新型コロナウイルス感染症 (The novel Coronavirus disease 2019, COVID-19) に対して気管内挿管を行った症例の一部において, 声門後部炎症, 声帯固定, 声門下肉芽, 声門下狭窄, 気管狭窄等の喉頭気管狭窄症を来し, 再挿管または気管切開による気道確保を余儀なくされたという報告が相次いでいる. 海外において同病態は COVID-19 関連喉頭気管狭窄症と呼称されている. 当科ではこれまでに3例の COVID-19 関連喉頭気管狭窄症を経験した. それらの病態は両側声帯固定, 声門後部炎症, 声門下肉芽, 声門下狭窄とさまざまであったが, いずれの症例も治療抵抗性であり, 発症から長期間が経過した現在も気管孔閉鎖には至っていない. COVID-19 関連喉頭気管狭窄症の要因としては, 長期挿管管理, 挿管時の喉頭粘膜損傷, 気管内チューブのサイズ不適合, 挿管管理中の腹臥位療法, 患者側背景として糖尿病, 高血圧, 心疾患等の既往歴, 肥満, 上気道炎の合併などが考えられている. いったん発症すると遷延化し難治性となる場合もある. 従って COVID-19 に対して挿管を必要とする症例においては, 適切な挿管操作, 挿管管理を行うことにより抜管後の喉頭気管狭窄症の発症を予防するだけでなく, 挿管前から上気道炎の有無, 程度の評価を含め, 耳鼻咽喉科による積極的なかかわりが重要であると考えた.

In the Welfare State established in the Western Europe after World War II, one of the fundamental principle is that welfare services must be provided for citizen not as charity but as rights. And it is thought that the development of the Welfare State promote the sense of solidality.After late-1970' under the crisis of the Welfare State, this principle has been criticized by many theorists. And it seems to occur that through these attacks, the essence of social security system change to more selective one. Under these circumstances, it appears to be requested to analyze rights and the idea of solidality in the Welfare State from siciological perspective.In this thesis, to analyze social rights in today's Welfare State we use the concept of distributive rights that U. K. Preuss proposes. So we make it clear that functions and problems of social rights in the Welfare State. Then we view the reconstruction of rights from procedural rights and participate rights. And we review the idea of solidality in the future of the Welfare State.

The Welfare State today is characterized by redistributive policies and positive social regulations. And laws are used as the instrument of the achievement of social policies and social regulations. But these legal regulations seem to fail in social control and rise over-regulation. In the recent discussion this phenomenon i s regarded as “Juridification”. Juridification seems to be an important socio-legal issue of the Welfare State today. In this thesis we refer to the arguement of J. Habermas and G. Teubner, then we make it clear the essence of Juridification and grope for the directions of less repressive regulation in the Welfare State.

問 今年はアテネオリンピックがあり、薄型テレビなどが景気の牽引役になると期待されています。 答 我々は薄型テレビの中でもプラズマテレビに特化しています。まず、プラズマテレビの日本市場だけを見ると、2002年はワールドカップがあって非常に良かった。ところが昨年は、期待していたほどは伸びなかった。別に悪かったわけじゃなくて、あまりにも期待が大きすぎたんですね。
