program. [Results] The intervention group improved significantly on the total Timed Up-and-Go duration and the turn-to-sit duration during single-task walking in comparison to the control group who received no additional training. Participants found the virtual reality dual task training pleasant and useful for their concentration, memory and balance. Pleasure and alertness were the two emotions which were mostly seen during the intervention. [Conclusion] The BioRescue is a pleasant and interesting treatment method, well suited for institutionalized older adults in need of lifelong physical therapy.

[Purpose] The purpose of this study was to identify factors inhibiting improvement in the quality of life after total knee arthroplasty in patients with rheumatoid arthritis. [Participants and Methods] This was a pilot case-control study. The sample comprised of five participants with rheumatoid arthritis and 11 participants with osteoarthritis, who underwent total knee arthroplasty. We compared the groups in terms of physical function, walking ability, Japanese Knee Osteoarthritis Measure, and Life-Space Assessment. Measurements were taken before surgery and at four weeks and five months post-surgery. All patients underwent rehabilitation for five months postoperatively, first as inpatients, and then as outpatients after discharge. [Results] In the period from 4 weeks to 5 months post-surgery, physical function improved similarly in both groups in terms of muscle strength and walking ability. Despite the patients with rheumatoid arthritis being younger, their self-health assessment score by the Japanese Knee Osteoarthritis Measure and measures of life-space mobility by Life-Space Assessment were lower. [Conclusion] It is important to consider exercise therapy, and gait instruction to alleviate anxiety about health status and improve the quality of life and life-space mobility in patients with rheumatoid arthritis who undergo total knee arthroplasty.

[Purpose] This study applied the lunge exercise using a variety of foot wedge boards to analyze differences in the muscle activities of the vastus lateralis (VL) and vastus medialis oblique (VMO) and the VMO/VL ratios. [Subjects] This study was performed on 20 asymptomatic males. [Methods] Surface electromyography measurements were obtained under 5 lunge exercise conditions. [Result] The EMG activity for the VL muscle significantly increased in the order of conditions as follows: anterior wedge < no wedge and medial wedge < lateral wedge and posterior wedge. The EMG activity for the VMO muscle significantly increased in the order of conditions as follows: anterior wedge < no wedge and lateral wedge < medial wedge and posterior wedge. The VMO/VL ratios under the medial wedge and posterior wedge conditions were significantly increased. [Conclusion] Use of medial and posterior wedge boards during the lunge exercise can selectively strengthen the VMO.

The purpose of this study was to investigate the function of Japanese Geta clogs. We measured the foot pressure and the distance between the Geta and the heel. An F-scan and three-dimensional motion analyses were done. Toe pressure increased in the stance phase at toe-off, and we confirmed that heel-Geta contact is present at the beginning of swing phase. The distance between the Geta and the heel increased from heel-off to toe-off and decreased from toe-off to mid-swing. The results suggest that active toe movement occurs during Geta gait, and the use of Geta clogs may contribute to preventing foot disorders and falls. Geta may be useful for foot and toe strengthening.

医療と福祉分野における理学療法は,障害者に日常生活活動と余暇活動における最大限の自立を促すことを目的にしている。同様に,産業保健での理学療法は職業活動における自立を促すことが目的である。これからは理学療法士が,障害の予防や,真の障害者のリハビリテーションに貢献することが望まれる。すなわち,日常生活活動,余暇活動に加えて,より積極的な職業活動における理学療法の役割を見出すべきである。ただし,わが国において,産業保健領域で働くために必要な資格がある。本稿は,これらの資格に衛生管理者があることを示し,さらに労働衛生コンサルタントがあることも紹介し,産業保健分野において理学療法の実践可能な開業権取得の可能性があることを述べた。これらの受験資格において理学療法士の置かれた立場は,まだ不利な点があり改善すべき課題があることを示した。

[Purpose] The aim of this study was to investigate the recovery process of standing postural control in hemiplegia after stroke. [Subjects and Methods] Thirty-four inpatients with hemiparesis after first-onset stroke were included in this study. We measured the center of pressure fluctuations during quiet standing using a force platform at 2, 4, and 6 weeks after admission. We assessed weight-bearing asymmetry, and velocity and amplitude of body sway. [Results] Weight-bearing asymmetry diminished in the first 2 weeks of observation. Velocity of body sway also decreased significantly in the first 2 weeks, though its amplitude only decreased significantly after 4 weeks of observation. [Conclusion] Amplitude of body sway requires a longer time for significant improvement than weight-bearing asymmetry and velocity of body sway. Although the loading function of the paretic lower limb improved at an early stage, attainment of optimum postural control, including management of the affected paretic lower limb, requires much time.

[Purpose] Adults with cerebral palsy often use a cane as a walking aid because of their decreased gait ability. However, it is unclear whether this affects lower limb muscle activity during walking. The purpose of this study was to clarify the influence of using a cane during walking on the spatio-temporal parameters of walking, lower limb muscle activity, and lower limb muscle coactivation in adults with spastic cerebral palsy. [Participants and Methods] Eleven participants with cerebral palsy were included. The spatio-temporal parameters of walking, lower limb muscle activity, and coactivation of lower limb muscle were measured during a 10 m trial with no cane, one cane, and two canes. [Results] Walking speed was lower and the stride time longer when using two canes than when using no cane. All muscle activities significantly reduced when using two canes. No significant difference was observed between using no cane and one cane, except for walking speed. In addition, there was no significant difference in coactivation between the conditions. [Conclusion] This study revealed that when two canes were used, the walking speed was reduced, and lower limb muscle activity was reduced, reducing the burden. In contrast, the movement pattern was not suggested to have changed.

[Purpose] To develop a quantitative motion analysis software specific to boxing (Fist Tactics Support) and to verify its effectiveness based on the percentage of hits that land on the participants. [Participants and Methods] A total of 24 male professional boxers were divided into two groups: those who used Fist Tactics Support to analyze fight videos and instituted training changes based on the results (Fist Tactics Support group, 12 participants) and those who did not (control group, 12 participants). The overall percentage of hits that landed on the participants in the fights was compared between the two groups and between pre- and post-intervention. [Results] There were no significant differences between the two groups; however, the percentage of hits that landed on the boxers of the Fist Tactics Support group was significantly lower at post-intervention than at pre-intervention. [Conclusion] The use of scientific analysis results in boxing may facilitate the primary prevention of sports injuries.
