Recent studies have elucidated the pathophysiology of the common placental disorders. It is believed that gestational hypertension, also known as preeclampsia, is a two-stage disease. Inadequate migration of the trophoblasts induces defective remodeling of the uterine spiral arteries. The poorly perfused and re-perfused placenta (Stage 1) produces antiangiogenic factors that cause maternal endothelial dysfunction (Stage 2). It has been hypothesized that a luck of fetomaternal immune tolerance leads to insufficient trophoblastic differentiation.Another placental disorder is chronic villitis or villitis of unknown etiology. The diagnosis of severe chronic villitis is important to the clinician because it causes intrauterine growth restriction, fetal death, and neonatal neurological impairment. These lesions could be of immune origin. It has been reported that the pathogenesis of chronic placentitis is the maternal anti-fetal cellular and antibody-mediated rejection.Disorders of gestational immune tolerance underlie these two placental diorders.

Concomitant therapy with acetaminophen (APAP) and low-dose aspirin is often used in clinical settings; however, it is unclear whether this combination is involved in the progression of chronic kidney disease (CKD). We hypothesized that concomitant therapy with APAP and low-dose aspirin may cause CKD progression. We carried out a retrospective 6-year cohort study that included all patients who received low-dose aspirin from January 2011 to December 2016 at Kaetsu Hospital. Primary outcome was defined as CKD progression at the end of the study compared with baseline. Among the 441 patients treated during the study period, we identified 89 cases of CKD progression. Multivariate regression analysis showed that exposure to APAP>50 g [odds ratio (OR), 2.68, 95% confidence interval (CI), 1.08-6.70], age increase by 1 year (OR, 1.05, 95% CI, 1.02-1.08), and diabetes mellitus (OR, 2.40, 95% CI, 1.41-4.08) had positive associations with CKD progression. Our findings suggested that concomitant therapy with APAP and low-dose aspirin increased the risk of CKD progression. Therefore, we recommend more thorough monitoring of serum creatinine when patients are on such concomitant therapy. Moreover, it is important to advise users of low-dose aspirin to avoid unnecessary use of APAP, in order to reduce the risk of CKD progression.

近年,青年期におけるひきこもりケースの中に発達障害を背景とするものが少なくないことが明らかになってきており,個々の発達特性や精神・心理状態を踏まえた支援のあり方が問われている.また,ひきこもりに至る以前の予防的な早期支援のあり方を検討することも重要な課題である.本稿では,まず,ひきこもり問題に占める発達障害の割合や,ひきこもり状態にある広汎性発達障害ケースの特徴について述べ,ひきこもりの発現を未然に防ぐことを目的とした予防的早期支援と不登校・ひきこもりケースへの支援における家族支援の要点について検討する.

生命保険販売従事者が直面する倫理的課題についてのアンケート結果をもとに日米比較研究を行ったCooper and Nakabayashi[2010]での,「保険会社が提供する商品種類とサービスが十分でないので,顧客のニーズにあった商品を販売できないことは倫理的に問題かどうか」という問題提起に対し,その後の環境変化を踏まえて一つの答えを出すことが本稿の目的である。<br />日本ではアンケート実施後,銀行窓販が全面解禁になるなど,提供可能な商品やサービスの幅が広がっているという面では問題は解消しつつある。しかし環境が整備されてきたからこそ,それぞれの顧客のニーズに合わせた販売および契約保全活動ができないことは倫理的に問題であるという状況になってきた。このような環境下では,営業職員の自覚はもちろん,生命保険会社が主導して営業職員の質を高め,より包括的で合理的なアドバイザーとしての役割を担えるような体制を整備することが,営業職員チャネルの存在意義を高めることになる。
