Background:Several studies have reported elevated troponin levels in coronavirus disease 2019 (COVID-19) patients, so we investigated myocardial damage by measuring high-sensitivity troponin T (hsTnT) levels and analyzed the relationship with comorbidities.Methods and Results:Of 209 patients who recently recovered from COVID-19, 65% had an elevated hsTnT level that was higher than levels in patients with acute phase infection despite most patients (79%) having a mild illness. The hsTnT levels correlated with disease severity, sex, comorbidities, and ACEi and ARB use.Conclusions:Myocardial damage occurs in the recovery phase of COVID-19, and its evaluation, regardless of patient age, should be considered.

Since the initial report of coronavirus disease (COVID-19) from the City of Wuhan, China in December 2019, there have been multiple cases globally. Reported here are 11 cases of COVID-19 at this hospital; all of the patients in question presented with relative bradycardia. The severity of the disease was classified into four grades. Of the patients studied, 3 with mild COVID-19 and 3 with moderate COVID-19 improved spontaneously. Lopinavir/ritonavir was administered to 3 patients with severe COVID-19 and 2 with critical COVID-19. Both patients with critical COVID-19 required mechanical ventilation and extracorporeal membrane oxygenation. Both patients with critical COVID-19 had a higher fever that persisted for longer than patients with milder COVID-19. The respiratory status of patients with critical COVID-19 worsened rapidly 7 days after the onset of symptoms. Relative bradycardia may be useful in distinguishing between COVID-19 and bacterial community-acquired pneumonia. In patients who have had a fever for > 7 days, the condition might worsen suddenly.

Earthquake sources represent dynamic rupture within rocky materials at depth and often can be modeled as propagating shear slip controlled by friction laws. These laws provide boundary conditions on fault planes embedded in elastic media. Recent developments in observation networks, laboratory experiments, and methods of data analysis have expanded our knowledge of the physics of earthquakes. Newly discovered slow earthquakes are qualitatively different phenomena from ordinary fast earthquakes and provide independent information on slow deformation at depth. Many numerical simulations have been carried out to model both fast and slow earthquakes, but problems remain, especially with scaling laws. Some mechanisms are required to explain the power-law nature of earthquake rupture and the lack of characteristic length. Conceptual models that include a hierarchical structure over a wide range of scales would be helpful for characterizing diverse behavior in different seismic regions and for improving probabilistic forecasts of earthquakes.

Due to the significant spread of a new type of coronavirus (SARS-CoV-2) infection (COVID-19) in China, the Chinese government blockaded several cities in Hubei Province. Japanese citizens lost a means of transportation to return back to Japan. The National Center for Global Health and Medicine (NCGM) helped the operation of charter flights for evacuation of Japanese residents from Hubei Province, and this article outlines our experiences. A total of five charter flights were dispatched, and the majority of returnees (793/829 [95.7%]) were handled at NCGM. A large number of personnel from various departments participated in this operation; 107 physicians, 115 nurses, 110 clerical staff, and 45 laboratory technicians in total. Several medical translators were also involved. In this operation, we conducted airborne precautions in addition to contact precautions. Eye shields were also used. The doctors collecting the pharyngeal swab used a coverall to minimize the risk of body surface contamination from secretions and droplets. Enhanced hand hygiene using alcohol hand sanitizer was performed. Forty-eight persons were ultimately hospitalized after the triage at NCGM operation, which was more than the number of persons triaged at the airport (n = 34). Of those hospitalized after NCGM triage, 8.3% (4/48 patients) ultimately tested positive for SARS-CoV-2, significantly higher than the positive rate among subjects not triaged (4/48 [8.3%] vs. 9/745 [1.2%]: p = 0.0057). NCGM participated in a large-scale operation to evacuate Japanese nationals from the COVID-19 epidemic area. We were able to establish a scheme through this experience that can be used in the future.

The aim of this study is to investigate myocardial damage in recovering coronavirus disease 2019 (COVID-19) patients with high-sensitivity troponin levels (hsTnT) and echocardiography. In this single-center cohort study, 215 COVID-19 recovered patients were recruited from all over Japan between April and September 2020. Demographic characteristics, hsTnT levels, and echocardiography data were collected for 209 patients, after excluding those without serum samples or good-quality echocardiographic images. The mean (± standard deviation) age was 44 (± 12) years (range: 36-55 years), and 50.7% of the patients were males. The median time interval (interquartile range) from COVID-19 onset to post-recovery examination was 56 days (34-96 days). Seventy-four recovered patients (35.4%) had hsTnT less than detection sensitivity (< 3 pg/mL) and 135 recovered patients (64.6%) had hsTnT ≥ 3 pg/mL. Ejection fraction was more than 50% in all cases. Left ventricular global longitudinal strain (LVGLS) and right ventricular free-wall longitudinal strain (RVFWLS) were reduced in 62 (29.7%) and 8 patients (3.8%), respectively. They were significantly associated with elevated hsTnT levels. In cases with hsTnT above 5 pg/mL, the LVGLS was greatly reduced to 19.0 ± 2.2% (p < 0.001). Multivariate linear regression analysis showed that elevated hsTnT level was an independent predictor of reduced LVGLS (standardized β = -0.34; p < 0.001). In recovered COVID-19 patients, even a slight increase in hsTnT above detection sensitivity was associated with decreased LVGLS. hsTnT and echocardiography may be useful tools to detect myocardial injury in recovered COVID-19 patients.

We report a case of patient in Japan with Coronavirus disease 2019 (COVID-19) with false-negative of reverse transcription polymerase chain reaction for Severe Acute Respiratory Syndrome Coronavirus 2 of pharyngeal swab, from a Chinese traveller returning from Wuhan, Hubei Province, China. If a patient is clinically or epidemiologically suspected of COVID-19, appropriate infection and prevention control measures such as standard, contact, and droplet precaution are needed until the patient is proven to be true-negative.

Objective In fever clinics screening coronavirus disease (COVID-19), there could be patients with life-threatening diseases that physicians should not overlook. We exploratorily investigated the final diagnosis among non-COVID-19 hospitalized patients who visited the fever clinic. Methods This was a retrospective, observational, and single-centered study conducted in the National Center for Global Health and Medicine (NCGM), Tokyo, Japan. We conducted a retrospective chart review of patients who visited the fever clinic in the NCGM from 11 March 2020 to 24 April 2020. Patients Patients who met the following clinical criteria visited the fever clinic in the NCGM: (1) body temperature >37.5°C, (2) any symptoms consistent with COVID-19 or (3) referral from local healthcare facilities. In the fever clinic, all patients who met the above criteria had severe acute respiratory syndrome coronavirus 2 polymerase chain reaction test with nasopharyngeal swab specimens. Patients with severe symptoms or an unstable condition were sent to an outpatient clinic for infectious diseases for further evaluation and treatment. Results Among 1,470 patients who visited the fever clinic, 84 patients were hospitalized, and 45 of them were diagnosed as having COVID-19. Among the remaining 39 non-COVID-19 patients, there were nine patients with life-threatening diseases. The life-threatening diseases included acute heart failure, septic shock, pneumocystis pneumonia, peritonsillar abscess, and necrotizing fasciitis. Conclusion Physicians should evaluate each patient carefully while considering other life-threatening conditions even in such a COVID-19 pandemic era.

保健・看護系の大学院生が、効率的に必要十分な統計学的知識の定着をはかり、データ解析ができるための学習支援のプログラムの開発に向けて、本学の保健・看護系修士課程大学院生における、①統計解析の学習に関する意向とニーズを明らかにすること、②統計解析スキル向上に向けた演習を構築しその評価をすること、③良く質問され、かつ研究遂行上重要なQ&Aを探索し整備すること、の3点を目的とした。 目的①に対しては一定の統計解析を行って修士論文を作成した本学保健・看護系大学院生・卒業生13名を対象とした自記式質問紙ないし構造化面接調査を実施した。また、目的②に対しては極力わかりやすい解説の元、論文の結果表を読み取り、自身の研究データ解析に活用できる授業の構築、ならびに、参加者が自分の研究データを扱っている感覚でデモデータを分析する演習の構築を行い、終了後に感想を聞くとともに、目的①の質問紙調査において感想を聞いた。目的③については、新たに専用の統計相談窓口を設置し、統計解析に関する相談を受け付けることを通じて、どのような質問が寄せられるかを整理した。 修士論文作成に使用した統計解析ソフトウエアはR/Rコマンダーが6名、SPSSが5名、Excel統計が4名であった。統計解析方法については、教員からの指導に依存し、補足的に自学自習をしているスタイルであった。事例が豊富な教材を期待する声が大きかった。講義、演習については、概ね良好に受け入れられたが、回数が限られており分量が多く、スピードが速いといった指摘が見られた。統計相談の内容の傾向としては、量的変数として扱ってよい場合とそうでない場合、必要なサンプルサイズについて多く寄せられていた。 統計解析に関する知識を概観し、自己学習のきっかけをつくるうえでの講義授業は重要であることが伺われた。同様に自主演習をすすめるきっかけとしての演習授業も重要であることが伺われた。

本研究の目的は、高齢者に対する教育的な介入プログラムである「物忘れ予防教室」を、地域介護支援センター職員がそのマニュアルに沿って展開し、高齢者の記憶や自己効力感に対するプログラムの影響を追試すると共に、地域における介護予防活動としてプログラムを展開していくことの可能性を模索することである。対象者は、物忘れ予防教室に参加した者のうち、調査協力が得られ、かつ教室前後でのデータを得ることができた男女16名である。結果から、すべての測定ツールにおいて介入後のプラス変化が見られ、特にメタ記憶の「記憶の補助方法」(p=0.039)と「記憶の自己効力感」(p=0.039)、および抑うつ感(p=0.050)に関して有意な変化が見られた。このことから、物忘れ予防教室によって対象者は有意に記憶に関する自己効力感を高め、また記憶の補助方法の活用頻度を高めていることが明らかとなった。支援センター職員が中心となって行われた「物忘れ予防教室」も、マニュアルに沿って介入を行うとで、参加者の記憶に対する認識や記憶の効力感を高め、高齢者をエンパワーしていく働きかけとなることが示唆され、地域における活動として展開していくことの有用性が確認されたといえる。Purpose of this study was to test the memory training program for its efficiency and applicability as a preventive service for elderly providing at a care support center in community. The subjects for this study were elder participants living in community who agreed to join this research. The result showed significant increase in metamemory (p=0.039) and memory self efficacy (p=0.039). This result suggested that the memory training program was useful as a program aimed at preventing elderly people from requiring nursing care and further losing physical and mental strength.
