A hand hygiene behavior questionnaire and environmental survey were conducted regarding the mobile X-ray system used in the emergency room. As a result, among a total of 22 radiological technologists at this hospital who replied to the questionnaire, 18 wore disposable gloves when performing X-ray imaging using the mobile system. Among those 18, 11 were found to touch computed radiology (CR) consoles and HIS/RIS terminals while still wearing the gloves, thus creating the potential for spreading pathogens to other medical equipment and systems. According to the results of an environmental survey of the emergency imaging preparation room, the highest levels of bacteria were detected on CR consoles and HIS/RIS terminals. A possible reason for this is that these locations are not wiped down and cleaned as a part of routine cleaning and disinfection protocols, thus demonstrating the importance of cleaning and disinfection. Hand hygiene by medical personnel and appropriate cleaning and disinfecting of the working environment are important for preventing the spread of nosocomial infections. Radiological technologists are also required to take effective measures against infections in consideration of the high frequency of contact with both infected patients and patients susceptible to infections.

In recent years, workload has increased with higher precision of radiotherapy. Although both efficiency and thoroughness of treatment are crucial, in such conditions, human error is easy to occur. In this study, five incident cases that occurred in four facilities were studied and analyzed from the viewpoint of human factors that contribute to errors using variation tree analysis. We also analyzed resilience (the ability to return to one’s original state even if the system deviates from a stable state), which has attracted attention in recent safety research. There were potential factors represented by patient factors in all cases. These factors caused deviations from standard operations, and incidents occurred due to unfamiliar situations and operations. Furthermore, in four of the five cases, the cause of the incident was a resilience action or judgment that was deemed to have required “some sort of ingenuity or adjustment.” It was found that human error occurred due to multiple simultaneous occurrences of potential factors, i.e., patient and human factors such as high workload, impatience, and work interruptions. A reduction in human errors can be achieved by avoiding time pressure and multitasking, creating work environment and working conditions that make resilience work well, revising ambiguous rules and procedures, and promoting standardized working methods.

The objectives of this study were to determine the usefulness of a newly-developed distal fibular axial view radiography modified method for depicting avulsion fracture of the lateral malleolus in children from a functional anatomy viewpoint. Conventional radiography was applied to sixty-nine avulsion fracture suspected ankles of 67 children. Average age and standard deviation at injury were 8.00 and 1.46, respectively. We compared the ability of the modified method to detect avulsion fractures of the lateral malleolus with those could not be depicted using the conventional method, and noted that 42 avulsion fractures (60.9%) could be depicted using the conventional method. We applied the modified method to 27 joints that the conventional method had diagnosed as normal. Of these, the modified method detected 13 avulsion fractures (48.1%). In conclusion, the modified radiography method made it possible to depict avulsion fracture of the lateral malleolus in cases that resisted detection by the conventional method.

In this article, we present a physical characterization of the agility™ (Elekta). agility™ is composed of 160 interdigitating multileaf collimators (MLCs) with a width of 5 mm at the isocenter. The physical characterizations that include leaf position accuracy, leakage, field penumbra and the tongue-and-groove (T&G) effect were evaluated using well-commissioned 4, 6 and 10-MV photon beams. The leaf position accuracy was within 0.5 mm for all gantry angles and each MLC. The leakage was 0.44% on average and reached 0.47% at 10 MV: remarkably low due to a new design with tilted leaves. However, the T&G effect occurred due to tilt. It was approximately 20.8% on average and reached 22.3% at 6 MV. The penumbra width increased up to 8.5 mm at a field size of 20×20 cm at 4 MV. High position designed MLCs create a wider penumbra but show lower leakage and large head clearance. Head clearance is an important factor in stereotactic radiotherapy with multiple non-coplanar beams.

<p>Capillaries are the most basic and important blood vessel of the circulatory systems. The evaluation of the blood flow may contribute to many studies in future. We evaluated the capillary blood flow change of lower limb muscle over time before and after the exercise used by magnetic resonance imaging-intravoxel incoherent motion (MRI-IVIM) obtained perfusion information. Furthermore, we examined an association between the muscle pain after the exercise and the diffusion weighted image (DWI) indexes. DWI was imaged using multi-b values for a thigh and calf muscles. MRI was performed just after an exercise test, 3, 6, and 24 hours later, and the IVIM index and diffusion index were calculated. Furthermore, we interviewed the degree of the muscle ache 24 hours later. As a result, pseudo diffusion coefficient (D*) and f value as IVIM index increased after-exercise as compared with pre-exercise and decreased in 3 hours later. A similar tendency was found in the apparent diffusion coefficient and the diffusion coefficient as diffusion index. Furthermore, all indexes increased in after exercise from before exercise and decreased with time passed and increased again 24 hours later. In conclusion, IVIM could obtain capillary blood flow information, and it was suggested to contribute for sports medicine in future.</p>

In recent years, as gradient magnetic resonance imaging (MRI)systems have become larger and faster, the influence of low-frequency alternating magnetic fields on the human body during rapid scanning have become significant in terms of patient health. It has become more important to monitor time-variant magnetic fields (dB/dt). From this point of view, we measured the maximum dB/dt for clinical scanning using a self-made search coil and storage oscilloscope that conformed to international standard IEC 60601-2-33(1995). The results were almost the same as the calculated values. In this brief report, we introduce to clinical operators the method of measuring maximum dB/dt.

医療に向けられる社会の評価はかつてなく厳しいものになっており,その厳しさを示す具体的なものとして,医療事故をめぐる紛争,訴訟の増加が挙げられる.また日常の診療においては,患者への危害のみならず,物損や患者からの苦情など病院のイメージダウンを引き起こす要因が潜んでいる.これらも医療事故,紛争に直結するおそれがあり,病院の経済的損失につながる.したがって,自分の職場ではどのようなリスクが存在しているのか,また実際に起こっているかを調査し,その内容を把握することが重要である.これらのことを受け,本報ではアンケート調査を実施し,放射線業務において発生している患者からの苦情・質問についてモダリティ別に事例集としてまとめたので報告する.
