一般人の音声障害に関する音声治療については多くの報告があり,高いエビデンスレベルのものもある.しかし歌唱者の音声障害に対する音声治療については国内外で報告は少なく,現時点で確立された手技もない.今回われわれは歌唱者の音声障害に対し音声治療を行い,症状に一定の改善を認めた.対象は声帯結節,声帯瘢痕,声帯萎縮,過緊張性発声障害のいずれかと診断され,音声治療を施行した歌唱者9例(男性5例,女性4例,平均年齢53.3歳)である.口腔前部の共鳴を意識した音声治療を施行し,効果をGRBAS,ストロボスコピー,空気力学的検査,音響分析,自覚的評価,フォルマント周波数解析で評価した.治療後,音声の改善は個人差があるものの全例で認められ,MPTやVHI-10,GRBASで有意差が認められた.また,歌唱フォルマントもより強調されるようになった.歌唱者の音声障害に対する音声治療は一定の効果が期待できると考えられた.

We report herein on a case of minocycline-induced black thyroid with papillary thyroid carcinoma. A 60-year-old female, who had taken oral minocycline intermittently for osteomyelitis of the mandible for two years, noticed a palpable thyroid nodule. Fine needle aspiration of the nodule revealed atypical cells and the serum thyroglobulin level was high. Under the diagnosis of suspected thyroid carcinoma, a hemithyroidectomy with central compartment neck dissection was performed. The color of the tumor, which was diagnosed as a papillary carcinoma, was white. On the other hand, the color of the normal thyroid gland was dark red to black with minocycline pigmentation. Minocycline therapy may induce headache, nausea, vomiting, liver dysfunction and dental hyperpigmentation in children. But a minocycline-induced black thyroid is uncommon. Recently, there have been some reports that the risk of malignancy in black thyroid glands is higher than in non-black thyroids.

We assessed the surgical effects of Isshiki's thyroplasty on 10 functional voice disorder patients including seven with mutational voice disorder, two with spasmodic dysphonia and one with asthenic voice disorder. Our concept is to select and combine proper surgical procedures according to the laryngeal findings of each case. We conveyed thyroplasty type I and III for six patients, and type II and III for one with mutational voice disorder. For spasmodic dysphonia, thyroplasty type II and III was performed in one case with atrophy of the vocal folds accompanied by contraction of false vocal folds during phonation, while type II thyroplasty was selected for the other case without vocal fold atrophy and supraglottic contraction. One asthenic voice disorder was treated with thyroplasty type II and III. We showed the efficiency of framework surgery for functional voice disorders and discussed how to select the type of thyroplasty for each case.

胃食道逆流症(GERD)では、胃酸が食道へ逆流することにより胸やけなどの症状を来すが、音声障害との強い関連が指摘されている。上皮組織は外部の刺激から深層組織を保護する機能的バリアとして働いているが、声帯上皮における接着分子の発現ならびにその役割は十分には分かっていない。本研究では、まず正常ラット声帯上皮において発現しているクローディンのサブタイプを同定してその発現部位を明らかにする。さらに胃酸による上皮バリアの傷害とその修復過程をクローディンの分子発現とバリア機能の両面から解析することで、GERDによる音声障害の発生機序を明らかにし、声帯上皮におけるバリア機構を解明する。

目的: 2004年に日本臨床細胞学会において提唱された, 唾液腺細胞診に関する新報告様式の当科での運用状況, 有用性と問題点を検討すること. 対象と方法: 2006年から2010年までの4年間に, 術前穿刺吸引細胞診と外科的切除を行った44例を対象とし, カルテをレトロスペクティブに調査し, 細胞診の結果, 病理組織診断等を記録した. 結果: 耳下腺原発が33例, 顎下腺原発は11例, 悪性は8例, 良性は36例であった. 良性例のうちの2例は検体不適正であり, また, 良性例のうちの4例と悪性例のうちの1例は「鑑別困難」と判定された. 真陽性は3例, 真陰性は30例で, 偽陰性が4例あり, 偽陽性例はなかった. 感度, 特異度, 正診率はそれぞれ42.9% (4/7), 100% (30/30), 89.2% (34/37) であった. 精度管理指標については, 検体不適正率は4.5% (2/44), 良悪性鑑別困難率は11.4% (5/44), 悪性疑い例の悪性率は100% (2/2) であった. 結論: 新報告様式の使用により, パパニコロウ分類の報告結果に対する臨床医の解釈の曖昧さが減少し, 臨床的に有意義であると考えられた.
